Background: Endoscopic submucosal dissection (ESD) for gastric neoplasms can be challenging due to limited traction and visualization. The efficacy of traction-assisted endoscopic submucosal dissection (TA-ESD) continues to require additional validation. This study aims to explore the safety and efficiency of TA-ESD for early gastric neoplasms located at varying sites, in comparison with conventional endoscopic submucosal dissection (C-ESD).

Purpose Of Review: This review delves into the prospects and challenges offered by a potential pan-histological utilization of trastuzumab deruxtecan (T-DXd) in patients with advanced solid tumors.

Recent Findings: The HER2-targeted antibody-drug conjugate (ADC) T-DXd has shown broad activity across cancer types, with current indications for patients with biomarker-selected breast, gastric, and non-small-cell lung cancer and relevant activity observed in multiple histology-specific trials. Moreover, two recently reported phase 2 trials (DESTINY-Pantumor02 and HERALD) have supported the potential for a pan-cancer utilization of this ADC in patients with advanced cancers expressing HER2 or with HER2 amplifications.

Background: Patients with end-stage liver disease (ESLD) who are not transplant candidates often have a trajectory of rapid decline and death similar to patients with stage IV cancer. Palliative care (PC) services have been shown to be underutilized for such patients. Most studies examining the role of PC in ESLD have been done at transplant centers.

Background: Serotypes 15B and 15C have been added to new different pneumococcal-conjugate vaccines (PCV20 and V116, respectively). We determined a serum anti-15B antibody level that would be a correlate of protection (COP) against nasopharyngeal colonization and assessed functional cross-reactivity against serotype 15B and 15C in children following natural immunization.

Method: IgG-antibody to serotype 15B polysaccharide was measured by ELISA in 341 sera from 6 to 36 month old children collected before, at the time of, and after pneumococcal colonization caused by serotypes 15B and 15C.

Article Synopsis
  • Traumatic dislocations of the fourth and fifth metacarpals, along with fractures of the hamate bone, can lead to instability in the CMC joints of the hand.
  • * A specific fracture pattern of the hamate can cause joint displacement, but surgical intervention with proper fixation can restore joint stability.
  • * The described technique involves directly accessing the affected joints and using a specialized plate for secure fixation of the hamate to improve alignment and stability in multi-injury cases.

Background: Takotsubo syndrome (TS) during pregnancy and postpartum is rare but may lead to significant maternal and fetal morbidity. We compared clinical characteristics and prognosis according to [a] timing of presentation (pregnancy vs post-partum) and [b] modes of delivery (cesarean section vs vaginal delivery).

Methods: Systematic review of articles published in PubMed, Scopus, Embase, and Medline databases from inception to July 30, 2023.

Hemophagocytic Lymphohistiocytosis and Pancreatic Cancer: A Rare Association.

J Community Hosp Intern Med Perspect

September 2023

Department of Internal Medicine, Monmouth Medical Center, Long Branch, NJ, USA.

Introduction: Hemophagocytic lymphohistiocytosis (HLH) or hemophagocytic syndrome (HPS) is a life-threatening and relatively rare condition that usually presents as a multisystem febrile illness. It is associated with excessive activation of the immune system and hypercytokinemia, leading to an unregulated aggregation of macrophages and lymphocytes. Here, we present the first likely case of HLH with metastatic pancreatic carcinoma being the underlying etiology.

Variability of vaccine responsiveness in early life.

Cell Immunol

January 2024

Center for Infectious Diseases and Immunology, Research Institute, Rochester General Hospital, 1425 Portland Ave, Rochester, NY 14621, USA. Electronic address:

Article Synopsis
  • Vaccinations during early life can lead to varying immune responses, sometimes resulting in inadequate protection against serious infections.
  • Modifiable factors, such as antibiotic use, feeding methods, and delivery type, play a significant role in shaping infant immune responses by affecting the development of the microbiota and metabolome.
  • Understanding how these early life factors influence vaccine effectiveness could help identify a pre-vaccination endotype to improve vaccine response predictions and reduce infection risks.

End-stage renal disease (ESRD) and atrial fibrillation (AF) are commonly encountered, with ESRD itself serving as a well-established risk factor for AF. The 2018 AF guidelines have recommended apixaban across all the spectrums of renal impairment, including patients on hemodialysis (HD), and the 2019 American Heart Association/American College of Cardiology/Heart Rhythm Society updated guidelines have suggested careful consideration of reduced dose of direct oral anticoagulants (DOACs) in patients with ESRD. The current data on the safety and efficacy of warfarin versus DOACs in patients with AF with ESRD and HD is variable.
