Article Synopsis
  • - A man in his 70s was diagnosed with a large left inguinoscrotal mass, and tests revealed extremely high levels of hCG, leading to the identification of paratesticular dedifferentiated liposarcoma (DDLPS) with rhabdomyosarcomatous features after surgical resection.
  • - The patient unfortunately passed away 11 months later due to distant metastasis, highlighting that hCG-producing soft tissue sarcomas (STS) are typically aggressive, poorly differentiated, and associated with a poor prognosis.
  • - This case emphasizes the need for more research on the treatment of DDLPS, particularly the potential benefits of adjuvant radiotherapy, given the rapid growth and

The use of radiotherapy has increased in recent years, especially for pelvic neoplasms, and this can result in long-term complications such as recurrent haemorrhagic radiation cystitis (RHC). A 73-year-old male patient presented to a hospital emergency department multiple times with visible haematuria and clots leading to urinary clot retention; he was finally diagnosed with RHC. During the last presentation, the bladder was irrigated continuously with saline using a three-way catheter.

Background: Inconsistent nomenclature and anatomical descriptions of regional anesthetic techniques hinder scientific communication and engender confusion; this in turn has implications for research, education and clinical implementation of regional anesthesia. Having produced standardized nomenclature for abdominal wall, paraspinal and chest wall regional anesthetic techniques, we aimed to similarly do so for upper and lower limb peripheral nerve blocks.

Methods: We performed a three-round Delphi international consensus study to generate standardized names and anatomical descriptions of upper and lower limb regional anesthetic techniques.

Article Synopsis
  • Platelet activation is linked to the spread of cancer, and ongoing trials are testing if aspirin can help prevent this process by reducing platelet activation.
  • A study involving 716 cancer patients measured a specific biomarker (U-TXM) related to platelet activation after treatment and its relation to patient factors and aspirin use.
  • Results showed that while lower doses of aspirin significantly reduced U-TXM levels across different cancer types, higher doses didn’t provide any additional benefit; elevated U-TXM levels suggest ongoing malignancy activity, especially in patients with colorectal and gastro-oesophageal cancers.

Article Synopsis
  • Lateral column instability in adult acquired flatfoot deformity leads to poor arch support, and this study aims to quantify the contribution of various ligaments to that stability using cadaver samples.
  • Researchers sectioned ligaments from 17 preserved cadaveric specimens and analyzed how each ligament affected the movement of the metatarsal head under different dorsal forces.
  • The findings showed that the lateral plantar ligaments and calcaneocuboid capsule were key to metatarsal motion and overall foot stability, which could inform future surgical techniques for restoring arch support.

Background: To date, evaluation of the heel external rotation test has not been taken with respect to AAFD. Traditional 'gold standard' tests do not account for the contribution of the midfoot ligaments towards instability. These tests would be flawed as any midfoot instability may produce a false positive result.

There remains a debate whether the ventricular volume within prolapsing mitral valve (MV) leaflets should be included in the left ventricular (LV) end-systolic volume, and therefore factored in LV stroke volume (SV), in cardiac magnetic resonance (CMR) assessments. This study aims to compare LV volumes during end-systolic phases, with and without the inclusion of the volume of blood on the left atrial aspect of the atrioventricular groove but still within the MV prolapsing leaflets, against the reference LV SV by four-dimensional flow (4DF). A total of 15 patients with MV prolapse (MVP) were retrospectively enrolled in this study.
