Clinical Effectiveness Unit, Royal College of Surgeons of England, London, UK; Department of Health Services Research and Policy, London School of Hygiene and Tropical Medicine, London, UK.

Objective: Major amputation and death are significant outcomes after lower limb revascularisation for chronic limb threatening ischaemia (CLTI), but there is limited evidence on their association with the timing of revascularisation. The aim of this study was to examine the relationship between time from non-elective admission to revascularisation and one year outcomes for patients with CLTI.

Methods: This was an observational, population based cohort study of patients aged ≥ 50 years with CLTI admitted non-electively for infra-inguinal revascularisation procedures in English NHS hospitals from January 2017 to December 2019 recorded in the Hospital Episode Statistics database.

The high success rate of current dental implant protocols makes studies of catastrophic failures a rarity; however, this article presents a case study in which faulty medical intervention was the direct cause of traumatic implant failure. The patient, who had been treated with implants that successfully osseointegrated, ultimately experienced a life-threatening medical incident due to an ill-fitting definitive prosthesis. This case report includes a detailed narrative of how this patient, who began his dental journey with compromised and missing teeth, became edentulous and then, following an arduous recovery, was restored to full function with a meticulously planned revision treatment.

Article Synopsis
  • Older adults (65+) represent a significant portion of emergency laparotomy cases in the UK, yet there's limited data on patients who require surgery but do not undergo it, referred to as NoLap patients.
  • A study across 64 surgical centers tracked 750 NoLap patients over a 90-day period and found that 60% were female with a median age of 83, most suffering from frailty and severe comorbidities, leading to a high 90-day mortality rate of 79%.
  • The findings highlight the complex medical challenges faced by NoLap patients, with a small percentage surviving long-term but requiring increased care, emphasizing the need for further research on the decision-making process surrounding surgery in this high-risk group.

Signaling pathways activated and regulated by stem cell-derived exosome therapy.

Cell Biosci

August 2024

Department of ORL-HNS, Shanghai Fourth People's Hospital, School of Medicine, Tongji University, Shanghai, China.

Stem cell-derived exosomes exert comparable therapeutic effects to those of their parental stem cells without causing immunogenic, tumorigenic, and ethical disadvantages. Their therapeutic advantages are manifested in the management of a broad spectrum of diseases, and their dosing versatility are exemplified by systemic administration and local delivery. Furthermore, the activation and regulation of various signaling cascades have provided foundation for the claimed curative effects of exosomal therapy.

Immediate implant placement in the anterior maxilla remains complex, particularly when the labial cortical plate of the socket is deficient and there is an associated class IV ridge defect with both hard- and soft-tissue deficiencies. This case report describes a novel combination of polydioxanone (PDS) suture tenting to support a synthetic bone graft with simultaneous implant placement with implants featuring a body-shift design and subcrestal internal angle correction. In addition, the implants were immediately loaded with a four-unit provisional bridge at the time of surgery, thus reducing morbidity, costs, and total treatment time.
