Cognitive impairment and malnutrition are prevalent in patients on hemodialysis (HD), and they negatively affect the outcomes of HD patients. Evidence suggests that cognitive impairment and malnutrition may be associated, but clinical studies to assess this association in HD patients are lacking. The aim of this study was to evaluate the association between cognitive impairment evaluated by the Montreal Cognitive Assessment (MoCA) score and nutritional status evaluated by the malnutrition inflammation score (MIS) in HD patients.

Article Synopsis
  • Patients on haemodialysis often experience high mortality due to subclinical hypoxic stress during sessions, making the oxygen extraction ratio (OER) a potential new prognostic factor.
  • A study observed that chronic HD patients showed a significant correlation between intra-dialysis OER changes (ΔOER), with those having a ΔOER of 40% or more facing a higher mortality rate (60% vs. 30%).
  • The findings suggest that monitoring ΔOER during treatment could help identify vulnerable patients at risk for poor outcomes.

Article Synopsis
  • Cholangiocarcinoma (CCA) is a rare type of biliary cancer that has increasing incidence and mortality, varying by subtype: intrahepatic (iCCA), perihilar (pCCA), and distal (dCCA), with different risk factors and clinical courses.
  • In a study with 2,234 patients from the ENSCCA Registry, results showed that iCCA is often linked to obesity and chronic liver issues, while pCCA involves primary sclerosing cholangitis, and dCCA relates to choledocholithiasis; most patients are diagnosed with advanced disease, complicating treatment outcomes.
  • Surgical resection provided the best survival rates, especially with clear margins, while patients with

Frataxin (FXN) deficiency is responsible for Friedreich's ataxia (FRDA) in which, besides the characteristic features of spinocerebellar ataxia, two thirds of patients develop hypertrophic cardiomyopathy that often progresses to heart failure and premature death. Different mechanisms might underlie FRDA pathogenesis. Among them, the role of miRNAs deserves investigations.

Background: Routine systems for monitoring vascular access (VA) performance are lacking. We developed a vascular access triage system to evaluate the monthly performance of the access, developed a specific score and assessed the association between score improvement and clinical outcomes.

Methods: Vascular access was triaged (Green, Yellow or Red) according to a score generated by dialytic and clinical parameters in all patients who, from 1/1/2014 to 31/03/2014, had been receiving haemodialysis treatment for at least 3 consecutive months in our Unit and who were then followed up for 4 years.

Article Synopsis
  • Liver fibrosis is an important prognostic factor in primary biliary cholangitis (PBC), and this study aimed to evaluate the effectiveness of vibration-controlled transient elastography (VCTE) in diagnosing advanced fibrosis (AF) at disease presentation.
  • A total of 167 PBC patients were analyzed, with VCTE demonstrating good diagnostic accuracy (AUROC of 0.89) for identifying AF, using specific liver stiffness measurement (LSM) cutoffs (≤6.5 kPa to rule out and >11.0 kPa to confirm AF).
  • The findings indicate that while VCTE can accurately distinguish between the presence and absence of AF using certain LSM cutoff values, liver biopsy remains necessary for patients with intermediate L

Oxygen extraction ratio to identify patients at increased risk of intradialytic hypotension.

Sci Rep

February 2021

Department of Translational and Precision Medicine, Nephrology Unit at Policlinico Umberto I Hospital, "Sapienza" University of Rome, Viale del Policlinico 155, 00161, Rome, Italy.

Intradialytic hypotension (IDH) is a hemodynamic phenomenon recently associated with decreased blood oxygen saturation (SO). The ratio between peripheral oxygen saturation (SpO) and central venous SO (ScvO) or Oxygen Extraction Ratio (OER), which represents a roughly estimate of the amount of oxygen claimed by peripheral tissues, might be used to estimate haemodialysis (HD) related hypoxic stress. Aim of this pilot study was to evaluate the relationship between OER increments during dialysis sessions (ΔOER) and episodes of IDH.

Inflammation, Oxidative Stress, and Bone in Chronic Kidney Disease in the Osteoimmunology Era.

Calcif Tissue Int

Division of Renal Medicine, Department of Clinical Science, Intervention and Technology, Karolinska Institutet, Stockholm, Sweden.

Bone is not only a mineralized and apparently non-vital structure that provides support for locomotion and protection to inner organs. An increasing number of studies are unveiling new biologic functions and connections to other systems, giving the rise to new fields of research, such as osteoimmunology. The bone marrow niche, a new entity in bone physiology, seems to represent the site where a complex crosstalk between bone and immune/inflammatory responses takes place.

Background: Doppler ultrasound (DU) monitoring early after arteriovenous fistula (AVF) creation allows the identification of low blood flow (Qa) requiring prompt revision, but it is costly (needs skilled operators and technical instruments) and is not available in all dialysis units. Therefore alternative first-line methods to measure Qa would be welcomed. We reasoned that once an AVF is created, an increment in central venous oxygen saturation (ScvO) is predictable and proportional to Qa.

Frataxin deficiency, responsible for Friedreich's ataxia (FRDA), is crucial for cell survival since it critically affects viability of neurons, pancreatic beta cells and cardiomyocytes. In FRDA, the heart is frequently affected with typical manifestation of hypertrophic cardiomyopathy, which can progress to heart failure and cause premature death. A microarray analysis performed on FRDA patient's lymphoblastoid cells stably reconstituted with frataxin, indicated HS-1-associated protein X-1 (HAX-1) as the most significantly upregulated transcript (FC = +2, P < 0.

Background: When medical therapy is unable to achieve biochemical control of secondary hyperparathyroidism, parathyroidectomy (PTX) is indicated, fortunately in a minority of patients. Thus, data on PTX prevalence and biochemical control are limited and, in particular in Italy, date back to 1999.

Methods: We designed a prospective, observational and multicenter study to collect data from dialysis units distributed throughout the Italian regions.

The definition of the number and nature of the signal transduction pathways involved in the pathogenesis and the identification of the molecules promoting metastasis spread might improve the knowledge of the natural history of osteosarcoma, also allowing refine the prognosis and opening the way to novel therapeutic strategies. Phosphatydil inositol (4,5) bisphosphate (PIP2), belonging to the Phosphoinositide (PI) signal transduction pathway, was related to the regulation of ezrin, an ezrin-radixin-moesin protein involved in metastatic osteosarcoma spread. The levels of PIP2 are regulated by means of the PI-specific Phospholipase C (PLC) enzymes.
