Background: Drug-induced phospholipidosis (DIPL) is an acquired lysosomal storage disorder characterized by the accumulation of lamellar bodies and phospholipids, typically associated with the use of cationic amphiphilic drugs (CADs). Over 200 marketed CADs, including widely prescribed β-blockers, have the potential to induce phospholipid deposition in various organs. In rare cases, DIPL may lead to secondary cardiomyopathy.

Article Synopsis
  • - Drug-eluting therapies, especially the Eluvia stent, have significantly reduced restenosis rates in treating peripheral artery disease, but concerns over aneurysm formation following their use are emerging.
  • - An 80-year-old man developed a giant aneurysm 27 months after receiving the Eluvia stent for a blocked artery, which was identified through ultrasound and treated with endovascular-covered stents.
  • - The presence of a 'low echoic area' near the stent is a common ultrasound finding after Eluvia implantation; while typically benign for two years, it can lead to serious complications like aneurysms due to factors such as overlapping stents and mechanical stress.

  • Undernutrition has a negative impact on patients with heart failure and preserved ejection fraction (HFpEF), affecting their long-term prognosis after discharge.
  • A study categorized 547 HFpEF patients based on their nutritional status at discharge and one year later, identifying those who normalized or worsened in their nutritional scores.
  • The findings revealed that worsening nutritional scores increased the risk of all-cause mortality or readmission, while improving scores were linked to better outcomes, with certain predictors identified for changes in nutritional status.

We report a case of onychomycosis due to Fusarium solani with gray-green staining, which improved after nail plate removal and antifungal liquid of effinaconazole. Fungal cultures revealed light-brown and dark-green colonies. Gray-green nail might have occurred due to the combination of these colonies, which necessitated differentiation from green nail.

Gut microbiota plays a crucial role in the development and progression of prostate cancer, with previous studies indicating that certain bacterial taxa are more abundant in castration-resistant prostate cancer (CRPC) compared to hormone-sensitive prostate cancer (HSPC). Notably, the composition of gut microbiota can vary significantly by geographic region, and Japanese individuals have a distinct microbial profile. However, research exploring these differences within Japanese populations remains limited.

Serum leucine-rich alpha-2 glycoprotein (LRG) can monitor disease activities during biologics treatment in patients with inflammatory bowel disease (IBD). It is unclear whether the pretreatment serum LRG level can predict clinical effectiveness including serum trough levels of ustekinumab in patients with IBD. This multicenter prospective cohort study included 184 patients (Crohn's disease, 104; ulcerative colitis, 80) who received ustekinumab (n = 119) or anti-tumor necrosis factor (n = 65) between January 2019 and March 2023.

  • The study investigates the safety and effectiveness of thoracic epidural analgesia (TEA) for pain management in patients undergoing minimally invasive cardiac surgery (MICS) via lateral minithoracotomy.
  • The analysis included data from 70 patients, revealing a low average pain score post-surgery and a chronic pain rate of 21% six months after discharge.
  • The findings suggest that TEA is a viable option for managing both acute and chronic pain post-MICS, without severe complications, as long as safety protocols are adhered to.

Background: With the help of an evidence-based approach called "Enhanced Recovery After Surgery" (ERAS), patients can receive standardised perioperative care and recover more quickly. Many surgical specialities, such as orthopaedics, gynaecological onco-surgery, breast surgery, urology, and colorectal surgery, use ERAS protocols extensively. Improved postoperative recovery is beneficial in lowering hospital stays, and costs, and increasing patient satisfaction.

Background: Traumatic brain injury (TBI) is a critical global health concern characterized by elevated rates of both morbidity and mortality. The pathological and physiological changes after TBI are closely related to microglia. Microglia, the primary immune cells in the brain, are closely linked to the mechanisms and treatment of TBI.

Purpose: To reveal the rate of 1-year freedom from restenosis and to determine the factors associated with the restenosis risk in femoropopliteal (FP) lesions treated with a Ranger drug-coated balloon (DCB) in real-world clinical settings.

Methods: This multicenter, prospective observational study enrolled 1131 patients and 1453 de novo or restenotic FP lesions (mean age=75±9 years; female=35.3%, mean lesion length=19.

Objectives: N-terminal pro brain natriuretic peptide (NT-proBNP) is a biomarker for myocardial stress that is used in diagnosing and prognosticating heart failure (HF). However, its interpretation is complicated by clinical factors. This study aims to clarify the prognostic value of NT-proBNP in patients with heart failure with preserved ejection fraction (HFpEF), and risk-prediction cutoffs considering various clinical factors.

Purpose: To evaluate the impact of intravascular ultrasound (IVUS)-evaluated tissue morphology on recurrence following drug-coated balloon (DCB) angioplasty for the treatment of femoropopliteal in-stent restenosis (FP-ISR).

Methods: This study was a single-center, retrospective, observational study. Study subjects were 65 FP-ISR lesions (mean lesion length: 165 ± 88 mm, occlusive restenosis: 25%) in 53 patients (age: 76 ± 8, diabetes mellitus: 66%) who underwent DCB angioplasty and whose IVUS data of tissue morphology were available.

  • - Volkmann's ischemic contracture involves permanent muscle and nerve damage from insufficient blood flow, leading to flexion contractures, whereas pseudo-Volkmann's contracture shares similar symptoms but can fully recover without ischemic damage.
  • - A case study describes a 39-year-old man who experienced finger extension failure after blunt forearm trauma, showing progressive symptoms over two months despite no broken skin or bones.
  • - Surgical intervention after two years revealed severe muscle adhesions due to the trauma, and immediate recovery was achieved after the adhesions were released, suggesting a diagnosis of pseudo-Volkmann's contracture.

  • Darolutamide combined with androgen-deprivation therapy (ADT) and docetaxel significantly lowered the risk of death by 32.5% in the ARASENS study, indicating its effectiveness for treating metastatic hormone-sensitive prostate cancer (mHSPC).
  • In the Japanese subgroup analysis of 148 patients, higher percentages of participants were older (≥75 years) and had lower body mass indexes compared to the overall study population, but the treatment showed a similar safety and efficacy profile.
  • While overall survival trends were positive for darolutamide in Japanese patients, the combined treatment was well tolerated, with no new safety concerns identified, despite some patients experiencing more frequent adverse events like neutropenia.

  • - This study compared open reduction and plate fixation (ORPF) with intramedullary nailing (IMN) for treating displaced midshaft clavicle fractures in active young adults aged under 60, focusing on outcomes like healing rates and complications.
  • - A total of 39 patients were analyzed, revealing that while ORPF provided faster recovery in the first 6 months, IMN had a higher healing rate (100% vs. 90%) and shorter average healing time (20.8 weeks vs. 23.1 weeks).
  • - The findings suggest that IMN is the better option for young adults with these fractures due to its higher healing rate, fewer complications, and less risk of needing additional surgeries, although both methods

The rapid restoration of blood flow in patients with acute myocardial infarction with ST elevation through percutaneous coronary intervention (PCI) is crucial for the survival of this population. Attempts to decrease the time from diagnosis of ST-segment elevation myocardial infarction (STEMI) to arrival at the catheterization laboratory have been extensively investigated. However, strategies during the procedure aiming to reduce the time to reperfusion are lacking.
