Background: Digoxin is widely used in patients with atrial fibrillation despite the lack of randomized controlled trials. Observational studies report conflicting results regarding its association with mortality, perhaps because of residual confounding by the presence of heart failure (HF).

Methods And Results: In the ENGAGE AF-TIMI 48 (Effective Anticoagulation With Factor Xa Next Generation in Atrial Fibrillation-Thrombolysis in Myocardial Infarction 48) trial, clinical outcomes of patients with atrial fibrillation with and without HF were examined by baseline digoxin use during a median follow-up of 2.

Practical Insight to Monitor Home NIV in COPD Patients.

COPD

August 2017

c AP-HP, Respiratory Physiology Department , Cochin Hospital, René Descartes University, Paris , France.

Home noninvasive ventilation (NIV) is used in COPD patients with concomitant chronic hypercapnic respiratory failure in order to correct nocturnal hypoventilation and improve sleep quality, quality of life, and survival. Monitoring of home NIV is needed to assess the effectiveness of ventilation and adherence to therapy, resolve potential adverse effects, reinforce patient knowledge, provide maintenance of the equipment, and readjust the ventilator settings according to the changing condition of the patient. Clinical monitoring is very informative.

Thrombus composition in sudden cardiac death from acute myocardial infarction.

Resuscitation

April 2017

Département de cardiologie, Hôpital Européen Georges Pompidou, Université Paris Descartes, Paris Cardiovascular Research Centre (PARCC), INSERM UMRS 970, Paris Sudden Death Expertise Centre, Paris, France.

Background And Aim: It was hypothesized that the pattern of coronary occlusion (thrombus composition) might contribute to the onset of ventricular arrhythmia and sudden cardiac death (SCD) in myocardial infarction (MI).

Methods: The TIDE (Thrombus and Inflammation in sudden DEath) study included patients with angiographically-proven acute coronary occlusion as the cause of a ST elevation MI (STEMI) complicated by Sudden Cardiac Death (SCD group) or not (STEMI group). Thrombi were obtained by thrombo-aspiration before primary percutaneous coronary stenting and analyzed with a quantitative method using scanning electron microscopy.

Article Synopsis
  • Few studies have examined how oral anticoagulants (OACs) are prescribed to very elderly patients (≥80 years) with atrial fibrillation (AF), compared to younger patients (<80 years) in the PREFER in AF study from 2012.
  • Patients ≥80 years were more likely to have permanent AF, report fatigue and dyspnea, and have a higher prevalence of various chronic conditions, though most received OAC treatment.
  • Although overall use of vitamin K antagonists was similar in both age groups, fewer patients ≥80 years received non-vitamin K antagonist OACs; additionally, those with a poorer quality of life were less likely to be prescribed OACs, indicating a potential need
