Pain is a significant consequence of cardiac surgery and newer techniques in cardiac anesthesia have provided an impetus for the development of multimodal techniques to manage acute pain in this setting. In this regard, regional anesthesia techniques have been increasingly used in many cardiac surgical procedures, for the purposes of reducing perioperative consumption of opioid agents and enhanced recovery after surgery. The present investigation focuses on most currently used regional techniques in cardiac surgical procedures.

Article Synopsis
  • Monthly intravenous immunoglobulin (IVIG) and weekly subcutaneous immunoglobulin (SCIG) are both used to treat primary immunodeficiency diseases (PIDD), and IgG trough levels are key for monitoring infection risks.* -
  • A systematic review analyzed studies from 2010 to 2018, finding that SCIG led to higher IgG trough levels than IVIG, with a significant decrease in infections correlating with increased trough levels in SCIG patients.* -
  • The study concluded that SCIG is more effective than IVIG in achieving higher IgG levels and reducing infection rates, highlighting the importance of treatment choice in managing PIDD.*

Article Synopsis
  • Recent studies indicate a notable rise in allergic conditions globally, prompting an examination of their prevalence over time in the U.S.
  • Analyzing data from 2001 to 2014, researchers found increasing hospitalization rates for angioedema and stable rates for anaphylaxis, with a decrease in urticaria cases.

Transcatheter mitral valve-in-valve (ViV) & valve-in-ring (ViR) are relatively novel therapeutic alternatives for patients with degenerated bioprostheses or failed annuloplasty rings whose reoperative risk is too high. The predominant procedural access for both procedures is transapical or transseptal. However, whether there are differences in outcomes of this procedure using transseptal versus transapical access has not yet been defined.

IgG4-related sclerosing cholangitis (IgG4-SC) is one of the most common extra-pancreatic manifestation of IgG4-related disease (IgG4-RD) and is clinically distinct from primary sclerosing cholangitis (PSC). IgG4-RD is an increasingly recognized immune-mediated fibroinflammatory systemic disease, mostly affecting middle-aged and older male populations that can affect multiple organs. The presence of extra-biliary clinical manifestations of IgG4-RD, such as parotid and lacrimal swelling, lymphadenopathy, autoimmune pancreatitis, and retroperitoneal fibrosis, if present could provide important clues to diagnosis.

Although Hyper-IgE Syndrome (HIES) is a rare immunodeficiency disorder, presenting symptoms may be as common as lung and skin infections. Symptoms are usually nonspecific such as recurrent abscesses, folliculitis, and pneumonias along with skeletal abnormalities. Careful history of susceptibility to skin and lung infections, thorough family history, and findings on physical exam can guide towards the diagnosis of this often-eluded condition.
