Adequate postoperative pain control in patients who have undergone total joint arthroplasty allows faster rehabilitation and reduces the rate of postoperative complications. Multimodal pain management involves the introduction of adjunctive pain control methods in an attempt to control pain with less reliance on opioids and fewer side effects. Current research suggests that traditional nonsteroidal anti-inflammatory drugs (NSAIDs) and the associated cyclooxygenase type-2 (COX-2) inhibitors improve pain control in most cases.

Periprosthetic joint infection is now the leading cause of failure after a total knee arthroplasty, and Staphylococcus aureus, most commonly from the patient's own flora, typically is the infective agent. Several preoperative screening tests have been developed to identify patients who are carrying methicillin-resistant S aureus. Background: Periprosthetic infection (PPI) is a difficult complication in total joint arthroplasty, and while erythrocyte sedimentation rate (ESR) and C-reactive protein (CRP) are acute phase reactants thought to be of high predictive value for diagnosing infection, no clear cut-off values have been defined. The current study aimed to determine the cut-off values for ESR and CRP that improve clinical differentiation between aseptic failure and PPI in total hip arthroplasty (THA).
Methods: Four hundred and seventy-nine patients who underwent revision THA for either aseptic mechanical failure or PPI during the period of 2000 to 2005 were included in the study.
