Comparison of current staging systems for predicting pediatric cholesteatoma outcomes.

Int J Pediatr Otorhinolaryngol

December 2024

Department of Otolaryngology, University of Minnesota, Minneapolis, MN, USA; HealthPartners Medical Group, St. Paul, MN, USA. Electronic address:

Article Synopsis
  • This study evaluates the effectiveness of two staging systems (EAONO-JOS and Potsic) in predicting recurrence of cholesteatoma in children with different types (congenital, primary acquired, and secondary acquired).
  • Conducted from 2015 to 2023, the research analyzed surgical outcomes from 31 ears in 30 pediatric patients, examining variables like residual disease and factors like stapes erosion that might signal recidivism.
  • Results indicated high rates of residual disease, particularly in congenital and primary acquired cases, and found a potential correlation between certain variables and poorer outcomes; however, limitations in the staging systems decreased the predictivity for residual disease.

Recent literature demonstrated a 24-hour reduction in vancomycin duration of therapy (DOT) for skin and soft tissue infections (SSTIs) with a negative methicillin-resistant staphylococcus aureus (MRSA) nasal screening versus a positive nasal screening. Objective of this study was to investigate vancomycin DOT in patients with SSTIs who received MRSA nasal polymerase chain reaction (PCR) screening versus those who did not receive MRSA nasal PCR screening. A retrospective, multi-center, cohort study was completed in admitted adult patients on vancomycin for SSTI from 01/01/2020 to 09/30/2022.

Article Synopsis
  • A young patient with neuromyelitis optica spectrum disorder experienced recurrent cardiac asystole and syncope following persistent hiccups, but successfully underwent cardioneuroablation (CNA) treatment, avoiding the need for a permanent pacemaker.
  • Previous cases of symptomatic bradycardia-arrhythmia related to area postrema syndrome (APS) showed a significant number required pacemaker implantation, highlighting the severity of the condition.
  • The findings suggest that CNA can effectively treat bradyarrhythmia associated with APS, and healthcare professionals should consider the potential link between these symptoms and APS in their clinical assessments.*

Comparison of elective implant removal and complication rates between mini and small fragment implants for lateral malleolar fixation.

Injury

December 2024

Department of Orthopaedic Surgery, Regions Hospital, Saint Paul, MN, USA; Department of Orthopaedic Surgery, University of Minnesota Medical School, Minneapolis, MN, USA. Electronic address:

Article Synopsis
  • The study investigates the use of mini-fragment implants versus small-fragment implants for open reduction and internal fixation (ORIF) of ankle fractures, aiming to determine if mini-fragments lead to lower rates of elective implant removal and complications.
  • It analyzed data from 505 patients, finding similar rates of elective implant removal (8.3% for mini-fragment vs. 10.8% for small-fragment) and complication rates (6.7% for mini-fragment vs. 6.5% for small-fragment).
  • The authors conclude that while mini-fragment implants are popular, their higher cost must be weighed against their similar effectiveness to small-fragment implants in terms of implant removal and complications.

Traditionally, surgical repair of phalanx fractures was performed under general anesthesia. However, the emergence of regional and local anesthesia, otherwise known as Wide-awake Local Anesthesia No Tourniquet, provides an alternative approach where general anesthesia is undesirable. The choice of anesthetic approach resides with clinicians, though it is important to factor in the evidence that regional/local provides not only an alternative anesthesia approach but also potentially avoids comorbidities associated with general anesthesia.
