Article Synopsis
  • Schizophrenia and ADHD are serious mental health disorders that may share some genetic and symptomatic links, but there’s limited guidance on treating patients with both conditions, specifically regarding psychostimulants.
  • This scoping review will systematically gather and analyze existing literature on the effects of psychostimulants for these patients, using a structured framework and screening multiple databases along with grey literature.
  • The review aims to fill the gap in high-quality evidence for healthcare providers, helping to inform better treatment decisions and improve outcomes for patients with comorbid schizophrenia and ADHD.

Objectives: Remote monitoring of health has the potential to reduce the burden to patients of face-to-face appointments and make healthcare more efficient. Apps are available for patients to self-monitor vision at home, for example, to detect reactivation of age-related macular degeneration (AMD). Describing the challenges when implementing apps for self-monitoring of vision at home was an objective of the MONARCH study to evaluate two vision-monitoring apps on an iPod Touch (Multibit and MyVisionTrack).

Post-traumatic Stress Disorder: Focus on Neuroinflammation.

Mol Neurobiol

July 2023

Department of Anatomy, College of Basic Sciences, China Medical University, Shenyang, Liaoning, China.

Article Synopsis
  • Post-traumatic stress disorder (PTSD) is a complex psychiatric condition that arises after experiencing traumatic events, and it's gaining more attention in research.
  • Recent studies have highlighted the important connection between PTSD and neuroinflammation, which involves the activation of immune cells in the brain, such as microglia and astrocytes.
  • The review explores how stress impacts the hypothalamic-pituitary-adrenal (HPA) axis and its relationship with immune responses in the brain, alongside examining changes in inflammatory markers linked to PTSD.

IgD multiple myeloma is uncommon. Patients generally present at a younger age and have shorter progression free and overall survivals (OSs). Its rarity has inhibited development of a specific risk stratification system or informed best treatment protocols.

Article Synopsis
  • The study investigates potential soluble biomarkers in blood and bronchoalveolar lavage fluid (BALF) that could help diagnose systemic sclerosis-associated interstitial lung disease (SSc-ILD), which is a rare and complex condition.
  • A systematic review of 768 studies narrowed down to 38 relevant articles, with 13 included in meta-analyses, identifying three key biomarkers: KL-6, SP-D, and IL-8 that show significant elevation in SSc-ILD patients compared to healthy controls.
  • The findings suggest that KL-6, SP-D, and IL-8 are promising as reliable diagnostic biomarkers for SSc-ILD, but more research is needed to validate other proposed biomarkers

Purpose: To report prevalence and risk factor associations for age-related macular degeneration (AMD) and AMD features from multimodal retinal grading in a multidisciplinary longitudinal population-based study of aging in Northern Ireland.

Study Design: Population-based longitudinal cohort study.

Methods: Retinal imaging at the Norther Ireland Cohort for the Longitudinal Aging Study health assessment included stereo Colour Fundus Photography (CFP) (Canon CX-1, Tokyo, Japan) and Spectral-Domain Optical Coherence Tomography (SD-OCT) ((Heidelberg Retinal Angopgraph (HRA)+OCT; Heidelberg Engineering, Heidelberg, Germany).

This systematic review assessed the long-term outcomes for patients treated with intravitreal antivascular endothelial growth factor or dexamethasone for macular oedema (MO) secondary to retinal vein occlusion (RVO). Studies investigating patients of all ages with MO due to RVO only were included. The review was deliberately broad in scope, including comparative and non-comparative studies to ensure inclusion of real-world type evidence.

'Visiting uncertainty': an immersive primary care simulation to explore decision-making when there is clinical uncertainty.

July 2022

Queen's University Belfast Faculty of Medicine Health and Life Sciences, Clinical Skills Education Centre, Belfast, United Kingdom of Great Britain and Northern Ireland.

Doctors are required to be able to care for patients in a variety of settings, including the patient's home. Patients requiring urgent care in their own homes are at risk of acute deterioration. However, differentiating acute deterioration from self-limiting conditions in the primary care environment can be challenging, even for GPs who are experienced in managing clinical uncertainty and ambiguity.

Purpose: To determine the effect of depth of sedation on intensive care mortality, duration of mechanical ventilation, and other clinically important outcomes.

Methods: We searched MEDLINE, Embase, Cochrane Register of Controlled Trials, Cumulative Index to Nursing and Allied Health Literature, PsycINFO from 2000 to 2020. Randomised controlled trials (RCTs) and cohort studies that examined the effect of sedation depth were included.

Introduction: Obesity and being overweight are major risk factors for metabolic syndrome and non-communicable diseases. Despite the recommendation that a healthy diet and physical activity can reduce the severity of these diseases, many fail to adhere to these measures. From a behavioural economic perspective, adherence to such measures can be encouraged through financial incentives.

Melphalan at a myeloablative dose followed by autologous stem cell transplantation (ASCT) remains the standard of care for transplant-eligible patients with myeloma. However, therapies such as new immunomodulatory drugs and proteasome inhibitors and, more recently, monoclonal antibodies and chimeric antigen receptor T cells are challenging the traditional role of ASCT. Which patients benefit from ASCT? Can its use be delayed until first relapse? The field is moving rapidly as novel agents lead to new patient care strategies.

No examination of a child's abdomen or gastrointestinal system is complete without an examination of the 'backside' or perineum, however there is often reluctance among paediatricians to perform this examination routinely. This article aims to describe how to use perineal examination in infants and children. It discusses the indications and stepwise approach to performing a per-rectal examination and interpretation of the findings one could expect to encounter.

MicroRNA-containing extracellular vesicles released from endothelial colony-forming cells modulate angiogenesis during ischaemic retinopathy.

J Cell Mol Med

December 2017

Centre for Experimental Medicine, School of Medicine, Dentistry and Biomedical Sciences, Queen's University Belfast Faculty of Medicine Health and Life Sciences, The Wellcome-Wolfson Institute, Belfast, Co Antrim, UK.

Endothelial colony-forming cells (ECFCs) are a defined subtype of endothelial progenitors that modulate vascular repair and promote perfusion in ischaemic tissues. Their paracrine activity on resident vasculature is ill-defined, but mediated, at least in part, by the transfer of extracellular vesicles (EVs). To evaluate the potential of isolated EVs to provide an alternative to cell-based therapies, we first performed a physical and molecular characterization of those released by ECFCs.

In February 2016, the National Institute for Health and Care Excellence (NICE) published guidelines on multiple myeloma. NICE have published numerous guidelines relating to haematology, but this was the first guideline focusing on a single haematological malignancy. The purpose of this review was to highlight the recommendations made in the guideline and the implications for the management of patients in the UK and also internationally.
