Behavioral Determinants of Older Adults' and Caregivers' Willingness to Deprescribe: A Systematic Review.

Public Health Nurs

December 2024

Clinical Pharmacy and Pharmacoepidemiology Research Group, Louvain Drug Research Institut, Université Catholique de Louvain, Woluwé, Belgium.

Background: Deprescribing is a strategy to optimize medication use and to prevent medication harm. Despite the fact that behavioral theories have been shown to be useful in explaining health behaviors, the literature on deprescribing relies almost exclusively on attitudes as an explanatory factor for deprescribing behavior. This study systematically reviews the literature that made explicit use of the constructs included in health behavior theories (HBTs) to explain older adults' and informal caregivers' deprescribing behavior and outcomes.

Transcranial direct current stimulation (tDCS) has the potential to modulate spatial attention by enhancing the activity in one hemisphere relative to the other. This study aims to inform neurorehabilitation strategies for spatial attention disorders by investigating the impact of tDCS on the performance of healthy participants. Unlike prior research that focused on visual detection, we extended the investigation to visual search and visual imagery using computerized neuropsychological tests.

Let's focus on the insula in addiction: A refined anatomical exploration of insula in severe alcohol and cocaine use disorders.

Eur Psychiatry

November 2024

Normandie Université, UNICAEN, INSERM, U1237, PhIND "Physiopathology and Imaging of Neurological Disorders", NeuroPresage Team, Cyceron, Caen, France.

Article Synopsis
  • The study investigates the changes in gray matter (GM) volume in the insula, a brain region linked to addiction, comparing severe alcohol use disorder (sAUD) and severe cocaine use disorder (sCUD).
  • Researchers analyzed 12 subregions of the insula using a sample of 50 sAUD patients, 61 sCUD patients, and 36 healthy controls, finding overall lower insula volume in both disorders, particularly in the anterior long gyrus (ALG).
  • The results highlight both shared and distinct patterns of insula volume deficits between sAUD and sCUD, suggesting that while the insula is important in substance use disorders, each disorder may have unique characteristics that could inform treatment approaches.

Objective: Prominent models postulate that alcohol-related attentional bias (AB), emerging from the overactivation of the reward system, plays a key role in severe alcohol use disorder (sAUD) and is independent from voluntary control. We determined whether AB is indeed compulsive or can be modulated by the control/inhibition system.

Method: Patients with sAUD (17 women, 13 men, mean age of 47, White) and matched healthy controls (16 women, 14 men, mean age of 44, White) performed a visual probe task with behavioral (reaction time) and eye-tracking (first fixation location and duration, second fixation location, dwell time) measures.

Previous evidence suggests males and females differ with respect to interoception-the processing of internal bodily signals-with males typically outperforming females on tasks of interoceptive accuracy. However, interpretation of existing evidence in the cardiac domain is hindered by the limitations of existing tools. In this investigation, we pooled data from several samples to examine sex differences in cardiac interoceptive accuracy on the phase adjustment task, a new measure that overcomes several limitations of the existing tools.

Is severe alcohol use disorder really associated with increased utilitarian moral judgment? Exploration using the CNI model.

Drug Alcohol Depend

November 2024

Objectives: The psychology of moral decision-making classically contrasts utilitarianism (based on consequences) and deontology (based on moral norms). Previous studies capitalizing on this dichotomy have suggested the presence of a utilitarian bias among patients with severe alcohol use disorder (SAUD). We aimed to further disentangle the processes involved in such bias through a more validated approach, the CNI model of moral decision-making.

Emotional processing in binge drinking, tobacco use disorder and their comorbidity in youth: A preregistered PRISMA scoping review.

Prog Neuropsychopharmacol Biol Psychiatry

September 2024

Louvain Experimental Psychopathology research group (LEP), Psychological Science Research Institute, UCLouvain, Louvain-la-Neuve, Belgium. Electronic address:

Background: Binge drinking (BD) and tobacco use disorder (TUD) are prevalent among youth, with significant social and health implications. However, research into the emotional impairments associated with BD and TUD during adolescence is sparse and lacks integration within a comprehensive model of emotional processes. Moreover, the impact of comorbid BD and TUD on emotional deficits remains largely unexplored.

Article Synopsis
  • The continuum hypothesis suggests that binge drinking and severe alcohol use disorder (SAUD) might cause similar problems with thinking and feelings.
  • A study compared binge drinkers and light drinkers on moral choices, finding that both groups made decisions similarly without major differences.
  • The results indicate that binge drinkers can still make social decisions well, meaning that their ability to handle complex social situations isn’t as affected as thought.

