Hematuria in a runner after treatment with whole body vibration: a case report.

The use of whole body vibration (WBV) for therapeutic purposes is far from being standardized and the training protocols reported in the literature vary considerably. Currently, the optimal threshold for a beneficial effect is undetermined, and caution regarding potential health risks due to WBV is always necessary. In this case report, we present a 34-year-old otherwise healthy elite athlete (steeplechase runner) who suffered two episodes of hematuria (HT) after WBV training.

Bibliometric indicators: a snapshot of the scientific productivity of leading European PRM researchers.

Aim: The aim of this paper was to explore the validity and practical usefulness of a set of bibliometric indicators with a focus on the scientific production of influential European researchers in Physical and Rehabilitation Medicine (PRM).

Methods: We randomly selected 24 European PRM specialists from the list of invited lecturers or chairpersons at the 17th ESPRM Congress in 2010. Using the time window 1996-2010, we recorded the number of papers published, total number of citations, and h-index from Web of Science (WoS) and Publish or Perish (PoP) databases.

Bibliometric indicators and core journals in physical and rehabilitation medicine.

Background And Objective: The concept of the "standing" of scientific journals (in terms of influence, prestige, popularity, etc.) is multi-dimensional and cannot be captured adequately by a single indicator. The aim of this report is to compare and comment on different bibliometric indicators related to some leading journals in rehabilitation, in order to provide further insights regarding their practical usefulness for Physical and Rehabilitation Medicine.

Measuring mobility in people with lower limb amputation: Rasch analysis of the mobility section of the prosthesis evaluation questionnaire.

Objective: To assess the psychometric properties of the Mobility Section of the Prosthesis Evaluation Questionnaire (PEQ-MS).

Design: A postal survey, including self-report assessment of prosthetic capability and performance with the PEQ-MS and the Locomotor Capabilities Index, and of other variables associated with prosthetic wear and use. The PEQ-MS data underwent Rasch analysis for rating scale diagnostics and a reliability and validity study.

A case of impaired naming and knowledge of body parts. Are limbs a separate sub-category?

A left hemisphere stroke patient presented a disproportionate difficulty for body parts knowledge without autotopagnosia. The deficit concerned the lexical-semantic representation of body parts and was most severe for limbs. The ability to gesture was spared and action naming was not more impaired than object naming.

Crossed right hemisphere syndrome following left thalamic stroke.

In most right-handed people, language and motor functions are more reliant on systems of the left hemisphere while several non-linguistic visuo-spatial and attentional processes depend more on the right hemisphere. The rare exceptions to these rules provide important clues as to what functions co-lateralise, and are thus crucial for models of cerebral specialization. Here we report on the case of a patient, who, after a lesion restricted to the left thalamic region, showed signs normally associated with right hemispheric injury including motor impersistence, visuo-spatial dysfunction and poor comprehension of facial expression.

Severe humeral erosion in a bedridden patient: a comprehensive intervention to prevent bone fracture.

This article reports a 1-yr prospective study related to a bedridden patient with rotator cuff tear arthropathy featuring severe erosion of the proximal part of the humerus. To prevent spontaneous humeral fracture, a comprehensive intervention was planned. This included drug therapy for osteoporosis and pain, nurse team training on patient handling techniques, and patient and caregiver education.

Reliability, validity, and responsiveness of the locomotor capabilities index in adults with lower-limb amputation undergoing prosthetic training.

Objective: To assess the reliability, validity, and responsiveness of both the standard and revised Locomotor Capabilities Index (LCI) in people with lower-limb amputation who undergo prosthetic training.

Design: Reliability and validity study.

Setting: Two freestanding rehabilitation centers.

Category-specific semantic deficits do not reflect the sensory/functional organization of the brain: a test of the "sensory quality" hypothesis.

We report the performance of a herpes simplex encephalitis patient, EA, who shows a dissociation between poor performance for living things and spared performance for sensory quality categories, when appropriate measures are taken to control for baseline differences among categories in task difficulty and when we eliminate the contribution of visual processing deficits in the recognition of objects. This pattern of performance is problematic for theories of category-specific deficits that are based on the assumption that these deficits reflect selective damage to modality-specific semantic subsystems but is consistent with domain-specific accounts and theories based on the assumption that the structure of conceptual knowledge reflects the correlational structure among object properties.

