Acute kidney injury (AKI) is associated with a very high mortality and an increased risk for progression to chronic kidney disease (CKD). Ischemia-reperfusion injury (IRI) is a model for AKI, which results in tubular damage, dysfunction of the mitochondria and autophagy, and in decreased cellular nicotinamide adenine dinucleotide (NAD+) with progressing fibrosis resulting in CKD. NAD+ is a co-enzyme for several proteins, including the NAD+ dependent sirtuins.

Introduction: Asthma is associated with prolonged time to pregnancy and a higher need for fertility treatment. However, the mechanism underlying this association remains incompletely understood. Previous research points to asthma-driven systemic inflammation also affecting the reproductive organs and thereby fertility.

Myeloid and lymphoid malignancies are postulated to have distinct pathogenetic mechanisms. The recent observation that patients with a myeloproliferative neoplasm have an increased risk of developing lymphoproliferative malignancy has challenged this assumption. We collected a nationwide cohort of patients with both malignancies.

Article Synopsis
  • The study aimed to validate the CT-Valve score to identify patients with valvular heart disease who are at low risk for coronary artery disease (CAD) and could opt for multislice CT (MSCT) instead of coronary angiography (CAG).
  • Conducted as a prospective cohort study over two years, it involved patients referred for valve surgery, excluding those with existing CAD or chronic kidney disease, and assessed the need for follow-up CAG after MSCT based on the CT-Valve score.
  • Results indicated that the CT-Valve score is effective; using a score of 7 or below showed a reduction in both costs (40%) and radiation exposure (41%), while follow-up revealed no ischemic events for patients

Article Synopsis
  • Mitochondrial DNA haplogroup H is linked to a higher risk of hypertrophic cardiomyopathy (HCM), though specific SNPs associated with this risk have not been identified.
  • A study compared the distribution of H haplogroup subtypes in 55 HCM patients with two control groups from Denmark and found a higher proportion of the H3 subtype in the HCM group.
  • The distinct proportions of H subhaplogroups suggest a difference between HCM cases and controls, although the significance of these differences remains unclear without further functional studies.

Background: Although not rare, hidradenitis suppurativa (HS) is often under-recognized by physicians. The diagnosis of HS is clinical via the recognition of lesions typical of the disease, but universally accepted definitions of these latter are currently lacking, which means that certain severity scores employed for HS classification/management are used differently by different physicians. Our aim was to develop a set of descriptive definitions and associated images of HS lesions, in order to enable doctors to better recognize and evaluate the disease.

Survival of patients with high-risk diffuse large B-cell lymphoma (DLBCL) is suboptimal, and the risk of central nervous system (CNS) progression is relatively high. We conducted a phase 2 trial in 139 patients aged 18 to 64 years who had primary DLBCL with an age-adjusted International Prognostic Index (aaIPI) score of 2 to 3 or site-specific risk factors for CNS recurrence. The goal was to assess whether a dose-dense immunochemotherapy with early systemic CNS prophylaxis improves the outcome and reduces the incidence of CNS events.

Background: Our current understanding of medical team competence is traditionally influenced by an individualistic perspective focusing on individual team members' knowledge, skills as well as on effective communication within the team. However, team dynamics may influence team performance more than previously anticipated. In particular, recent studies in other academic disciplines suggest that social ties between team members may impact team dynamics but this has not been explored for medical teams.

Objective: Complex soft tissue reconstruction of the head and neck requires a viable, versatile, and dependable flap. Free flaps, such as the radial forearm and anterolateral thigh flap, have been the mainstay of complex head and neck reconstruction. However, a local pedicled flap, such as the submental island flap (SIF), could be a more effective and less demanding alternative.
