Article Synopsis
  • Post-traumatic stress disorder (PTSD) is a challenging mental health condition often inadequately addressed by existing treatments, leading to interest in MDMA-assisted psychotherapy (MDMA-AT) as a potential solution.
  • A comprehensive analysis of nine studies involving 297 PTSD patients revealed that MDMA-AT significantly reduced PTSD severity scores and increased rates of patient response and remission compared to control groups receiving placebo or inactive doses of MDMA.
  • The findings suggest that MDMA-AT is an effective treatment option for chronic, treatment-resistant PTSD patients while showing no significant increase in adverse effects compared to traditional treatment methods.

Background: Cerebral venous thromboembolism (CVT) poses a significant risk of venous infarction and haemorrhage, which can lead to neurological deficits and, in severe cases, even death. The optimal treatment regimen for patients with CVT remains unclear.

Methods: MEDLINE, Embase, Google Scholar, Web of Science (WoS), and Cochrane Central databases were searched for randomized controlled trials (RCTs) and observational studies assessing the efficacy and safety of rivaroxaban in patients with CVT.

Background: Patients with non-valvular atrial fibrillation with diabetes face increased stroke and cardiovascular risks. This study compares factor Xa inhibitors and warfarin using data from randomized controlled trials (RCTs).

Methods: MEDLINE, Embase, and Cochrane CENTRAL databases were searched for RCTs comparing the risk of efficacy and safety of any factor Xa inhibitors with dose-adjusted warfarin by diabetes status.

A rare case of Swyer syndrome from Pakistan in a young girl with primary amenorrhea and 46XY genotype.

Womens Health (Lond)

November 2023

Department of Medicine and Allied Specialties, Hayatabad Medical Complex, Peshawar, Pakistan.

Swyer syndrome is a condition where individuals with a 46XY karyotype, typically associated with males, display complete gonadal dysgenesis and lack testicular differentiation. This results from a mutation in the SRY gene, which is essential for testis development. As a consequence, affected individuals who appear phenotypically female have male chromosomes but do not develop functional testes.

Tranexamic acid (TXA) reduces the risk of death and is recommended as a treatment for women with severe postpartum bleeding. There is hope that giving TXA shortly before or immediately after birth could prevent postpartum bleeding. Extending the use of TXA to prevent harmful postpartum bleeding could improve outcomes for millions of women; however we must carefully consider the balance of benefits and potential harms.

Article Synopsis
  • - Breast cancer is the second leading cause of cancer-related deaths; antibody-drug conjugates (ADCs) target specific tumor proteins, offering potent treatment with less toxicity compared to traditional chemotherapy.
  • - A meta-analysis of 14 clinical trials (N = 5,960) found that ADCs significantly improved progression-free survival (PFS) and overall survival (OS) in patients with HER-2 positive and triple-negative breast cancer compared to chemotherapy.
  • - The study highlighted that among ADCs, trastuzumab-deruxtecan (T-der) showed superior effectiveness over trastuzumab-emtansine (T-DM1), with common serious side effects including anemia and fatigue observed in a small percentage of

Background: Azithromycin has been adopted as a component of the COVID-19 management protocol throughout the global healthcare settings but with a questionable if not downright unsubstantiated evidence base.

Objectives: In order to amalgamate and critically appraise the conflicting evidence around the clinical efficacy of Azithromycin (AZO) vis a vis COVID-19 management outcomes, a meta-analysis of meta-analyses was carried out to establish an evidence-based holistic status of AZO vis a vis its efficacy as a component-in-use of the COVID-19 management protocol.

Methods: A comprehensive systematic search was carried out through PubMed/Medline, Cochrane and Epistemonikos with a subsequent appraisal of abstracts and full-texts, as required.

Effect of Adjunctive Simvastatin on Depressive Symptoms Among Adults With Treatment-Resistant Depression: A Randomized Clinical Trial.

JAMA Netw Open

February 2023

Department of Psychological Medicine, Institute of Psychiatry, Psychology and Neuroscience, King's College London, London, United Kingdom.

Importance: Immune-metabolic disturbances have been implicated in the pathophysiology of major depressive disorder and may be more prominent in individuals with treatment-resistant depression (TRD). Preliminary trials suggest that lipid-lowering agents, including statins, may be useful adjunctive treatments for major depressive disorder. However, no adequately powered clinical trials have assessed the antidepressant efficacy of these agents in TRD.

Article Synopsis
  • The study assessed the impact of pulse oximetry on mortality rates in children hospitalized with pneumonia, finding that it significantly reduced case fatality risk (CFR) from 5.8% to 2.1%).
  • Among the studied children, nearly 20% with chest-indrawing pneumonia were hypoxemic, leading to a 10.3% CFR for those cases, highlighting the critical need for monitoring oxygen levels.
  • Key mortality risk factors identified included younger age, moderate malnutrition, and female sex; the findings suggest that integrating pulse oximetry into pneumonia care for children under 5 is vital.

Assembling a global database of child pneumonia studies to inform WHO pneumonia management algorithm: Methodology and applications.

J Glob Health

December 2022

Department of Maternal, Newborn, Child and Adolescent Health and Ageing, World Health Organization (WHO), Geneva, Switzerland.

Background: The existing World Health Organization (WHO) pneumonia case management guidelines rely on clinical symptoms and signs for identifying, classifying, and treating pneumonia in children up to 5 years old. We aimed to collate an individual patient-level data set from large, high-quality pre-existing studies on pneumonia in children to identify a set of signs and symptoms with greater validity in the diagnosis, prognosis, and possible treatment of childhood pneumonia for the improvement of current pneumonia case management guidelines.

Methods: Using data from a published systematic review and expert knowledge, we identified studies meeting our eligibility criteria and invited investigators to share individual-level patient data.