Is the Rivermead Mobility Index a suitable outcome measure in lower limb amputees?--A psychometric validation study.

Objective: To examine the internal consistency, validity, responsiveness and test scalability of the Rivermead Mobility Index.

Design: Methodological research (consecutive sampling, prospective longitudinal study).

Patients: 140 unilateral lower limb amputees (79 above-knee and 61 below-knee).

Psychometric properties of the Rivermead Mobility Index in Italian stroke rehabilitation inpatients.

Objective: To examine the internal consistency, validity, responsiveness and test scalability of the Rivermead Mobility Index (RMI) in Italian rehabilitation inpatients recovering from stroke.

Design: Seventy-three stroke inpatients undergoing rehabilitation were assessed at admission (T0) and five weeks later (T1), using RMI, the motor (motFIM) and cognitive (cognFIM) subscales of the Functional Independence Measure, the 'leg' section of the Motricity Index and Trunk Control Test.

Results: Cronbach's alpha of the RMI was 0.

Satisfaction with hospital rehabilitation: is it related to life satisfaction, functional status, age or education?

Satisfaction with care, functional and cognitive status, life satisfaction, anxiety, and sociodemographic variables were correlated in 55 in-patients admitted to a rehabilitation unit after hip or knee surgery. The study aimed at investigating whether, as an index of care quality, patient satisfaction can be considered as a distinct domain or instead is subsidiary to other patient characteristics. Patient satisfaction with rehabilitation care was measured through a questionnaire, SAT-16.

Caregiver strain associated with tracheostomy in chronic respiratory failure.

Study Objectives: The aim of this study was to evaluate the psychological and psychosocial impact of tracheostomy on the caregivers of patients who have undergone tracheostomies.

Setting: Rehabilitation facility.

Participants: Sixty-three lay caregivers of inpatients and outpatients with various underlying diseases who have undergone tracheostomies.

Wisconsin card sorting test: a new global score, with Italian norms, and its relationship with the Weigl sorting test.

The Wisconsin card sorting test and the Weigl test are two neuropsychological tools widely used in clinical practice to assess frontal lobe functions. In this study we present norms useful for Italian subjects aged from 15 to 85 years, with 5-17 years of education. A case of prevailing deficit of nonliving categories or a case of prevailing sparing of living categories?

We describe a new case of semantic deficit in which nonliving categories are disproportionately impaired. PL, a woman affected by progressive degeneration of the left temporal lobe, was examined twice, at a distance of 1 year. The deficit was first apparent on naming and on a verbal semantic questionnaire, but a year later nonliving categories were disproportionately affected also on verbal comprehension (word/picture matching task).

Postural coordination in elderly subjects standing on a periodically moving platform.

Objective: To test body segment coordination during dynamic equilibrium on a moving platform, and assess age-related differences in the performance.

Design: Subjects had infrared light-emitting diode (LED) markers fixed on their temporomandibularjoint, greater trochanter, and lateral malleolus. Subjects stood upright on a sinusoidally translating platform with both feet, either with eyes open (EO) or eyes closed (EC).

The 'preliminary neuropsychological battery'. An instrument to grade the cognitive level of minimally responsive patients.

An early, formalized cognitive evaluation of'minimally responsive' patients could be important in the planning of their care and rehabilitation as well as in providing a realistic prognosis and studying the modality of the cognitive recovery. A short, bedside, neuropsychologically-oriented test-battery, the Preliminary Neuropsychological Battery, a psychometric tool which enables the cognitive evaluation of these patients and of patients unable to give verbal or complex motor answers, has been devised. The BNP was administered to a sample of 40 head-injured patients and to a sample of 34 healthy subjects.

Influence of clinical history on airways bacterial colonization in subjects with chronic tracheostomy.

Patients with chronic tracheostomy are subject to significant bacterial colonization of the airways, a risk factor for respiratory infections. The aim of our study was to verify whether bacterial colonization and humoral immune response in the airways can be influenced by the disease which led to chronic respiratory failure and tracheostomy. Thirty-nine clinically stable outpatients with chronic tracheostomy were considered: 24 were affected by chronic obstructive pulmonary disease (COPD) (mean age 66 years, range 54-78, M/F 19/3; months since tracheostomy 23, range 3-62), 15 by restrictive lung disease (RLD) (12 thoracic wall deformities, three neuromuscular disease; age 57 years, range 41-72; M/F 3/12, months since tracheostomy 22, range 2-68).

