Article Synopsis
  • Tenecteplase (TNK) is an effective thrombolytic treatment for acute ischemic stroke within 4.5 hours, but its efficacy after this window is less clear, particularly for patients with large vessel occlusions who cannot undergo thrombectomy.
  • The TRACE-III trial involved 516 patients with large vessel occlusion and assessed the safety and effectiveness of TNK compared to standard medical treatment, focusing on outcomes like disability at 90 days and mortality rates.
  • Results showed that TNK led to a higher percentage of patients with less disability after 90 days, similar mortality rates to standard treatment, but also a greater risk of symptomatic intracranial hemorrhage within the first 36 hours.

Purpose: Specimen Mammography (SM) is commonly used in Breast Conserving Surgery (BCS) for intraoperative margin analysis. A systematic scoping review was conducted to identify sources of methodological variation in Specimen Mammography Interpretation (SMI) and assess the role of Artificial Intelligence (AI) techniques to optimise Diagnostic Accuracy (DA).

Methods: Embase, Pubmed, Cochrane and web of science databases were searched.

Introduction: Periprosthetic femoral fractures (PPFFs) represent an important healthcare problem, with a rising incidence noted due to an increase in the number of arthroplasty surgeries being performed. There is a current lack of national consensus as to how these complex, often frail patients are managed.

Aims: Our primary aim was to present the epidemiology of PPFFs in England and Wales over the first two years of data collection by the National Hip Fracture Database (NHFD).

Article Synopsis
  • This study investigates the role of inversions—structural variants that involve the rearrangement of DNA—in genetic diseases, using data from 33,924 families involved in the 100,000 Genomes Project.
  • Researchers identified 47 ultra-rare rearrangements, including de novo inversions, in genes linked to disease, with analyses correlating genetic findings to clinical outcomes in some cases, including a specific diagnosis for three family members.
  • The findings suggest that while inversions are less common in genetic diseases compared to other structural variants, they can significantly contribute to the etiology in approximately 1 in 750 families with rare conditions.

Phenotypic effect of GBA1 variants in individuals with and without Parkinson's disease: The RAPSODI study.

Neurobiol Dis

November 2023

Department of Clinical and Movement Neurosciences, UCL Queen Square Institute of Neurology, London, UK; Aligning Science Across Parkinson's (ASAP) Collaborative Research Network, Chevy Chase, MD, USA. Electronic address:

Article Synopsis
  • GBA1 gene variants are linked to Gaucher disease and increase the risk for Parkinson's disease, but not all carriers develop PD, making it important to identify higher-risk individuals.
  • The study compared motor and non-motor symptoms in GBA1 variant carriers and non-carriers using standardized assessments and included data from additional cohorts.
  • Results showed GBA1-positive Parkinson's patients had worse cognitive and olfactory performance, while no significant differences were found between non-affected GBA1 carriers and controls, pointing to a distinct phenotype in GBA1-positive PD without confirming earlier reports of prodromal PD signs in carriers.

Background: Little is known about employment following revision total knee arthroplasty (RTKA). This study aims to describe factors associated with returning to employment in patients of working age who underwent RTKA surgery.

Methods: We performed a retrospective assessment of all patients aged ≤65 who underwent RTKA at our NHS institution between 2006 and 2020.

Article Synopsis
  • * A study analyzed data from 174 CF patients with severe lung impairment (FEV ≤ 40%), finding that lower peak work rate (W) and peak oxygen uptake were significant predictors of death/LTX.
  • * The research revealed that patients with a peak work rate of 49.2% or lower had a much higher risk (45.2%) of death/LTX compared to those above that threshold (10.9%), indicating that W could be crucial for deciding on transplant referrals.

Review of infection and bleeding complications in excisional skin surgery.

Clin Exp Dermatol

January 2024

Wallsall Healthcare NHS Trust, Wallsall, UK.

Skin excision is the primary treatment for skin cancer. Complication rates from skin cancer excision are generally low but rates of complications may vary according to procedural complexity, site and patient factors. It is important that patients are fully informed through the consent process considering individual circumstances, the Montgomery ruling and material risks.

Heterozygous, pathogenic CUX1 variants are associated with global developmental delay or intellectual disability. This study delineates the clinical presentation in an extended cohort and investigates the molecular mechanism underlying the disorder in a Cux1 mouse model. Through international collaboration, we assembled the phenotypic and molecular information for 34 individuals (23 unpublished individuals).

The aim of this article is to share experience and learning of managing recurrent urinary tract infections (UTIs) within a specialist urology nurse-led team based at a district general hospital. It looks at current practice and supporting evidence for how to manage and treat recurrent UTIs in both male and female patients. Two case studies are presented to illustrate the management strategies and outcomes, demonstrating a planned approach that informs the design of a local management guideline to organise patients' care.

Background: Interstitial lung disease is a known complication of rheumatoid arthritis, with a lifetime risk of developing the disease in any individual of 7·7%. We aimed to assess the safety, tolerability, and efficacy of pirfenidone for the treatment of patients with rheumatoid arthritis-associated interstitial lung disease (RA-ILD).

Methods: TRAIL1 was a randomised, double-blind, placebo-controlled, phase 2 trial done in 34 academic centres specialising in interstitial lung disease in four countries (the UK, the USA, Australia, and Canada).
