October 2021

Department of Morphology, Surgery and Experimental Medicine, Section of Anaesthesia and Intensive Care, University of Ferrara, Azienda Ospedaliero-Universitaria S. Anna, Via Aldo Moro, 8, 44121, Cona, Ferrara, Italy.

Purpose: The onset of the coronavirus disease 19 (COVID-19) pandemic in Italy induced a dramatic increase in the need for intensive care unit (ICU) beds for a large proportion of patients affected by COVID-19-related acute respiratory distress syndrome (ARDS). The aim of the present study was to describe the health-related quality of life (HRQoL) at 90 days after ICU discharge in a cohort of COVID-19 patients undergoing invasive mechanical ventilation and to compare it with an age and sex-matched sample from the general Italian and Finnish populations. Moreover, the possible associations between clinical, demographic, social factors, and HRQoL were investigated.

Background: Prone positioning (PP) has been used to improve oxygenation in patients affected by the SARS-CoV-2 disease (COVID-19). Several mechanisms, including lung recruitment and better lung ventilation/perfusion matching, make a relevant rational for using PP. However, not all patients maintain the oxygenation improvement after returning to supine position.

Article Synopsis
  • The Alport Variant Collaborative's recent meeting expanded the criteria for screening certain gene mutations linked to Alport syndrome, allowing for broader clinical indications like persistent proteinuria and familial kidney issues.
  • They updated the ACMG guidelines for assessing gene variants related to Alport, including identifying specific mutation hotspots in collagen IV chains and noting the limitations of current functional assays.
  • The complexity of Alport syndrome's inheritance patterns prevents establishing a clear threshold for benign variants, and interpreting less severe mutations (hypomorphic variants) in related genes remains difficult, with some variants frequently appearing in normal databases.

COVID-19 pandemic era: is it time to promote home dialysis and peritoneal dialysis?

Clin Kidney J

March 2021

Department of Experimental Diagnostic and Specialty Medicine (DIMES), Nephrology, Dialysis and Renal Transplant Unit, S. Orsola Hospital, University of Bologna, Bologna, Italy.

The novel coronavirus, called severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2), was declared a pandemic in March 2020 by the World Health Organization. Older individuals and patients with comorbid conditions such as hypertension, heart disease, diabetes, lung disease, chronic kidney disease (CKD) and immunologic diseases are at higher risk of contracting this severe infection. In particular, patients with advanced CKD constitute a vulnerable population and a challenge in the prevention and control of the disease.

Longest reported support (7.5 years) with postauricular type of Jarvik 2000 axial-flow left ventricular assist device.

J Artif Organs

December 2021

Cardiac Surgery Unit, Cardio Thoracic and Vascular Department, S. Orsola Hospital, University of Bologna, Via Massarenti n.9, 40138, Bologna, Italy.

Mechanical circulatory supports with left ventricular assist devices (LVAD) are nowadays an established treatment in end-stage heart failure for those patients who are waiting for an organ donation or are unsuitable for transplantation. The duration of LVAD support is variable, depending on the device, the intention to treat and the issues occurring during treatment, which can change the purpose treatment or accelerate the transplantation. Moreover, length of reported supports in the literature is heterogenous.

Diagnostic Accuracy of North America Expert Consensus Statement on Reporting CT Findings in Patients Suspected of Having COVID-19 Infection: An Italian Single-Center Experience.

Radiol Cardiothorac Imaging

August 2020

Radiology Unit, Department of Experimental, Diagnostic and Speciality Medicine (DIMES), S. Orsola Hospital, University of Bologna, 15 Albertoni St, 40138 Bologna, Italy.

Purpose: To evaluate the diagnostic accuracy of the four standardized categories for CT reporting proposed by the Radiological Society of North America (RSNA) to support a faster triage compared with real-time reverse-transcription polymerase chain reaction (RT-PCR), which is the reference standard for suspected coronavirus disease 2019 (COVID-19), but has long reporting time (6-48 hours).

Materials And Methods: A retrospective analysis of 569 thin-section CT examinations performed for patients suspected of having COVID-19 from February 27 to March 27, 2020 (peak of infection in Italy) was conducted. The imaging pattern was classified according to the statement by the RSNA as "typical," "indeterminate," "atypical," and "negative" and compared with RT-PCR for 460 patients.

Cure rate following standard first-line regimens for Helicobacter pylori eradication is decreasing so several patients require two or more treatments. Antibiotic susceptibility-based therapy, advised in current guidelines, is largely impracticable in clinical practice. Some 'standard' regimens (triple therapies based on either levofloxacin or rifabutin, bismuth-based quadruple therapies, sequential, concomitant and hybrid therapies) were empirically used as rescue therapies.

Background: Polymerase-ε (POLE)-mutated endometrial carcinomas (ECs) have displayed an increased number of tumor-infiltrating lymphocytes (TIL) compared to POLE-wild-type ECs. However, it is unclear if TIL may aid in identifying POLE-mutated ECs when molecular data are unavailable. The identification of a POLE mutation surrogate may be crucial to translate TCGA/ProMisE risk assessment in the clinical practice.

Ventricular assist devices implantation: surgical assessment and technical strategies.

Cardiovasc Diagn Ther

February 2021

Cardio-Thorac-Vascular Department, Cardiac Surgery Unit, S. Orsola Hospital, Bologna University, Bologna, Italy.

Along with the worldwide increase in continuous left ventricular assist device (LVAD) strategy adoption, more and more patients with demanding anatomical and clinical features are currently referred to heart failure (HF) departments for treatment. Thus surgeons have to deal, technically, with re-entry due to previous cardiac surgery procedures, porcelain aorta, peripheral vascular arterial disease, concomitant valvular or septal disease, biventricular failure. New surgical techniques and surgical tools have been developed to offer acceptable postoperative outcomes to all mechanical circulatory support recipients.

Article Synopsis
  • The adoption of rice as a staple in many Asian diets has significantly impacted nutritional habits and created challenges in managing blood sugar levels due to rice's high carbohydrate content and glycemic index.
  • Epidemiological data shows differences in type 2 diabetes and obesity rates among Asian populations, with South Asians experiencing higher prevalence than East Asians, potentially due to historical dietary adaptations.
  • Genetic analysis of various East and South Asian populations suggests that those with a long history of rice or millet consumption may have developed biological adaptations to mitigate the negative effects of high carbohydrate diets.

Axillary vein access for antiarrhythmic cardiac device implantation: a literature review.

J Cardiovasc Med (Hagerstown)

April 2021

Cardiology Division, Ospedale Provinciale AREA VASTA 3, Macerata, Italy.

The current narrative review provides an update of available knowledge on venous access techniques for cardiac implantable electronic device implantation, with a focus on axillary vein puncture. Lower procedure-related and lead-related complications have been reported with extrathoracic vein puncture techniques compared with intrathoracic accesses. In particular, extrathoracic lead access through the axillary vein seems to be associated with lower complication incidence than subclavian vein puncture and higher success rate than cephalic vein cutdown.
