Article Synopsis
  • Rectal prolapse (RP) is a rare condition where part or all of the rectum protrudes through the anal canal, more commonly seen in elderly females with various risk factors.
  • A unique case involved a previously healthy 39-year-old man with incarcerated RP, requiring an urgent surgical procedure (Altemeier procedure) and a later reversal of a colostomy.
  • The case emphasizes the need for surgical management in instances of incarcerated RP and highlights the absence of specific management guidelines, making decisions based on each patient's unique circumstances.

Perineal Rectosigmoidectomy (Altemeier's Procedure) in the Treatment of Strangulated Rectal Prolapse: A Case Series and Literature Review.

J Pers Med

November 2024

4th Department of Surgery, General Hospital of Thessaloniki "G. Papanikolaou", School of Medicine, Faculty of Health Science, Aristotle University of Thessaloniki, 541 24 Thessaloniki, Greece.

Background: Rectal prolapse (RP) predominantly affects women over the age of 50 and presents as mucosal, internal, or full thickness prolapse. Strangulated rectal prolapse requires immediate medical intervention, and surgical treatment options include both abdominal and perineal approaches. We aim to present a case series of perineal rectosigmoidectomy performed urgently due to strangulation and argue that Altemeier's procedure is the preferred method for treating strangulated rectal prolapse.

A rare account of incidentally discovered anal melanoma.

J Surg Case Rep

November 2024

Department of Colorectal Surgery, Huntington Hospital, Huntington, NY, United States.

Anal melanoma is a rare and highly aggressive malignancy that carries a poor prognosis. Due to its variable and ambiguous presentation, it is often misdiagnosed as a hemorrhoid, polyp, or an ulcer with a concomitant rectal prolapse. Clinicians usually have a low suspicion of anal melanoma due to its rarity and most people present with metastatic disease at the time of diagnosis.

The prolapse of a pedunculated juvenile rectal polyp is a rare event. It occurs when the polyp protrudes through the anus and appears as a fleshy mass at the anus, leading to an alarming scenario for both the parent and patient and typically prompting a visit to the emergency department (ED). We report a case of a five-year-old male patient who presented to the ED with a prolapsed rectal mass.

Article Synopsis
  • - Rectal mucosal prolapse syndrome (RMPS) often presents with symptoms like rectal bleeding and can be mistaken for malignant rectal tumors (MRT) during endoscopic exams; this study aimed to differentiate between the two and examine RMPS characteristics.
  • - A total of 826 patients were analyzed, revealing that RMPS patients were younger, had smaller lesions, and a longer clinical course compared to those with MRT.
  • - Five distinct morphological phenotypes of RMPS lesions were identified, with gender differences noted in lesion types; key features like age, lesion size, and clinical duration can help differentiate RMPS from MRT.

Article Synopsis
  • Rectal prolapse surgery outcomes were reviewed in predominantly male populations, examining factors like recurrence, complications, and bowel function.
  • Eight studies involving 452 male patients showed that abdominal procedures had lower recurrence rates (6%) compared to perineal procedures (19.3%), but longer operative times.
  • The best results for improving constipation and fecal incontinence were found after resection rectopexy and posterior mesh rectopexy, respectively.
