Article Synopsis
  • - Liver resections are vital for treating liver cancers, but many patients aren't resectable due to the risk of post-hepatectomy liver failure, as only about 70% of liver tissue can be safely removed.
  • - Regenerative liver surgery techniques, like portal vein embolization (PVE) and the newer Associating Liver Partition and Portal vein ligation for Staged hepatectomy (ALPPS), have evolved to increase the amount of healthy liver left after surgery, improving resection rates.
  • - Despite the promise of accelerated liver regeneration from methods like ALPPS, challenges remain, including a high mortality rate and the need for careful patient selection, prompting a plethora of modifications to enhance safety and effectiveness.

Article Synopsis
  • Diabetic foot ulcers (DFUs) are costly to manage and often don’t heal, with links found between skin microbes, blood sugar control, and Vitamin C levels.
  • A study compared the skin microbiome of DFUs, diabetic skin, and nondiabetic skin to see how these factors relate to healing.
  • Results showed bacterial loads were significantly higher in DFUs, but no major differences in microbial structures were linked to blood sugar or Vitamin C levels, although uncontrolled blood sugar correlated with larger wounds and lower Vitamin C levels.

Background: Falls are inherent to Parkinson's disease (PD) progression, and risk assessment is mandatory for optimal long term management.

Objective: To determine if the telehealth application of two observer-based, objective measures of fall-risk in PD- (FTSTS) and (RTT)-is feasible and safe.

Methods: Following in-clinic training, 15 people with Hoehn and Yahr Stage 2 (n = 8) and 3 (n = 7) PD, median MoCA score 25 (range 14-29), and subjective freezing-of-gait (n = 13), participated in four televisits with care partners biweekly for 10 weeks where virtual FTSTS/RTT assessments were performed.

Background Venous thromboembolism (VTE) contributes significantly to COVID-19 morbidity and mortality. The urokinase receptor system is involved in the regulation of coagulation. Levels of soluble urokinase plasminogen activator receptor (suPAR) reflect hyperinflammation and are strongly predictive of outcomes in COVID-19.

Background Evidence from randomized trials and updated professional society guidelines supports patent foramen ovale (PFO) closure after cryptogenic stroke in select patients. It is unclear how this has been integrated into real-world practice, so we aimed to compare practice patterns between cardiologists and neurologists. Methods and Results In March of 2021, a survey of cardiologists and neurologists who work or previously trained at the University of Pennsylvania Health System assessed practice preferences with respect to PFO closure after stroke.

Introduction: Development of novel diagnostic tools is a top research priority in vascular dementia. A major obstacle is the lack of a simple, non-invasive method to visualize cerebral arteriolar walls in vivo. Retinal arterioles offer a window into the cerebral circulation.

Department of Hematological Laboratory Science, Jiangsu Key Laboratory of Medical Science and Laboratory Medicine, School of Medicine, Jiangsu University Zhenjiang 212013, Jiangsu, China.

Circular RNAs (circRNAs), a class of non-coding RNAs, play an essential role in embryo development and carcinogenesis, circNRIP1 was recently identified to promote development of multiple human cancers. This study investigated the role of circNRIP1 in osteosarcoma (OS) cells and the potential mechanisms relating to the sponging miRNAs and their target genes. OS cell lines and normal human osteoblasts were grown for qRT-PCR analysis of circNRIP1 expression and functions of circNRIP1 expression in OS cell proliferation, migration, and invasion .

Article Synopsis
  • The DRAGON 1 trial investigates the training, implementation, safety, and feasibility of a combined portal- and hepatic-vein embolization (PVE/HVE) approach to encourage liver growth in patients with borderline resectable colorectal cancer liver metastases.
  • The study is a worldwide multicenter, single-arm trial, with primary endpoints focusing on the safety of the procedure and 90-day mortality, while secondary endpoints look at resection feasibility, liver function, and overall survival rates.
  • The findings aim to standardize procedures for future trials and enhance understanding of the safety and effects of PVE/HVE on liver growth in this patient population.

Background: Persistent atrial fibrillation (AF) is associated with high recurrence rates of AF and atypical atrial flutters or tachycardia (AFT) postablation. Laser balloon (LB) ablation of the pulmonary vein (PV) ostia has similar efficacy as radiofrequency wide area circumferential ablation (RF-WACA); however, an approach of LB wide area circumferential ablation (LB-WACA) may further improve success rates.

Objective: To evaluate freedom from atrial tachyarrhythmia (AFT/AF) recurrence postablation using RF-WACA versus LB-WACA in persistent AF patients.

Program evaluation is an "essential responsibility" but is often not seen as a scholarly pursuit. While Boyer expanded what qualifies as educational scholarship, many still need to engage in processes that are rigorous and of a requisite academic standard to be labelled as scholarly. Many medical educators may feel that scholarly program evaluation is a daunting task due to the competing interests of curricular change, remediation, and clinical care.

Background: The use of autologous free-tissue transfer is an increasingly utilized tool in the ladder of reconstructive options to preserve and restore function in patients with head and neck cancer. This article focuses on the evidence surrounding perioperative care that optimizes surgical outcomes and describes one tertiary center's approach to standardized free-flap care.

Data Sources: This article examines English literature from PubMed and offers expert opinion on perioperative free-flap care for head and neck oncology.

Introduction: In addition to formal training, informal training often occurs through a hidden curriculum. As the hidden curriculum shapes the knowledge and values held by learners, we must consider its role in implicit bias. One example is through the selection of images used in formal instruction.

  • Bifurcation lesions are complex challenges in coronary interventions, making up 20% of cases and often leading to lower success rates and higher restenosis.
  • A meta-analysis of 29 clinical trials involving 8318 patients found no significant differences in major outcomes between 2-stent techniques and provisional stenting overall.
  • The double kissing crush technique showed substantial advantages over provisional stenting and other 2-stent methods, resulting in reduced risks for cardiac death and other adverse events.

Objectives: The Standardized Letter of Evaluation (SLOE) is a vital portion of any medical student's emergency medicine (EM) residency application. Prior literature suggests gender bias in EM SLOE comparative ranking, but there is limited understanding of the impact of gender on other SLOE components. The study objective was to evaluate the presence of gender differences in the 7 Qualifications for EM (7QEM), Global Assessment (GA), and anticipated Rank List (RL) position.

Higher vitamin K intakes have been associated with better cognitive function, suggestive of a vitamin K mechanistic effect or simply reflective of a healthy diet. To test the hypothesis that brain vitamin K is linked to cognitive decline and dementia, vitamin K concentrations were measured in four brain regions, and their associations with cognitive and neuropathological outcomes were estimated in 325 decedents of the Rush Memory and Aging Project. Menaquinone-4 (MK4) was the main vitamin K form in the brain regions evaluated.
