Human treponemal infections are caused by a family of closely related Treponema pallidum that give rise to the diseases yaws, bejel, pinta and, most famously, syphilis. Debates on both a common origin for these pathogens and the history of syphilis itself has weighed evidence for the "Columbian hypothesis", which argues for an American origin, against that for the "pre-Columbian hypothesis", which argues for presence of the disease in Eurasia in the Medieval period and possibly earlier. While molecular data has provided a genetic basis for distinction of the typed subspecies, deep evolution of the complex has remained unresolved due to limitations in the conclusions that can be drawn from the sparse paleogenomic data currently available.

Background: Antibiotic therapy is currently considered a safe and effective treatment alternative for computed tomography (CT)-confirmed uncomplicated acute appendicitis with recent studies reporting good results on both oral antibiotics only and outpatient management. Furthermore, there are promising pilot results on uncomplicated acute appendicitis management with symptomatic treatment (placebo). This trial aims to assess whether both antibiotics and hospitalization can be safely omitted from the treatment of uncomplicated acute appendicitis.

The dripper clogging due to the development of biofilm can reduce the benefits of micro-irrigation technology implementation using reclaimed wastewater. The narrow cross-section and labyrinth geometry of the dripper channel enhance the fouling mechanisms. The aim of this study was to evaluate the water distribution and biofouling of drip irrigation systems at the field scale during irrigation with treated wastewater.

Article Synopsis
  • Antibiotics can effectively treat uncomplicated acute appendicitis without an appendicolith, but this study focuses on how the presence and characteristics of an appendicolith (a small calculus in the appendix) relate to the severity of appendicitis.
  • In a review of over 3,000 patients with suspected appendicitis, it was found that those with an appendicolith had a significantly higher rate of complications (47.1% vs. 21.5%) compared to those without.
  • Key findings indicated that certain features of the appendicolith, such as its size, location, and associated appendiceal wall changes, are linked to an increased risk of serious complications in appendicitis patients.

November 2024

Department of Internal Medicine, Yale School of Medicine, New Haven, CT, USA; Department of Cellular & Molecular Physiology, Yale School of Medicine, New Haven, CT, USA; Howard Hughes Medical Institute, Chevy Chase, MD, USA. Electronic address:

Article Synopsis
  • * There was no significant difference in mitochondrial oxidation rates between the MASL/MASLD groups and controls, indicating that fatty liver conditions do not impair this process.
  • * An increase in plasma glucagon levels enhanced mitochondrial oxidation rates by 50%-75% in both groups and boosted glucose production by about 50% specifically in the MASL group, partially through increased mitochondrial activity.

Cancer Stem Cells and Fibroinflammatory Microenvironment Group, Cancer Department, Instituto de Investigaciones Biomédicas (IIBM) Sols-Morreale CSIC-UAM, Madrid, Spain

Article Synopsis
  • The study focuses on pancreatic ductal adenocarcinoma (PDAC) and how cancer stem cells (CSCs) contribute to its aggressive nature and resistance to therapies, particularly immune checkpoint inhibitors.
  • Researchers used a mouse model and primary tumor cell lines to identify CSC populations and their immune evasion strategies, discovering that the gene peptidoglycan recognition protein 1 (PGLYRP1) is significantly overexpressed in these cells.
  • The findings suggest PGLYRP1 plays a key role in helping CSCs evade immune responses, highlighting its potential as a new target for immunotherapy in PDAC patients.

Importance: Current short-term evidence has shown that uncomplicated acute appendicitis can be treated successfully with oral antibiotics alone, but longer-term results are lacking.

Objective: To assess the treatment effectiveness of oral antibiotic monotherapy compared with combined intravenous (IV) and oral antibiotics in computed tomography-confirmed uncomplicated acute appendicitis at a longer-term follow-up.

Design, Setting, And Participants: This secondary analysis of a predefined year 3 follow-up of the Appendicitis Acuta II (APPAC II) noninferiority, multicenter randomized clinical trial compared oral moxifloxacin with combined IV ertapenem plus oral levofloxacin and metronidazole for the treatment of uncomplicated acute appendicitis.

  • The Galapagos rail has lost flight ability since colonizing the Galapagos Islands about 1.2 million years ago and has faced severe habitat loss due to overgrazing by introduced goats, resulting in declining populations and reduced genetic diversity.
  • Researchers investigated the evolutionary history and genetic distribution of Galapagos rails across different islands, revealing distinct populations and isolation events linked to the separation of landmasses.
  • The study highlights that long runs of homozygosity in the rails' genomes likely stem from historical population bottlenecks and emphasizes the importance of goat eradication to prevent inbreeding in surviving rail populations.
