Propofol is used for sedation, anxiolysis, anesthesia induction, and as an anticonvulsant. In cases of refractory status epilepticus (RSE), propofol is more efficient than barbiturates. We present a case of a 3-year-old female with RSE who developed propofol-related infusion syndrome (PRIS) despite low dosage after failed attempts with multiple anti-epileptic drips and bolus therapies.

Although there is evidence that psychological stress may be associated with increased cancer risk, the effect of stress on cancer risk is difficult to study, both in humans, due to socioeconomic factors, and in animal models, due to questionable biological relevance. Here, we test whether heritable canine temperament that increases psychological stress is associated with cancer risk. The study data are breed-specific averages of incidences of multiple cancer types and of temperament classes.

In this case report, we present a critical situation during an open calvarial reconstruction involving an 11-month-old infant. The patient experienced accidental extubation, requiring immediate intervention while in the prone position. Approximately two hours post-incision, ventilation became increasingly difficult due to a significant leak detected in the system.

Mitochondrial encephalomyopathy, lactic acidosis, and stroke-like episodes (MELAS) syndrome is a complex and infrequently encountered mitochondrial cytopathy. Patients with MELAS often present with multi-systemic manifestations, making their anesthetic management particularly challenging. In this case report, we describe in detail our anesthetic approach for a 19-year-old male with confirmed MELAS linked to an m.

In this systematic review, we address the status of intracortical brain-computer interfaces (iBCIs) applied to the motor cortex to improve function in patients with impaired motor ability. This study followed the Preferred Reporting Items for Systematic Reviews and Meta-Analyses (PRISMA) 2020 Guidelines for Systematic Reviews. Risk Of Bias In Non-randomized Studies - of Interventions (ROBINS-I) and the Effective Public Health Practice Project (EPHPP) were used to assess bias and quality.

Sclerotherapy with bleomycin can cause cosmetic complications, including flagellate dermatitis and hyperpigmentation, induced or exacerbated by microtrauma to the skin. We report a case of a 9-year-old pediatric patient with congenital vascular malformations in which a cohesive bandage (eg, 3M Coban) was utilized to prevent bleomycin-induced hyperpigmentation. Postoperatively and on follow-up, there were no signs of hyperpigmentation or dermatitis in our patient.

  • Postmortem redistribution (PMR) can alter drug concentrations after death, influenced by factors like drug properties, blood collection site, body storage, and time since death (PMI).
  • A study evaluated PMR of THC in rabbits, showing that THC levels increased in heart blood at 2 hours and in peripheral blood at 16 hours postmortem, depending on whether the body was refrigerated or at room temperature.
  • The research found varying ratios of THC in different body fluids and tissues, with lungs having the highest THC concentrations, while this study provides essential insights for toxicologists in legal investigations involving THC.

Article Synopsis
  • Proton pump inhibitors (PPIs), like omeprazole, are commonly prescribed for various gastrointestinal issues but their long-term effects, particularly concerning celiac disease (CD), are not fully understood.
  • Recent research indicates that PPIs may influence the development of CD by affecting the gut microbiome and triggering an inflammatory response, emphasizing the need for further studies.
  • The review advocates for a personalized approach to medication for patients on long-term PPI therapy, considering genetic factors like HLA-DQ2 and HLA-DQ8 that may increase the risk of developing CD.

  • Oral squamous cell carcinoma (OSCC) is the most common form of cancer in the oral cavity, often appearing as a red or white ulcerative growth.
  • Early-stage OSCC can be disguised as benign vascular tumors, leading to missed diagnoses and delays in treatment.
  • This case report describes a patient misdiagnosed with a benign vascular tumor that was actually an invasive SCC, aiming to raise awareness about the diverse ways OSCC can present.

Purpose: To perform a systematic review of randomized controlled trials comparing clinical outcomes of rotator cuff repair with and without patch augmentation.

Methods: A systematic review was conducted according to the PRISMA (Preferred Reporting Items for Systematic Reviews and Meta-Analyses) guidelines by searching PubMed, the Cochrane Library, and Embase to identify randomized controlled trials that directly compared outcomes between rotator cuff repair (RCR) with versus without patch augmentation. Patients were evaluated based on retear rate, histological outcomes, radiological outcomes, and patient-reported outcomes (Constant score; American Shoulder and Elbow Surgeons [ASES] score; University of California-Los Angeles shoulder scale; Simple Shoulder Test; EuroQol-visual analog scale; Disabilities of the Arm, Shoulder and Hand score; and PENN shoulder score questionnaire).

An elderly male with an extensive medical history was admitted for sepsis of unspecified origin; the source was later found to be an urinary tract infection. Urinalysis and conventional urine culture were used in diagnosing this infection, and it was successfully treated with a 7-day course of intravenous ceftriaxone. The patient had no recurrence of urinary symptoms or development of complications at his follow-up.

To report the clinical and anatomic outcomes of a single-stage rescue and sutureless 30-gauge needle-assisted transconjunctival intrascleral fixation of dislocated 3-piece rigid poly(methyl methacrylate) (PMMA) intraocular lenses (IOLs). This retrospective noncomparative single-surgeon interventional study comprised eyes that had concurrent 23- or 25-gauge pars plana vitrectomy and IOL rescue with sutureless transconjunctival needle-assisted flanged haptic intrascleral fixation. Lamellar scleral dissection for haptic fixation was performed 3 mm posterior to the surgical limbus with 30-gauge needles.

Total force fitness (TFF) is a multi-domain framework designed to assess, promote, and sustain human performance optimization across a service member's career arc. During the September 2021, TFF Summit sponsored by the Consortium for Health and Military Performance (CHAMP), a working group explored partnerships that could facilitate effective implementation and sustainment of TFF principles. Many potential partners were identified, both internal and external to the DoD.

Antimicrobial stewardship programs (ASPs) exist to optimize antibiotic use, reduce selection for antimicrobial-resistant microorganisms, and improve patient outcomes. Rapid and accurate diagnosis is essential to optimal antibiotic use. Because diagnostic testing plays a significant role in diagnosing patients, it has one of the strongest influences on clinician antibiotic prescribing behaviors.

Context: The United States Medical Licensing Examination (USMLE) Step 1 and Comprehensive Osteopathic Medical Licensing Exam (COMLEX) Level 1 transitioned from a numeric scoring system to a Pass/Fail designation in 2022. This transition intended to decrease stress, improve medical student well-being, and encourage residency program directors to emphasize other aspects of residency applications. Pass/Fail score transitions in the undergraduate medical education curriculum have improved medical student psychological well-being and satisfaction; whether these same benefits translate to the board examination period is unknown.

Article Synopsis
  • The study focused on how sleep and fatigue metrics change during intensive surgical and trauma skills training, particularly looking at the link between eye movement (oculomotor metrics) and fatigue.
  • Thirty-nine military medical students participated in a 6-day training program, completing surveys on sleepiness and undergoing oculomotor tests to measure fatigue while also tracking their sleep with Fitbit devices.
  • Results indicated improvements in sleep quality over the first few days, and specific oculomotor metrics showed patterns tied to fatigue, suggesting a potential relationship between sleep quality and resilience to fatigue during skills training.
