J Rheumatol

December 2024

L. Raffray (Md, Phd), Internal Medicine and Dermatology Unit, Reunion University Hospital, Saint-Denis, Reunion Island, France; UMR PIMIT "Processus Infectieux en Milieu Insulaire Tropical" CNRS 9192, INSERM1187, IRD 249, Reunion Island University, Saint-Denis, Reunion Island, France.

Article Synopsis
  • * A total of 268 CLE cases were recorded, with a standardized prevalence of 43/100,000 and an estimated incidence of 5.7/100,000 person-years, showing higher rates compared to lighter-skinned populations.
  • * Findings indicate that darker-skinned patients are more likely to experience severe forms of CLE and that certain clinical features may indicate a higher risk of progression to systemic lupus erythematosus (SLE), underscoring the need for tailored patient follow-up.

Introduction: Right diaphragmatic excursion is a reliable and reproducible technique used in intensive care to assess diaphragmatic function. The aim of this study was to investigate the relationship between the appearance of diaphragmatic motion and the etiological diagnosis of patients admitted to the emergency department with acute respiratory failure (ARF).

Materials: A prospective, observational, and multicenter pilot study was conducted.

Article Synopsis
  • Sickle cell disease (SCD) affects many people worldwide, and a serious issue related to it is called acute chest syndrome (ACS), which can make patients really sick and require hospitalization.
  • Researchers studied data from 1998 to 2022 about young people with SCD who needed a special treatment called ECMO to help their lungs.
  • They found that 55.8% of patients survived ECMO treatment, but there were complications, and not all patients, especially young kids and some adults, survived when ECMO was used for heart resuscitation.

Electronic address:

Article Synopsis
  • A study in La Reunion examined women's preferences for vaginal sampling (VS) for HPV testing as part of cervical cancer (CC) screening, focusing on women aged 30 to 65 who were concerned about their screening status.
  • A total of 202 women participated, with results showing that many preferred to collect the sampling kit from a healthcare provider but also favored self-sampling (SS) at home, with clear instructions provided in French and visual aids.
  • The findings indicated no significant difference in preferences between women who were up to date on screenings versus those who were not, suggesting a potential need for further research on the implementation of self-sampling for a broader population.

  • Advanced systemic mastocytosis (AdvSM) is a complex condition linked to poor outcomes, and while midostaurin is the first approved treatment, its long-lasting effectiveness is limited.
  • Various prognostic scoring systems like MARS, IPSM, and GPSM have been developed to assess patients' outcomes, but it's essential to tailor these scores to specific AdvSM subtypes for better accuracy.
  • A study of patients treated with midostaurin revealed that MARS and AdvSM subtype significantly predict overall survival, identifying five distinct patient subgroups with varying median survival times, highlighting the need for personalized management strategies.

Introduction: Reunion Island, a French overseas department, is located in the southern hemisphere, close to the Capricorn tropic. This island has a multicultural and multiethnic population of 860 000 inhabitants, a quarter of whom are at high risk of developing skin cancer due to intense ultraviolet radiation. Melanoma is responsible for the majority of skin cancer deaths.

Anoctamins are a family of Ca-activated proteins that may act as ion channels and/or phospholipid scramblases with limited understanding of function and disease association. Here, we identified five de novo and two inherited missense variants in ANO4 (alias TMEM16D) as a cause of fever-sensitive developmental and epileptic or epileptic encephalopathy (DEE/EE) and generalized epilepsy with febrile seizures plus (GEFS+) or temporal lobe epilepsy. In silico modeling of the ANO4 structure predicted that all identified variants lead to destabilization of the ANO4 structure.

Backgroud: Modelling discrete-time cause-specific hazards in the presence of competing events and non-proportional hazards is a challenging task in many domains. Survival analysis in longitudinal cohorts often requires such models; notably when the data is gathered at discrete points in time and the predicted events display complex dynamics. Current models often rely on strong assumptions of proportional hazards, that is rarely verified in practice; or do not handle sequential data in a meaningful way.

Appearance of the bladder on initial voiding cystogram in boys with PUV and its relation to pre and postnatal findings.

Front Pediatr

April 2024

Department of Pediatric Urology, Hôpital Pellegrin-Enfants, CHU Bordeaux, France.

Introduction: Bladder profile in boys with Posterior Urethral Valves can be very varied with a spectrum going from high pressure, unstable, hypocompliant small bladders to hypercompliant, large acontractile bladders, with some being near-normal. Our question was whether appearance, specifically of the bladder, on initial VCUG was correlated to prenatal features and whether it could predict early postnatal outcome.

Method: We used a prospectively gathered database of boys with prenatally suspected PUV.

Systemic mastocytosis (SM) corresponds to a rare and heterogeneous spectrum of diseases characterized by the accumulation of atypical mast cells (MCs). Advanced mastocytosis (Adv-SM) is associated with poor survival; in contrast, patients with non-advanced SM (non-Adv-SM) usually have a normal life expectancy but may experience poor quality of life. Despite recent therapeutic progress including tyrosine kinase inhibitors, new treatment options are needed for refractory and/or intolerant patients with both severely symptomatic and Adv-SM.

Background And Aims: Systemic mastocytosis (SM) is characterized by the accumulation of atypical mast cells (MCs) in organs. Liver histology of SM has been marginally described and accurate histological classification is critical, given the consequences of aggressive SM diagnosis. We aimed to describe the histological features associated with liver SM using updated tools.

  • Duplications of the 3q29 chromosomal region are rare genetic variations linked to diverse neurodevelopmental disorders, often causing learning disabilities and neuropsychiatric issues.
  • A study involving 31 families revealed different sizes of 3q29 duplications: 14 recurrent, 8 overlapping, and 9 smaller ones, with some patients showing additional genetic factors influencing their conditions.
  • Most patients exhibited mild neurodevelopmental disorders, with many duplications being inherited and associated with low rates of intellectual disabilities, suggesting that severe cases might require more detailed genetic examination.

Infection by arthritogenic alphaviruses (aavs) can lead to reactive arthritis, which is characterized by inflammation and persistence of the virus; however, its mechanisms remain ill-characterized. Intriguingly, it has been shown that viral persistence still takes place in spite of robust innate and adaptive immune responses, characterized notably by the infiltration of macrophages (sources of TNF-alpha) as well as T/NK cells (sources of IFN-gamma) in the infected joint. Aavs are known to target mesenchymal stem cells (MSCs) in the synovium, and we herein tested the hypothesis that the infection of MSCs may promote the expression of immunoregulators to skew the anti-viral cellular immune responses.

Background: Mastocytosis and monoclonal mast cell (MC) activation syndrome (MMAS) are heterogeneous conditions characterized by the accumulation of atypical MCs. Despite the recurrent involvement of KIT mutations, the pathophysiologic origin of mastocytosis and MMAS is unclear. Although hereditary α-tryptasemia (HαT, related to TPSAB1 gene duplication) is abnormally frequent in these diseases, it is not known whether the association is coincidental or causal.
