Chapter 11: TREATMENT MODALITIES.

Ann Endocrinol (Paris)

January 2025

Internal Medicine, Diabetes and Metabolic Disorders, University Hospitals of Strasbourg, Strasbourg University, Strasbourg, France.

Treatment modalities for primary hyperparathyroidism must take account of the expected benefits and risks of each treatment envisaged, before choosing the definitive option to be proposed to the patient. In this section, a Foreword puts in perspective the difficulties involved in choosing the criteria for a particular treatment method. Treatments are then considered one after the other: surgery, local destruction and medical management.

Prevalence of orofacial and head pain: an umbrella review of systematic reviews.

J Oral Facial Pain Headache

September 2024

Laboratory of Orofacial NeuroBiology (EA 7543), Faculty of Dentistry, Paris-Cité University, 92120 Paris, France.

Head pain (HP) and orofacial pain (OFP) are the most prevalent types of pain worldwide, encompassing cranial, oral and facial pain. The aim of this umbrella review was to answer the following questions: "What is the overall prevalence of HP/OFP and the different prevalences of HP/OFP conditions in adults and children?". We searched for studies investigating the prevalence of HP/OFP in four major databases and two databases from the grey literature, based on the following PECOS inclusion criteria: (P)opulation: Adults and children; (E)xposure: Orofacial or head pain conditions such as (1) dental, periodontal and gingival, (2) temporomandibular disorders (TMD), (3) neuropathic conditions, (4) headaches, and (5) idiopathic pain conditions; (C)omparison: None; (O)utcome: Prevalence; (S)tudies: Systematic reviews and/or meta-analyses.

A growing body of evidence highlights the importance of microglia, the resident immune cells of the CNS, and their pro-inflammatory activation in the onset of many neurological diseases. Microglial proliferation, differentiation, and survival are highly dependent on the CSF-1 signaling pathway, which can be pharmacologically modulated by inhibiting its receptor, CSF-1R. Pharmacological inhibition of CSF-1R leads to an almost complete microglial depletion whereas treatment arrest allows for subsequent repopulation.

Article Synopsis
  • The study analyzed muscular fitness trends in French youth aged 6-16 from 1999 to 2023, using data from over 53,000 participants and measuring standing broad jump (SBJ) and handgrip strength (HGS).
  • Results showed a slight decline in SBJ performance and a small improvement in HGS, indicating negligible overall changes in muscular fitness across the population.
  • There was an increase in variability and differences in performance levels, suggesting that while high performers are getting better, low performers are struggling, highlighting the need for targeted fitness interventions for weaker youth.

Lifestyle and Lipoprotein(a) Levels: Does a Specific Counseling Make Sense?

J Clin Med

January 2024

Hypertension and Cardiovascular Risk Research Center, Medical and Surgical Sciences Department, Alma Mater Studiorum University of Bologna, 40100 Bologna, Italy.

Lipoprotein(Lp)(a) is a variant of low-density lipoprotein (LDL), bound to apolipoprotein B100, whose levels are associated with a significant increase in the risk of atherosclerosis-related cardiovascular events, but also to aortic stenosis and atrial fibrillation. Since plasma levels of Lp(a) are commonly considered resistant to lifestyle changes, we critically reviewed the available evidence on the effect of weight loss, dietary supplements, and physical activity on this risk factor. In our review, we observed that relevant body weight loss, a relatively high intake of saturated fatty acids, the consumption of red wine, and intense physical exercise seems to be associated with significantly lower plasma Lp(a) levels.

Article Synopsis
  • Direct stenting (DS) may reduce complications during heart procedures but could increase the risk of stent issues compared to conventional stenting (CS).
  • In a study involving 446 STEMI patients, both stenting methods showed similar effectiveness in terms of stent size, blood flow post-procedure, and ST-segment resolution.
  • The findings concluded that, for pretreated STEMI patients, DS and CS had comparable outcomes for early recovery and vessel health after one year.
