Article Synopsis
  • Vitamin K antagonists like warfarin are currently the approved oral anticoagulants for preventing complications in patients with mechanical heart valves, but the efficacy of apixaban in this context was unknown.
  • In a trial comparing apixaban to warfarin for patients with On-X mechanical aortic valves, there were significantly more thromboembolic events in the apixaban group, leading to the study being halted after enrolling 863 participants.
  • The results showed that apixaban was less effective than warfarin, failing to meet the criteria for noninferiority in preventing valve thrombosis, and had comparable but slightly lower major bleeding rates.

Article Synopsis
  • - This study explored how preoperative thyroid hormone replacement affects complications in patients undergoing major abdominal surgery over a decade at an academic institution.
  • - Among 2,700 patients, those on thyroid replacement had a significantly higher risk (1.5 times) of serious complications and longer hospital stays, along with an increased likelihood of undergoing open surgery.
  • - The findings suggest that thyroid hormone replacement is an important factor to consider when evaluating surgical risks, as it is linked to greater morbidity and extended recovery times.

Case: A 62-year-old nonambulatory female patient presented with wound dehiscence and purulent bloody drainage 2 weeks after modified Girdlestone resection arthroplasty for subtrochanteric femur fracture. On developing an enlarging thigh hematoma and hemodynamic instability, the patient was taken to the vascular suite where a profunda femoris artery pseudoaneurysm was identified and ligated by the vascular surgery team. The proximal free edge of the resected femur was determined to be the likely cause of arterial injury.

We present a case report of a previously healthy 29 year old woman who developed thromboembolic events in rapid fashion after a recent orthopedic surgery. This case highlights the importance of understanding risk factors for clot formation even in those who had no prior events, the efficacy of novel aspiration techniques to remove emboli, and how rapid triaging of acute symptoms can lead to the best outcomes. She presented to the emergency department with shortness of breath and was found to have a large bilateral pulmonary embolus.

Gastrointestinal tuberculosis (TB) is a rare manifestation of extra-pulmonary TB that is known to mimic many different gastrointestinal diseases. We present a case of an 85-year-old male patient with delayed diagnosis of gastrointestinal TB who underwent colonic resection for a cecal mass that was initially suspected to be malignant. Acid-fast staining of the surgical specimen later revealed acid-fast bacilli and multiple lymph nodes with necrotizing granulomas.

We present the case of a symptomatic patient with two separate accessory navicular bones, a novel finding that does not fall within current classification standards. Furthermore, there is a paucity of current literature with regard to the management of symptomatic cases. Accessory navicular bones, sometimes referred to as os naviculare, are ossicles that can occur in multiple configurations and are considered developmental anomalies.

Cervical cancer is an international public health crisis, affecting several hundred thousand women annually. While not universally protective due to other risk factors, many such cases are preventable with vaccination against high-risk serotypes of the human papilloma virus (HPV 6, 11, 16, 18, 31, 33, 45, 53, 58). Advanced-stage and recurrent cervical cancers are typically lethal and have been the focus in recent years of the integration of immune checkpoint inhibitors (CPIs) to improve survival.
