Introduction And Hypothesis: Sigmoid vaginoplasty is a safe and acceptable procedure for vaginal agenesis with good cosmetic results and acceptable complications rate. Sigmoid colon vaginoplasty is the treatment of choice because of its large lumen, thick walls resistant to trauma, adequate secretion allowing lubrication, not necessitating prolonged dilatation, and short recovery time. We investigate the feasibility, safety, and clinical therapeutic effect of laparoscopic sigmoid vaginoplasty in women with Mayer-Rokitansky-Kuster-Hauser (MRKH) syndrome.

Introduction: In robot-assisted sacrocolpopexy (RSC) for patients with cystocele, accurate identification of the vaginal point Aa from the serosal side is crucial for surgical mesh placement in the appropriate position. We developed a novel Pharus method for exactly locating the point Aa for RSC.

Methods: In the Pharus method, the tip of a rigid endoscope was placed directly on the vaginal point Aa.

Preoperative vaginal microbiome as a predictor of postoperative urinary tract infection.

Sci Rep

November 2024

Department of Surgery, Department of Obstetrics and Gynecology, Microbiome Program, Center for Individualized Medicine, Mayo Clinic, Rochester, MN, USA.

Article Synopsis
  • This study involved 22 postmenopausal women with pelvic organ prolapse set to undergo vaginal hysterectomy and pelvic reconstructive surgery, and it focused on the relationship between their vaginal microbiome and postoperative urinary tract infections (UTIs).
  • Researchers collected vaginal and urine samples at five different times during the surgical process to analyze the microbiome, particularly looking for changes in bacteria types through DNA sequencing.
  • The results showed that women who developed UTIs post-surgery had a unique vaginal microbiome profile, featuring low levels of Lactobacillus and higher levels of Prevotella and Gardnerella, suggesting that preoperative microbiome analysis can help predict UTI risk after the surgery.

Background: Some research indicates that unsaturated fatty acids (UFAs) in the diet could enhance reproductive outcomes in infertile women. However, other research holds different views, possibly due to differences in the conversion rates of UFAs from various foods and bioavailability in the body. Therefore, this research examined the link between serum UFAs and infertility issues.

The aesthetics of the periorbital region are significantly influenced by the position of the eyebrows, forehead wrinkles, and upper eyelid ptosis, with aging-related eyebrow ptosis playing a major role. Brow lift surgery has seen a marked increase in popularity, driven by a variety of techniques tailored to individual patient needs. The 'gliding brow lift' technique, which employs a minimally invasive subcutaneous approach using a hemostatic net, represents a recent advancement in eyebrow rejuvenation.

Objective: The objective of this study was to describe the single-surgeon experience on transvaginal natural orifice transluminal endoscopic surgery for uterosacral ligament suspension in patients with severe prolapse who had concomitant vaginal hysterectomy.

Methods: A total of 53 patients with severe uterine prolapse who underwent vaginal hysterectomy and transvaginal natural orifice transluminal endoscopic surgery for uterosacral ligament suspension between January 2021 and March 2023 were included in the study. Operation time, intraoperative and postoperative complications, de novo urinary continence, and duration of hospitalization were obtained from the patient records.

Third nerve palsies that result from head injuries or intracranial aneurysms may sometimes show symptoms of aberrant regeneration and only partially heal. The usual characteristics of oculomotor nerve palsy, caused by compression of the third cranial nerve, are severe ptosis, deficiencies in elevation, depression, and adduction, and a dilated, poorly responding pupil on the afflicted side. The parasympathetic fibres are usually spared from a vasculopathic lesion and impacted by compressive lesions because they are situated in the peripheral segment of the oculomotor nerve as it leaves the brain stem.

Ptosis and proximal weakness may develop in multiple diseases including oculopharyngeal muscular dystrophy (OPMD) and myasthenia gravis (MG). These two entities can be challenging to differentiate since they may have important clinical and neurophysiological overlap. Systemic lupus erythematosus (SLE), a common autoimmune disease, has been described in association with MG.

Introduction: We present a new technique, the bilobed lateral artery perforator-based flap, for breast-conserving surgery of large central tumors or nearby, combining Zymany's bilobed flap and a Lateral Intercostal Perforator (LICAP) flap, and its 10-year outcomes.

Materials And Methods: We studied 37 patients with malignant breast tumors near or involving the central skin, without ptosis or desire to correct it, who avoided mastectomy with this modified bilobed flap from 2013 to 2022. The same surgeon operated on them in different institutions.

Article Synopsis
  • A 50-year-old male exhibited a 10-year progression of cerebellar ataxia, multifocal dystonia with tremors, and abnormal eye movements but showed no cognitive decline or other neurological issues.
  • This case emphasizes the importance of accurately diagnosing adult-onset cerebellar ataxia by detailing the patient's symptoms and clinical evaluation.
  • It also illustrates how genomic testing, particularly whole-exome sequencing, can aid in understanding these complex conditions and enhance clinical phenotyping.

Article Synopsis
  • Pelvic organ prolapse (POP) is a common issue affecting many women worldwide, and recent research highlights the effectiveness of laparoscopic pectopexy (LP) but points out its cost and complications due to synthetic mesh.
  • A new procedure called laparoscopic non-mesh cerclage pectopexy with uterine preservation (LNMCPUP) has been developed, allowing for successful uterine suspension without using mesh, resulting in high success rates and minimal complications.
  • This technique not only avoids the risks associated with synthetic mesh but also offers a solution for women who want to keep their uterus during surgery, making it a viable option especially in areas where mesh is not accessible.

Myasthenia gravis (MG) is an autoimmune neuromuscular disorder that most frequently affects the extraocular muscles (EOMs), which causes symptoms such as ptosis and restricted eye movement. The EOMs in MG patients are representative of autoimmune inflammatory changes in muscle tissue. Currently, there is no reliable, and sensitive imaging technique for monitoring EOM changes to assist in the evaluation of underlying pathological changes.

Article Synopsis
  • The study assessed the link between inherited mitochondrial dysfunction and neuromuscular junction (NMJ) remodeling in patients with mitochondrial disorders, analyzing muscle biopsies from 15 patients and 10 controls.
  • Results showed that patients with mitochondrial disorders had significantly more remodeled and neoformed NMJ endplates, with a trend towards increased Schwann cell extensions, indicating NMJ alterations even without muscle weakness.
  • The findings suggest that mitochondrial disorders may lead to NMJ remodeling as a primary issue, separate from structural muscle damage, though the exact mechanisms and clinical indicators warrant further investigation.
