Background: The potential oncological benefit of extending the waiting period between neoadjuvant radiochemotherapy and surgical resection for rectal cancer is debated.

Objective: To evaluate the impact of prolonging this waiting period on the 5-year oncological prognosis and 2-year functional result of locally advanced rectal adenocarcinoma.

Design: Phase III, multicenter, randomized, open-label, parallel-group, controlled trial.

Cardiovascular diseases are the leading cause of death among women, and the incidence among younger women has shown the greatest increase over the last decades, in particular for acute myocardial infarction (AMI). Moreover, the prognosis of women post-AMI is poor when compared with men of similar ages. Since the 1990s, an abundant literature has highlighted the existing differences between sexes with regard to presentation, burden, and impact of traditional risk factors and of risk factors pertaining predominantly to women, the perception of risk by women and men, and the pathophysiological causations, their treatment, and prognosis.

Improving the hemocompatibility of centrifugal mechanical circulatory support devices at low flow rates.

Artif Organs

February 2025

Department of Cardiovascular and Thoracic Surgery, Institute of Cardiology, Pitié Salpêtrière Hospital, Assistance Publique Hôpitaux de Paris (AP-HP), Sorbonne University, Paris, France.

Background: An innovative solution has been recently proposed for the treatment of heart failure with preserved ejection fraction (HFpEF), using a centrifugal mechanical circulatory support (MCS) device. We sought firstly to assess the hemocompatibility of the proposed device. HFpEF treatment requires the blood pump to operate at low blood flow rate (0.

  • Niemann-Pick disease type C and Alzheimer's disease both show neurofibrillary tangles but are distinct disorders; this study investigated plasma phosphorylated-tau217 in these conditions.
  • The study found that individuals with Niemann-Pick disease type C had significantly higher levels of plasma phosphorylated-tau217 compared to controls, and its levels correlated with disease progression and lysosomal enlargement.
  • In comparison to Alzheimer's disease, while there was no significant difference in p-tau217 levels between the two diseases, Alzheimer's patients exhibited higher levels overall, suggesting p-tau217 may serve as a potential marker in Niemann-Pick disease type C.

Cannabidiol Treatment for Adult Patients with Drug-Resistant Epilepsies: A Real-World Study in a Tertiary Center.

Brain Behav

November 2024

AP-HP, Epilepsy Unit, Department of Neurology, Reference Center of Rare Epilepsies, ERN-EpiCare, Pitié-Salpêtrière Hospital, Paris, France.

Background And Purpose: Around 30% of patients with epilepsy show drug-resistant epilepsy (DRE). While cannabidiol has demonstrated efficacy as an adjunctive treatment in Dravet syndrome (DS), Lennox-Gastaut Syndrome (LGS), and epilepsy related to tuberous sclerosis complex (TSC), its more global effectiveness in adult patients with DRE apart from these three specific contexts needs to be clarified.

Methods: We conducted a retrospective study at the epilepsy unit of Pitié Salpêtrière Hospital.

  • Standard treatment for resectable peritoneal metastases (PM) involves cytoreductive surgery (CRS) and hyperthermic intraperitoneal chemotherapy (HIPEC), but recurrence rates are high, prompting consideration for repeat procedures in some patients.
  • A study analyzed 132 repeat CRS/HIPEC surgeries between 1994 and 2024, finding that most patients had multiple procedures with similar complications and recovery times, and no 90-day postoperative mortality was recorded.
  • Results indicated that while overall survival and recurrence-free survival varied, the procedure offers comparable outcomes for both initial and repeat surgeries, with improved survival observed, particularly for patients with peritoneal pseudomyxoma (

Kinetics of imidazole propionate from orally delivered histidine in mice and humans.

NPJ Biofilms Microbiomes

November 2024

Department of Internal and Vascular Medicine, Amsterdam University Medical Centers, Location AMC, Amsterdam, The Netherlands.

Imidazole Propionate (ImP), a gut-derived metabolite from histidine, affects insulin signaling in mice and is elevated in type 2 diabetes (T2D). However, the source of histidine and the role of the gut microbiota remain unclear. We conducted an intervention study in mice and humans, comparing ImP kinetics in mice on a high-fat diet with varying histidine levels and antibiotics, and assessed ImP levels in healthy and T2D subjects with histidine supplementation.

Background: Mechanical thrombectomy (MT) is an effective treatment for acute ischemic stroke from large vessel occlusion (LVO). While embolization to a new territory (ENT) after MT is well-documented, data on embolization in the same distal territory (EDT) are limited. Achieving modified Treatment In Cerebral Infarction (mTICI) 3 reperfusion presents significant clinical benefits over mTICI 2b/2c, necessitating strategies to reduce both ENT and EDT.

Science

November 2024

Institute for Human Genetics and Genomic Medicine, Medical Faculty, RWTH Aachen University, Aachen, 52074, Germany.

Malformations of the brain are common and vary in severity, from negligible to potentially fatal. Their causes have not been fully elucidated. Here, we report pathogenic variants in the core protein-folding machinery TRiC/CCT in individuals with brain malformations, intellectual disability, and seizures.

Background: In patients with a high clinical probability of pulmonary embolism (PE), the high prevalence can lower the D-dimer negative predictive value and increase the risk of diagnostic failure. It is therefore recommended that these high-risk patients should undergo chest imaging without D-dimer testing although no evidence supports this recommendation.

Objective: The objective was to evaluate the safety of ruling out PE based on D-dimer testing among patients with a high clinical probability of PE.

Electronic address:

Article Synopsis
  • The older population makes up about 50%-60% of newly diagnosed cancer patients, making it essential to consider their specific healthcare needs.
  • Older patients face higher risks of severe side effects from cancer treatments due to age-related factors and existing health conditions, leading to often being under-treated.
  • The review highlights the importance of understanding cancer drug pharmacology and advocating a cautious treatment approach to minimize dangerous toxicities in older patients.

Patterns of ictal surface EEG in occipital seizures: A simultaneous scalp and intracerebral recording study.

Clin Neurophysiol

December 2024

Reference Center for Rare Epilepsies, Neurology Department, University Hospital of Nancy, France; Clinical Neurosciences Research Project, Lorraine University, CNRS, UMR 7365, Nancy, France.

Article Synopsis
  • The study aimed to analyze the patterns of scalp EEG during occipital seizures (OS) and how they relate to simultaneous intracerebral EEG readings.
  • Researchers monitored 8 patients with confirmed OS, observing different seizure patterns including background activity suppression and high amplitude slow waves, with varied initial presentations across different regions of the brain.
  • The findings indicate that while scalp EEG can effectively pinpoint the onset of intracerebral seizures and carry lateralizing value, it does not always correlate with occipital electrode activity, emphasizing the complexity of seizure manifestations and aiding in better surgical planning for epilepsy treatment.

Background: Psychotic symptoms in adolescence are associated with social adversity and genetic risk for schizophrenia. This gene-environment interplay may be mediated by personality, which also develops during adolescence. We hypothesized that (i) personality development predicts later Psychosis Proneness Signs (PPS), and (ii) personality traits mediate the association between genetic risk for schizophrenia, social adversities, and psychosis.
