Key Clinical Message: Sarcoidosis-induced LETM represents a rare but life-threatening neurological manifestation of sarcoidosis, characterized by spinal cord inflammation, and associated neurological deficits. Sarcoidosis should be included in the differential diagnosis of LETM, particularly in patients with no lung involvement. Prompt recognition and management are obligatory to optimize outcomes and prevent long-term disability.

Introduction The prognostic significance of the controlling nutritional status (CONUT) score in hepatopancreatobiliary (HPB) surgery has been shown by many studies but the clinical significance of the CONUT score for postoperative short-term outcomes remains controversial. This study aimed to investigate the impact of the CONUT score on early postoperative outcomes in patients following major HPB surgery. Method This was a prospective study of 57 patients who underwent major HPB surgery from November 2019 to January 2021 at the Department of Surgical Gastroenterology, Tribhuvan University Teaching Hospital, Nepal.

Article Synopsis
  • A 43-year-old male with a history of Graves' disease developed neurological symptoms similar to a stroke, prompting an extensive evaluation for vasculitis and lymphoproliferative disease.
  • Imaging revealed narrowing of the left middle cerebral artery, leading to a diagnosis of probable cerebral vasculitis and treatment with methylprednisolone and cyclophosphamide.
  • Despite ongoing symptoms and further imaging showing additional narrowing, the treatment stabilized his condition, highlighting the diagnostic challenges of drug-induced ANCA-associated vasculitis.

Gram negative bacilli that are carbapenem resistant have emerged and are spreading worldwide. Infections caused by carbapenem resistant isolates posses a significant threat due to their high morbidity and mortality rates. Carbapenemases production by multi-drug resistant pathogens severely restricts treatment choices for illnesses caused by bacteria that are resistant to both carbapenems and majority of β-lactam antibiotics.

Article Synopsis
  • A systematic review and meta-analysis were conducted to explore the risk of cardiac complications, like arrhythmias and heart failure, in patients with ischemic stroke (pwIS).
  • The analysis included data from 5 studies, involving over 106,000 participants, and found that pwIS are significantly more likely to experience arrhythmias (with a hazard ratio of 4.2) and heart failure (hazard ratio of 2.9).
  • The study concluded that pwIS have a higher risk of cardiac issues, but further research is needed to better understand the extent of these complications.

  • The relationship between cancer and cardiovascular diseases (CVD), especially heart failure (HF), is being increasingly recognized for its impact on global health and mortality.
  • This review focuses on the intertwined risk factors—like lifestyle choices, genetics, and immune issues—that contribute to both conditions and their complications.
  • It emphasizes the need for further research on pathological links, the role of biomarkers in early detection, and the development of comprehensive treatment strategies to manage these interrelated health challenges.

Subarachnoid hemorrhage mimicking an acute migraine attack: A case report.

SAGE Open Med Case Rep

June 2024

Tribhuvan University, Institute of Medicine, Maharajgunj Medical Campus, Maharajgunj, Kathmandu, Nepal.

Article Synopsis
  • - Subarachnoid hemorrhage is a serious neurological emergency where blood leaks into the space surrounding the brain, most often due to a ruptured cerebral aneurysm, with high mortality rates between 8% to 67%.
  • - It can be misdiagnosed as it may present similarly to chronic migraine, but distinct symptoms like sudden severe headache, neck stiffness, and nausea can help differentiate between the two.
  • - A case study highlighted a 37-year-old woman with a history of migraines who experienced a sudden, intense headache, and imaging revealed a ruptured aneurysm, underscoring the need for careful evaluation in patients with similar symptoms.

Melbourne Veterinary School, Faculty of Science, The University of Melbourne, Parkville, Victoria, Australia. Electronic address:

A factor constraining the elimination of dog-mediated human rabies is limited information on the size and spatial distribution of free-roaming dog populations (FRDPs). The aim of this study was to develop a statistical model to predict the size of free-roaming dog populations and the spatial distribution of free-roaming dogs in urban areas of Nepal, based on real-world dog census data from the Himalayan Animal Rescue Trust (HART) and Animal Nepal. Candidate explanatory variables included proximity to roads, building density, specific building types, human population density and normalised difference vegetation index (NDVI).

Advancing opioid stewardship in low-middle-income countries: challenges and opportunities.

J Pharm Policy Pract

June 2024

Florence Nightingale Faculty of Nursing, Midwifery and Palliative Care, King's College London, London, United Kingdom.

The increased utilization of opioids in low- and middle-income countries (LMICs) presents a growing threat of opioid-related abuse, misuse and diversion. Pharmacists, as integral members of healthcare teams, are responsible for dispensing and monitoring opioid use and hold a pivotal role in opioid stewardship within LMICs. This commentary describes the potential and multifaceted roles of pharmacists in opioid stewardship in resource-constrained settings and highlights appropriate strategies for promoting responsible opioid utilization.

Article Synopsis
  • Snow-covered mountainous regions play a vital role in the hydrologic cycle, and changes in the cryosphere lead to extreme precipitation events, increasing the risk of flooding worldwide.
  • The increase in glacier melting results in the formation of glacial lakes, which can burst and cause catastrophic disasters, including flash floods that can destroy infrastructure and alter the environment.
  • Effective management of flooding in mountainous regions requires improved data collection, technology for climate resilience, and public awareness to mitigate risks and adapt to climate change.

Immersion pulmonary edema, also known as swimming-induced pulmonary edema (SIPE), manifests with cough, dyspnea, hemoptysis, and hypoxemia from flash pulmonary edema after surface swimming, often in healthy young individuals with no predisposing conditions. SIPE commonly resolves spontaneously within 24-48 hours but can be fatal. Post-mortem findings demonstrate heavy, edematous lungs and frothy airways.

Groundwater is the fundamental component of the hydrological system that acts as a major factor in comprehensions of the physical processes in both the land surface and the atmosphere. Determining groundwater, which directly affects the agricultural productivity of semi-arid mountainous regions, is crucial. Mountain ecosystems, once abundant with flowing water, now face immense pressure from a changing climate, evident in the drying of springs and the diminishing flow of groundwater.

Every clinical laboratory should ideally establish its own population-specific reference intervals (RIs) to promote precision and evidence-based medicine. However, clinical laboratories in Nepal find it easier to follow external RIs than establish their own, leading to a lack of RIs specific to the local population. This study thus aimed to establish RIs of routine LFTs for the adult population of Gandaki Province, Nepal, and compare them with the current RIs used by our laboratory.
