Recent Findings: Guidelines recommend adjunctive macrolide therapy as an option for patients with CAP, admitted to the hospital. A large data set collected both retrospectively and prospectively, including several recent randomized controlled trials (RCTs) have shown that adjunctive macrolide therapy can reduce mortality and improve outcomes in patients with severe CAP, more effectively than other alternative therapies.

Cefdinir is a broad-spectrum antibiotic with good antibacterial activity against gram-positive and gram-negative bacteria and can be used for the treatment of various sensitive bacterial infections, such as community-acquired pneumonia, urinary tract infection and gonorrhoea. Herein, a single-centred, randomized, open, single-dose, two-preparation, two-cycle, two-sequence, double-crossover trial with a 7-day washout was conducted to investigate the pharmacokinetics, bioequivalence and safety of cefdinir dispersible tablets and the reference formulation of cefdinir capsules in healthy Chinese volunteers. A 28-year-old male presenting with left-sided pleuritic chest pain, cough, palpitation, and fever with mild ST depression in II, III, and aVF, raised troponin I, troponin T, creatine phosphokinase-MB, and erythrocyte sedimentation rate was referred for F-18 2-fluoro 2-deoxyglucose positron emission tomography with noncontrast computed tomography ([ F]FDG-PET/CT) to rule out perimyocarditis. The first scan revealed incidental finding of [ F]FDG avid left lobar pneumonia and inadequate myocardial suppression, thus perimyocarditis could not be ruled out. The clinician was informed and after counseling, patient consented for a repeat study post-high fat-low carbohydrate diet.

We report for the first time whole-genome sequencing of four multidrug-resistant sequence type (ST) 307 recovered from patients in two hospitals in Armenia. Comparative genomic analysis revealed that the isolates were closely related, with a maximum of 39 single nucleotide polymorphism (SNP) differences in the core genome. All Armenian isolates carried the integrative and conjugative element ICE4, which bears the yersiniabactin locus, and shared a common evolutionary origin, diverging around 2005 (95% CI: 1999 to 2011).
