Background: Preeclampsia (PE) contributes to maternal and fetal mortality and morbidity. Supplementation with L-arginine, the precursor of nitric oxide, has not proven effective, possibly due to extensive arginine catabolism in the splanchnic bed. Citrulline is converted by the kidney to L-arginine.

Article Synopsis
  • This study investigated the incidence and risk factors for complete uterine rupture among women attempting vaginal birth after cesarean delivery (VBAC) over a 16-year period in France.
  • Out of 48,124 patients with a prior cesarean section, 65.8% attempted a VBAC, with a uterine rupture rate of 0.63%.
  • Key findings indicated that prior vaginal delivery decreased the risk of uterine rupture, while labor induction increased it; additionally, specific factors during spontaneous labor, like a low Bishop score and an arrest of cervical dilation, also heightened the risk.

Population Pharmacokinetics of Intravenous Paracetamol and Its Metabolites in Extreme Preterm Neonates in the Context of Patent Ductus Arteriosus Treatment.

Clin Pharmacokinet

December 2024

URP7323 Université Paris Cité, Pharmacologie et évaluations des thérapeutiques chez l'enfant et la femme enceinte, Hôpital Tarnier, 89 rue d'Assas, 75006, Paris, France.

Article Synopsis
  • The study aimed to analyze how paracetamol and its metabolites behave in extremely preterm neonates during treatment for patent ductus arteriosus and to identify factors influencing variability in individual responses.
  • Thirty preterm neonates receiving paracetamol were monitored, revealing that the drug was mostly metabolized through the sulfation pathway, which decreased with gestational age, while the glucuronidation pathway increased.
  • The results showed no link between the level of drug exposure and clinical outcomes or liver function indicators, suggesting that the dosages used might already achieve optimal effectiveness for ductus closure.

Aim: Infants born very preterm spend their early postnatal life in a neonatal intensive care unit, where irregular and unpredictable sounds replace the structured and familiar intrauterine auditory environment. Music interventions may contribute to alleviate these deleterious effects by reducing stress and providing a form of environmental enrichment.

Material And Methods: This was an ancillary study as part of a blinded randomised controlled clinical trial entitled the effect of music on preterm infant's brain development.

Background: Bronchopulmonary dysplasia (BPD) in extremely low gestational age neonates (ELGANs) was associated with neurodevelopmental impairment (NDI). However, the best endpoint of BPD assessment to predict subsequent NDI remains unclear.

Methods: We re-analyzed the PREMILOC trial, previously designed to test the effect of prophylactic hydrocortisone on survival without BPD at 36 weeks of postmenstrual age (BPD) in ELGANs, to compare predictive models of NDI considering baseline characteristics, respiratory course up to and BPD status at 36 or 40 weeks of postmenstrual age (BPD/BPD).

Device Closure of Hemodynamically Significant Patent Ductus Arteriosus in Premature Infants.

JACC Adv

October 2024

M3C-Necker, Department of Pediatric and Congenital Cardiology, Hôpital Necker-Enfants Malades, Assistance Publique-Hôpitaux de Paris (AP-HP), Paris, France.

The patent ductus arteriosus is a very common condition in preterm infants, and a hemodynamically significant patent ductus arteriosus increases morbidity and mortality in these vulnerable patients. However, despite numerous randomized controlled trials, there is no consensus regarding management. Medical therapy is typically offered as first-line treatment, although it yields limited success and carries the potential for severe adverse events.

Article Synopsis
  • The European Medicines Agency (EMA) is recommending precautionary measures due to concerns about neurodevelopmental disorders in children of men treated with valproate.
  • The UK's Medicines and Healthcare products Regulatory Agency (MHRA) went further, advising against prescribing valproate to anyone under 55.
  • Members of ENTIS and OTIS argue that these warnings are premature and that the scientific evidence does not strongly support the risk of paternal transmission of issues from valproate.

Eur J Pediatr

October 2024

Division of Pediatrics and Neonatal Critical Care, SMUR Pédiatrique 92, "Antoine Béclère" Hospital, Paris Saclay University Hospitals, APHP, Paris, France.

Article Synopsis
  • The paper compares the effectiveness of servo-controlled hypothermia (CTH) versus passive hypothermia in neonates with encephalopathy during transport, highlighting improvements in temperature management with CTH.
  • In the study, 100% of infants who received CTH reached the target temperature upon NICU arrival, contrasted with only 32% of those in passive hypothermia.
  • Findings suggest that CTH does not delay NICU arrival compared to passive methods, and there was no significant difference in short-term outcomes between the two groups.

The impact of hydroxyurea (HU) on the ovarian reserve of female patients with sickle cell disease (SCD) remains poorly elucidated. Only direct histological analysis of ovarian follicle density can effectively evaluate HU's effect on ovarian reserve. By analyzing digitized slides of ovarian tissue from girls and young women with SCD who underwent ovarian tissue cryopreservation (OTC) before hematological stem cell transplantation, we meticulously counted follicles and categorized them based on their growth stage.