Electronic address:

Background: Our brain uses interoceptive signals from the body to shape how we perceive emotions in others; however, whether interoceptive signals can be manipulated to alter emotional perceptions is unknown. This registered report examined whether alcohol administration triggers physiological changes that alter interoceptive signals and manipulate emotional face processing.

Methods: Participants (n=36) were administered an alcohol or placebo beverage.

Are we just talking in circles? Impact of psychoeducation on disease knowledge and relapse in severe alcohol use disorder.

Alcohol Clin Exp Res (Hoboken)

July 2024

UNICAEN, INSERM, U1237, PhIND « Physiopathology and Imaging of Neurological Disorders », Neuropresage Team, Cyceron, Normandie Univ, Caen, France.

Background: Psychoeducation constitutes a routine therapeutic practice in most treatment settings for severe alcohol use disorder (sAUD). This technique is considered an efficient way to help patients to learn more about their disease and achieve therapeutic objectives. However, this approach capitalizes on three untested assumptions: namely, that (1) patients with sAUD possess insufficient knowledge about sAUD at treatment entry; (2) patients with sAUD have the cognitive resources to learn new information and benefit from psychoeducation; and (3) psychoeducation positively impacts clinical outcomes.

Aims: Alcohol acutely impacts interoceptive processes, which in turn affect the perception of alcohol effects and the development of alcohol expectancies. However, previous research is limited by the tools used to measure cardiac interoception and subjective alcohol effects. This registered report proposes a re-examination of previous findings using a state-of-the-art measure of interoceptive capacity, the heart rate discrimination task, and measurements of subjective alcohol effects across both ascending and descending limbs.

Objective: Rumination is conceptualized as a critical transdiagnostic vulnerability and maintenance factor for affective dysregulation and related emotional disorders. Recent research has pointed to transcranial direct current stimulation (tDCS) as a novel therapeutic tool for alleviating rumination, especially stress-induced rumination. However, the mechanisms of action underlying this effect remain unclear, particularly regarding the potential moderating role of executive control and trait-like rumination.

Hostile attributional biases in severe alcohol use disorder: replication, gender specificity, and mechanistic insights.

Alcohol Alcohol

January 2024

Louvain Experimental Psychopathology Research Group (LEP), Psychological Science Research Institute, UCLouvain, B-1348 Louvain-la-Neuve, Belgium.

Aims: Despite their importance in the emergence and persistence of severe alcohol use disorder (SAUD), social cognition impairments remain understudied in this population. Hostile attributional biases (HAB), a key component of social cognition, may be involved in interpersonal problems and SAUD maintenance. However, current evidence for HAB in SAUD is highly preliminary, as it relies on a single study based on a small sample and on a task that cannot dissociate increased hostile from reduced benign attributions.

Social cognition impairments, and notably emotional facial expression (EFE) recognition difficulties, as well as their functional and clinical correlates, are increasingly documented in severe alcohol use disorder (SAUD). However, insights into their underlying mechanisms are lacking. Here, we tested if SAUD was associated with alterations in the attentional processing of EFEs.

Insular volumetry in severe alcohol use disorder and Korsakoff's syndrome through an anatomical parcellation: Let us go back to basics.

Addict Biol

October 2023

Normandie Univ, UNICAEN, INSERM, PhIND, Physiopathology and Imaging of Neurological Disorders, Institut Blood and Brain at Caen-Normandie, Cyceron, Caen, France.

Functional neuroimaging has demonstrated the key role played by the insula in severe alcohol use disorder (sAUD), notably through its involvement in craving and body signals processing. However, the anatomical counterpart of these functional modifications in sAUD patients with and without neurological complications remains largely unexplored, especially using state-of-the-art parcellation tools. We thus compared the grey matter volume of insular subregions (form anterior to posterior: anterior inferior cortex, anterior short gyrus, middle short gyrus, posterior short gyrus, anterior long gyrus, posterior long gyrus) in 50 recently detoxified patients with sAUD, 19 patients with Korsakoff's syndrome (KS) and 36 healthy controls (HC).

Background And Aims: Competing models disagree on three theoretical questions regarding alcohol-related attentional bias (AB), a key process in severe alcohol use disorder (SAUD): (1) is AB more of a trait (fixed, associated with alcohol use severity) or state (fluid, associated with momentary craving states) characteristic of SAUD; (2) does AB purely reflect the over-activation of the reflexive/reward system or is it also influenced by the activity of the reflective/control system and (3) does AB rely upon early or later processing stages? We addressed these issues by investigating the time-course of AB and its modulation by subjective craving and cognitive load in SAUD.

Design: A free-viewing eye-tracking task, presenting pictures of alcoholic and non-alcoholic beverages, combined with a concurrent cognitive task with three difficulty levels.

Setting: A laboratory setting in the detoxification units of three Belgian hospitals.