Do living and nonliving categories need further fractionation? A study of picture naming in a pathological sample.

A group of 51 patients affected by possible semantic memory deficit were given a picture naming task for the purposes of a comparison between six categories, three of Nonliving nature (tools, furniture, vehicles) and three of a Living nature (animals, fruits, and vegetables). A logistic regression analysis was used for a multiple single case study, where also the items' basic difficulty was included in the model. Besides some patients showing a dissociation between Living and Nonliving categories, other patients showed a finer selectivity on naming, differentiating animals from fruits and vegetables, tools from nonmanipulable objects, and even vehicles from furniture.

Physiologic determinants of ventilator dependence in long-term mechanically ventilated patients.

To investigate the pathophysiologic mechanisms of ventilator dependence, we took physiologic measurements in 28 patients with COPD and 11 postcardiac surgery (PCS) patients receiving long-term mechanical ventilation during a spontaneous breathing trial, and in 20 stable, spontaneously breathing patients matched for age and disease. After 40 +/- 14 min of spontaneous breathing, 20 of 28 patients with COPD and all 11 PCS patients were judged ventilator-dependent (VD). A new thermoplastic splint for proximal interphalangeal joint flexion contractures.

Background: Aim of this technical note is to describe the fabrication procedure of a new thermoplastic static-progressive hand-based splint for PIP joint flexion contractures, and report its effects in the treatment of a small group of sportsmen.

Methods: The serial-static splint consists of a short metacarpal gauntlet base, with a hole for the thumb and a dorsal finger gutter that extends to the distal extremity. A low-temperature thermoplastic material, two loop fastener straps, three small pieces of self-adhesive hook fastener and cooling spray are required.

Increased MCP-1 and MIP-1beta in bronchoalveolar lavage fluid of chronic bronchitics.

CC-chemokines are chemotactic factors expressed in a wide range of cell types and tissues. The aim of this study was to evaluate the involvement of CC-chemokines in the airways inflammation of patients affected by chronic bronchitis. The study evaluated, with an immunoassay, the concentrations of monocyte chemotactic protein-1 (MCP-1), macrophage inflammatory protein-1alpha (MIP-1alpha) and macrophage inflammatory protein-1beta (MIP-1beta), in the bronchoalveolar lavage fluid (BALF) of 12 smokers affected by chronic bronchitis and 14 smoking, 15 nonsmoking and six exsmoking healthy subjects.

Physiologic response of ventilator-dependent patients with chronic obstructive pulmonary disease to proportional assist ventilation and continuous positive airway pressure.

To investigate the physiologic effects of proportional assist ventilation (PAV) in difficult-to-wean, mechanically ventilated patients with advanced COPD, we measured in eight ICU patients the breathing pattern, neuromuscular drive (P0.1), lung mechanics, and inspiratory muscle effort (PTPdi and PTPpl) during both spontaneous breathing (SB) and ventilatory support with PAV, CPAP, and CPAP + PAV (in random sequence). PAV (volume assist [VA] and flow assist [FA]) was set as follows: dynamic lung elastance and inspiratory pulmonary resistance were measured during SB; then VA and FA were set to counterbalance the elastic and resistive loads exceeding the normal values, respectively, the inspiratory muscles bearing a normal elastic and resistive workload.

Standing on a continuously moving platform: is body inertia counteracted or exploited?

We describe the characteristics of displacement of the head and hip in normal young subjects standing on a moving platform undergoing continuous sinusoidal horizontal translation in the antero-posterior direction, at frequencies ranging from 0.1-1 Hz. The head, hip and malleolus were marked by light-emitting diodes (LEDs), and the displacement of each LED was quantified by (1) the measure of the shift during each cycle of translation, (2) the standard deviation (SD) of the path travelled during the whole trial, (3) the power spectrum (PS) of the signal and (4) the cross-correlation (CC) between pairs of LED signals.
