Rotator cuff repair is a widely performed surgery. Its purpose is to improve shoulder function, fix tendons to bones, restore anatomical structure, and prevent the progression of rotator cuff tear arthropathy and associated muscle degeneration. However, in large and massive tears, the tension imposed during repair becomes too high.

How tumor-associated macrophages transit from a predominant antitumor M1-like phenotype to a protumoral M2-like phenotype during the development of pancreatic ductal adenocarcinoma (PDA) remains to be elucidated. We thus conducted a study by employing a PDA-macrophage co-culture system, an "orthotopic" PDA syngeneic mouse model, and human PDA specimens, together with macrophages derived from GARP knockout mice and multiple analytic tools including whole-genome RNA sequencing, DNA methylation arrays, multiplex immunohistochemistry, metabolism measurement, and invasion/metastasis assessment. Our study showed that PDA tumor cells, through direct cell-cell contact, induce DNA methylation and downregulation of a panel of glucose metabolism and OXPHOS genes selectively in M1-like macrophages, leading to a suppressed glucose metabolic status in M1-like but not in M2-like macrophages.

Article Synopsis
  • A study examined osteoporosis medication usage in 497 patients over 60 years with fragility hip fractures (FHF), revealing that only 25.5% maintained continuous medication for three years, while 44.1% received no treatment.
  • It was found that a low Barthel index at discharge was a significant risk factor for both not starting treatment and not remaining on medication.
  • The findings suggest addressing the issues around low Barthel index could improve treatment persistence and overall outcomes for patients at risk of osteoporosis-related fractures.

Background: The preoperative distinction between uncomplicated and complicated appendicitis is important to determine the appropriate treatments, such as antibiotics, surgery, or interval appendectomy. Computed tomography (CT) plays an important role; however, combining clinical and imaging factors may make preoperative evaluation more reliable. This study evaluated and analyzed cases and the usefulness of several preoperative factors and clinical scoring models to detect complicated appendicitis.

Background/aim: Neoantigens are tumor-specific antigens that emerge due to gene mutations in tumor cells, and are highly antigenic epitopes that escape central immune tolerance in the thymus, making cancer vaccine therapy a desirable option.

Patients And Methods: Tumor neoantigens were predicted in 17 patients with advanced cancer. They were resistant to the standard treatment regime, and their synthetic peptides were pulsed to the patient's monocyte-derived dendritic cells (DCs), and administered to the patient's lymph nodes via ultrasound.

Article Synopsis
  • Researchers aimed to identify the optimal intragastric pressure (IP) needed for effective gastric wall extension during endoscopy, which was previously unclear.
  • An in vitro experiment used a specially equipped endoscope to accurately measure IP by connecting a pressure device directly to the forceps channel, while in vivo measurements were taken from 51 patients during the procedure.
  • Results showed that a mean IP of 14.7 mmHg was recorded, and strong gastric wall extension was achieved in 96.1% of cases at an IP of 20 mmHg, confirming the new measurement method's effectiveness.
