Unlabelled: Data about the long-term safety and efficacy of stent implantation for central venous stenosis in patients on dialysis are limited. We report the case of a 66-year-old man on hemodialysis for end-stage renal disease who presented with stasis dermatitis around an arteriovenous shunt with ulceration of the left forearm. Computed tomography angiography showed a tight stenosis of the proximal left subclavian vein and the development of collateral blood vessels around the stenosis.

We decided to be proactive by creating new workflows, education, and disease management so that patients could make well-informed choices, ultimately resulting in better outcomes.
Methods: Our shift toward value-based care focused on patient engagement, education, integrated infrastructure, collaboration, and monitoring of metrics associated with improved outcomes.

Background: Atherosclerotic cardiovascular disease is the leading cause of death in people with type 2 diabetes (T2D) and chronic kidney disease (CKD) or end-stage kidney disease (ESKD). Glucagon-Like Peptide-1 receptor agonists (GLP-1RA) reduce cardiovascular events, improve glycemic control, promote weight loss, and slow progression of nephropathy. Despite these benefits and professional society treatment guidelines recommendations, GLP-1RAs remain under-utilized in people with advanced CKD and ESKD due to tolerability and safety concerns.
