Cardiovascular clinical trials continue to under-represent children, older adults, females and people from ethnic minority groups relative to population disease distribution. Here we describe strategies to foster trial representativeness, with proposed actions at the levels of trial funding, design, conduct and dissemination. In particular, trial representativeness may be increased through broad recruitment strategies and site selection criteria that reflect the diversity of patients in the catchment area, as well as limiting unjustified exclusion criteria and using pragmatic designs that minimize research burden on patients (including embedded and decentralized trials).

Cardiac mechanics and the risk of atrial fibrillation in a community-based cohort of older adults.

Eur Heart J Cardiovasc Imaging

November 2024

Cardiovascular Division, Department of Medicine, University of Minnesota Medical School, 420 Delaware Street SE, MMC 508, Minneapolis, MN 55455, USA.

Article Synopsis
  • The study aimed to compare standard and new cardiac measures to better predict atrial fibrillation (AF) risk in older adults who have not previously experienced AF or stroke.
  • A total of 5,050 participants, average age 75, were analyzed over seven years, finding that specific measures related to the left atrium (LA) significantly enhanced AF risk prediction compared to previous methods like the CHARGE-AF score.
  • Results indicated that LA function metrics, such as reservoir strain and contraction strain, were particularly effective in improving prediction accuracy, suggesting their importance in assessing AF risk among older adults.

Background: Recent studies suggest that follow-up paracentesis (FuP) in cases of spontaneous bacterial peritonitis (SBP) should only be performed if there is a clinical and/or analytic worsening. We aimed to evaluate which patients with SBP would benefit from the FuP.

MÉtodos: This retrospective study included consecutive patients diagnosed with SBP between January 2011 and June 2018 in our tertiary center.

Serviço de Saúde Ocupacional, Centro Hospitalar São João, E.P.E, Porto, Portugal. EPIUnit - Institute of Public Health, University of Porto, Porto, Portugal. Department of Public Health and Forensic Sciences, and Medical Education, University of Porto Medical School, Porto, Portugal.

Background: Work accidents (WA) due to percutaneous or mucocutaneous injury are extremely frequent among health professionals. The notification of these accidents ensures not only the compliance with legal requirements, but it also enables health institutions to develop, implement and evaluate strategies to prevent them. This study aimed to estimate the proportion of underreporting of work accidents caused by percutaneous and mucocutaneous lesions in a hospital setting, as well as its determinants.

Endolymphatic hydrops (EH) is considered the histological hallmark of Meniere's disease. Visualization of EH has been achieved by special sequences of inner ear magnetic resonance imaging (MRI) with a gadolinium-based contrast agent intravenous or intratympanic administration. Although it has been applied for more than 10 years since 2007, a unified view on this technique has not yet been achieved.

Intermediate- to high-grade non-muscle invasive bladder cancer is preferably treated with transurethral resection followed by adjuvant intravesical immunotherapy with Bacillus Calmette-Guérin (BCG). BCG acts as an immune stimulator, inducing a complex inflammatory response that selectively targets tumoral cells. Mild side effects of BCG instillation, such as fever, malaise, and bladder irritation are frequent, while severe treatment-associated complications of the genito-urinary tract are rare.

Article Synopsis
  • - The study examined the link between left atrial (LA) function and the occurrence of subclinical cerebral infarcts (SCIs) and strokes in older adults without atrial fibrillation, finding that age significantly affected these associations.
  • - Among 407 participants, LA contractility parameters, such as global longitudinal strain and stiffness, worsened with age and were correlated with a higher likelihood of SCIs and strokes.
  • - The results indicated that poorer LA function (as measured by strain) was associated with increased odds of SCIs and strokes as age increased, highlighting the importance of LA health in relation to brain health in elderly individuals.

Background: Limited data exist to characterize novel measures of left atrial (LA) structure and function in older adults without prevalent heart failure (HF).

Objectives: The aim was to assess reference range of LA measures, their associations with N-terminal pro-B-type natriuretic-peptide (NT-proBNP) and the related risk for incident HF or death.

Methods: We analyzed LA structure (LA maximal [LAViMax] and minimal volume indexed by body surface area) and function (LA emptying fraction, LA reservoir, conduit, and contraction strain) in 4,901 participants from the ARIC (Atherosclerosis Risk In Communities) study (mean age 75 ± 5 years, 40% male, and 19% Black) without prevalent HF.

Eur Heart J Cardiovasc Imaging

March 2022

Multidisciplinary Cardiovascular Research Centre (MCRC) and Leeds Institute of Cardiovascular and Metabolic Medicine, Biomedical Imaging Science Department, University of Leeds, Clarendon Way, Leeds LS2 9JT, UK.

Aims: To provide an overview of the role of cardiovascular (CV) imaging in facilitating and advancing the field of precision medicine in CV disease.

Dynamic Facial Reanimation in an Overweight Patient and with Significant Comorbidities: An Objective Analysis of Labbè Technique.

Surg J (N Y)

October 2021

FMUP - Faculdade de Medicina da Universidade do Porto, INEB - Instituto Nacional de Engenharia Biomédica, i3S - Instituto de Investigação e Inovação em Saúde, Porto, Portugal.

Gracilis free muscle transfer is considered the gold standard technique for facial reanimation in cases of facial palsy. However, it is limited by its long operative and recovery times, the need for a second surgical site, and its outcomes that can sometimes show midfacial bulk and oral commissure malposition. Facial reanimation with lengthening temporalis myoplasty (LTM)-Labbé technique- carries the advantage of having a shorter surgical time, a faster recovery, and being a less invasive surgery.

Background: To inform the on-going debate about the use of universal prescriptive versus national intrauterine growth charts, we compared perinatal mortality for small and large-for-gestational-age (SGA/LGA) infants according to international and national charts in Europe.

Methods: We classified singleton births from 33 to 42 weeks of gestation in 2010 and 2014 from 15 countries ( = 1,475,457) as SGA (birthweight <10th percentile) and LGA (>90th percentile) using the international Intergrowth-21st newborn standards and national charts based on the customised charts methodology. We computed sex-adjusted odds ratios (aOR) for stillbirth, neonatal and extended perinatal mortality by this classification using multilevel models.
