The origin of autoantibodies in patients with antiphospholipid syndrome (APS) is unknown. The gut microbiome contributes to autoimmunity and contains peptide homologues to the main APS autoantigen, which affect disease activity in animal models. Alteration of the gut microbiota with vancomycin diminishes disease activity in mice but no data on the effect of gut microbiota alteration in APS patients are available to date.

Academic Medical Centres (AMCs) are large organisations with a complex structure due to various intertwined missions and (public) roles that can be conflicting. This complexity makes it difficult to adapt to changing circumstances. The literature points to the use of business models to address such challenges.

Introduction: Academic medical centres (AMCs) are designed to perform multiple tasks within a single organisation. This institutional complexity gives rise to intricate governance challenges and promotes incrementalism and muddling.

Method: In this study, we hypothesised that radical change could provide a solution to the current incrementalism and we explored the conditions under which such changes could or could not be achieved.

Developmental or Procedural Vena Cava Interruption and Venous Thromboembolism: A Review.

Semin Thromb Hemost

September 2024

Cardiovascular Medicine Division, Brigham and Women's Hospital, Harvard Medical School, Boston, Massachusetts.

Article Synopsis
  • The inferior vena cava (IVC) and superior vena cava play crucial roles in returning blood from the body to the heart, and interruptions in these vessels can lead to stasis and increased risk of deep vein thrombosis (DVT) and pulmonary embolism (PE).
  • This study reviewed causes of IVC and superior vena cava interruption, including congenital issues like IVC agenesis, which heightens the risk of DVT, especially in younger patients.
  • Modern management predominantly involves vena cava filters for patients at risk of thromboembolism who cannot take anticoagulants, moving away from older surgical methods that posed significant risks and complications.

Targeting thromboinflammation in antiphospholipid syndrome.

J Thromb Haemost

April 2023

Department of Rheumatology, Radboudumc and Sint Maartenskliniek, Nijmegen, The Netherlands.

Antiphospholipid syndrome (APS) is a systemic autoimmune disease, where persistent presence of antiphospholipid antibodies (aPL) leads to thrombotic and obstetric complications. APS is a paradigmatic thromboinflammatory disease. Thromboinflammation is a pathophysiological mechanism coupling inflammation and thrombosis, which contributes to the pathophysiology of cardiovascular disease.

Article Synopsis
  • This study looked at how to safely check if pregnant women might have a serious condition called pulmonary embolism (PE) using different tests and rules.
  • They found that using the Wells rule and certain blood tests (D-dimer) can help figure out if PE is likely, and these methods worked well without too many false alarms.
  • However, another test using ultrasound on veins (CUS) didn’t work well unless the patients had more obvious symptoms of another condition, so it's better to use the blood tests in these cases.

Aims: Deciding to stop or continue anticoagulation for venous thromboembolism (VTE) after initial treatment is challenging, as individual risks of recurrence and bleeding are heterogeneous. The present study aimed to develop and externally validate models for predicting 5-year risks of recurrence and bleeding in patients with VTE without cancer who completed at least 3 months of initial treatment, which can be used to estimate individual absolute benefits and harms of extended anticoagulation.

Methods And Results: Competing risk-adjusted models were derived to predict recurrent VTE and clinically relevant bleeding (non-major and major) using 14 readily available patient characteristics.
