Background: The application of massively parallel sequencing has led to the identification of aberrant druggable pathways and somatic mutations within therapeutically relevant genes in gastro-oesophageal cancer. Given the widespread use of formalin-fixed paraffin-embedded (FFPE) samples in the study of this disease, it would be beneficial, especially for the purposes of biomarker evaluation, to assess the concordance between comprehensive exome-wide sequencing data from archival FFPE samples originating from a prospective clinical study and those derived from fresh-frozen material.

Bridging the Age Gap in breast cancer: Impact of chemotherapy on quality of life in older women with early breast cancer.

Eur J Cancer

February 2021

Department of Medicine, Breast Unit the Royal Marsden Hospital NHS Foundation Trust, London, UK; Breast Cancer Research Division, The Institute of Cancer Research, London, UK.

Introduction: Older patients with early breast cancer (EBC) derive modest survival benefit from chemotherapy but have increased toxicity risk. Data on the impact of chemotherapy for EBC on quality of life in older patients are limited, but this is a key determinant of treatment acceptance. We aimed to investigate its effect on quality of life in older patients enrolled in the Bridging the Age Gap study.

Therapeutic options for treating advanced renal cell cancer (RCC) are rapidly evolving. Vascular endothelial growth factor (VEGF)-directed therapy, predominantly VEGF receptor (VEGFr) tyrosine kinase inhibitors (TKIs) had been the most effective first line treatment since 2005 irrespective of International Metastatic RCC Database Consortium (IMDC) risk stratification. However, immune checkpoint inhibitors (ICI) have recently changed the treatment paradigm for advanced RCC particularly as the first-line systemic treatment modality.

Introduction: Stereotactic body radiotherapy (SBRT) is an established ablative treatment for liver tumors with excellent local control rates. Magnetic resonance imaging guided radiotherapy (MRgRT) provides superior soft tissue contrast and may therefore facilitate a marker-less liver SBRT workflow. The goal of the present study was to investigate feasibility, workflow parameters, toxicity and patient acceptance of MRgSBRT on a 1.

Neuroablative surgical treatments for pain due to cancer.

Neurochirurgie

April 2021

Nuffield Department of Clinical Sciences, John-Radcliffe Hospital, OX3 9DU Oxford, United Kingdom. Electronic address:

Cancer pain is common and challenging to manage - it is estimated that approximately 30% of cancer patients have pain that is not adequately controlled by analgesia. This paper discusses safe and effective neuroablative treatment options for refractory cancer pain. Current management of cancer pain predominantly focuses on the use of medications, resulting in a relative loss of knowledge of these surgical techniques and the erosion of the skills required to perform them.

Image-guided Radiotherapy to Manage Respiratory Motion: Lung and Liver.

Clin Oncol (R Coll Radiol)

December 2020

The Royal Marsden NHS Foundation Trust, Sutton, UK; Division of Medical Radiation Physics, Department of Radiation Oncology, Inselspital, Bern University Hospital, University of Bern, Bern, Switzerland; Joint Department of Physics, The Institute of Cancer Research, The Royal Marsden Hospital NHS Foundation Trust, London, UK. Electronic address:

Organ motion as a result of respiratory and cardiac motion poses significant challenges for the accurate delivery of radiotherapy to both the thorax and the upper abdomen. Modern imaging techniques during radiotherapy simulation and delivery now permit better quantification of organ motion, which in turn reduces tumour and organ at risk position uncertainty. These imaging advances, coupled with respiratory correlated radiotherapy delivery techniques, have led to the development of a range of approaches to manage respiratory motion.

Article Synopsis
  • - The study explored the effectiveness and safety of using a combination of CDK4/6 inhibitor (palbociclib) and PI3K inhibitor (taselisib), along with the hormone therapy (fulvestrant), in treating advanced ER-positive HER2-negative breast cancer with specific genetic mutations.
  • - Results showed that the triplet therapy led to a 37.5% response rate in the targeted patient group, while both doublet and triplet therapies were well tolerated and provided durable disease control.
  • - High levels of cyclin E1 and changes in circulating tumor DNA (ctDNA) were linked to shorter progression-free survival, indicating that monitoring these factors could help refine treatment strategies for breast cancer patients.

Article Synopsis
  • - Chromosomal instability in cancer leads to significant changes in chromosome number and structure, allowing for diverse somatic copy number alterations (SCNAs) that drive tumor evolution across various cancer types.
  • - An analysis of over 1,400 tumor samples revealed that 37% showed parallel evolutionary events affecting the same key genes, with most recurrent chromosomal losses happening before the whole-genome doubling event.
  • - Furthermore, certain SCNAs were found frequently in metastatic samples, indicating that chromosomal instability facilitates ongoing genetic changes that aid in the progression and diversity of tumors.
