Background: The effect of a liberal red-cell transfusion strategy as compared with a restrictive strategy in patients during the critical care period after an aneurysmal subarachnoid hemorrhage is unclear.

Methods: We randomly assigned critically ill adults with acute aneurysmal subarachnoid hemorrhage and anemia to a liberal strategy (mandatory transfusion at a hemoglobin level of ≤10 g per deciliter) or a restrictive strategy (optional transfusion at a hemoglobin level of ≤8 g per deciliter). The primary outcome was an unfavorable neurologic outcome, defined as a score of 4 or higher on the modified Rankin scale (range, 0 to 6, with higher scores indicating greater disability) at 12 months.

Department of Radiation Oncology, Nelune Comprehensive Cancer Centre, Prince of Wales Hospital, Randwick, 2031 New South Wales, Australia; School of Clinical Medicine (Randwick Campus), Faculty of Medicine and Health, University of New South Wales, Randwick, 2031 New South Wales, Australia; Department of Radiation Oncology, Tamworth Base Hospital, Tamworth, 2340 New South Wales, Australia.

Linear accelerator (LINAC) stereotactic radiosurgery (SRS) is a specialised treatment used for various brain conditions, including arteriovenous malformations (AVMs). This paper investigates the obliteration rate of AVMs treated by the LINAC-based facility at Prince of Wales Hospital, defines factors influencing obliteration rate, time to obliteration and complications post-SRS. A retrospective audit review of patient notes sourced from electronic medical records was conducted.

Despite these challenges, a total of 28,793 transplants, including 18,518 allogeneic and 10,275 autologous transplants, were performed in 719 facilities in 2020 in the Asia-Pacific (AP) region. This represented a 5.

Next-generation immunotherapy: igniting new hope for lung cancer.

Ther Adv Med Oncol

December 2024

State Key Laboratory of Translational Oncology, Department of Clinical Oncology, The Chinese University of Hong Kong, Shatin, Hong Kong.

Adoption of immunotherapy has completely transformed the treatment landscape of cancer. Patients with advanced cancer treated with immunotherapy may benefit from durable tumor response and long-term survival. The most widely used immunotherapy in solid tumors is anti-programmed-death (ligand) protein (PD-(L)1), which is now an integral part of non-small cell lung cancer (NSCLC) treatment irrespective of histological cell types and tumor stage.

A first-in-class vaccine adjuvant delivery system, Mn-ZIF, is developed by incorporating manganese (Mn) into the zinc-containing zeolitic-imidazolate framework-8 (ZIF-8). The mixed metal approach, which allowed for tunable Mn doping, is made possible by including a mild reducing agent in the reaction mixture. This approach allows up to 50% Mn, with the remaining 50% Zn within the ZIF.

Neurophysiological assessment of cortical motor function: A direct comparison of methodologies.

Clin Neurophysiol

December 2024

Neuroscience Research Australia, 139 Barker Street, Randwick, 2031, Sydney, Australia; University of NSW and Department of Neurology, Prince of Wales Hospital, South Eastern Sydney Area Health Service, Sydney, Australia.

Objective: Assessment of cortical function with threshold tracking transcranial magnetic stimulation (TT-TMS) has developed as a biomarker to inform disease pathophysiology, particularly in neurodegenerative disease and dementia. At present, a fully integrated testing system does not exist. To advance clinical utility, and to streamline software design to integrate with diagnostic approaches in an outpatient setting, the present series of studies assessed the effects of altering diagnostic paradigms to measure interstimulus interval (ISI) including serial ascending [T-SICIs] and parallel [T-SICIp] methodologies as measures of cortical motor function (the MagXite software).

Department of Autoimmune Diseases, Institut D'Investigacions Biomèdiques August Pi I Sunyer (IDIBAPS), University of Barcelona, Gran Via de les Corts Catalanes, 585, L'Eixample, 08007 Barcelona, Spain.

Giant cell arteritis (GCA) is a chronic inflammatory vasculitis with a significant impact on vascular and patient health. It may present with non-specific symptoms and can lead to severe complications if not managed effectively. This narrative review explores the treatment of GCA with interleukin-6 (IL-6) pathway inhibitors, focusing on key studies from selected databases published between 2018 and 2024.

Introduction: Evidence-based clinical practice guidelines drive optimal patient care and facilitate access to high-quality treatment. Creating guidelines for rare diseases such as haemophilia, where evidence does not often come from randomized controlled trials but from non-randomized and well-designed observational studies and real-world data, is challenging. The methodology used for assessing available evidence should consider this critical fact.

Article Synopsis
  • The paper aims to explore the challenges rural nurses face during resuscitations, highlighting how this contributes to their professional isolation, and to propose strategies to alleviate these issues.
  • It emphasizes that professional isolation is prevalent in low-resourced rural healthcare settings, particularly where small emergency departments are located far from major hospitals, and notes a lack of research on effective interventions.
  • The findings suggest that professional isolation hinders skill development and career intent among rural nurses, and recommends solutions like leadership training, mentorship, and debriefing sessions to help improve their professional experiences and connections.

Food Allergy in Children in China.

Clin Exp Allergy

December 2024

Department of Paediatrics, Faculty of Medicine, Prince of Wales Hospital, The Chinese University of Hong Kong, Hong Kong, China.

The prevalence of food allergies in China seems to be increasing, but there are limited studies describing the pattern of food allergies across the country. This review highlights regional variations observed across China, with data indicating a higher prevalence in the more economically developed eastern and southern coastal regions compared to inland areas. Egg and milk are the most common allergies among children under 3 years old; for children above 3 years old, specific food allergens also show regional differences, with shellfish allergies being more common in southern and eastern coastal areas, while wheat and fruit allergies are more prevalent in northern regions.

Improving Lynch syndrome detection: a mixed-methods process evaluation of a hybrid type III effectiveness-implementation trial.

Implementation to Impact (i2i), School of Population Health, Faculty of Medicine, UNSW, Samuels Building, Sydney, NSW, 2052, Australia.

Introduction: Translating evidence-based practices into real-world healthcare settings is challenging, particularly in the rapidly evolving field of genomics. A pragmatic two-arm cluster-randomized clinical trial (Hide and Seek Project - HaSP) tested two implementation approaches for improving hereditary cancer referral practices with one key distinction: implementation strategies that were designed 1) explicitly using psychological theory, or 2) using healthcare professional intuition. This mixed-methods process evaluation aimed to provide insights into how and why change occurred by examining contextual determinants, identifying mechanisms of action, and exploring the role of theory.

Article Synopsis
  • The study aimed to analyze the prevalence of in-person visits and virtual communication methods (like phone and video calls) in ICUs across Australia and New Zealand, especially during the COVID-19 pandemic.
  • Findings revealed that 77% of ICUs had visiting restrictions, with a significantly reduced median visiting time of 9 hours per day, resulting in only 65% of patients receiving in-person visits.
  • The majority of in-person visits were for family interaction, while telephone calls were primarily for clinical updates, with visit patterns showing higher activity during daytime hours rather than the evening.
