Background: Participant recruitment in rural and hard-to-reach (HTR) populations can present unique challenges. These challenges are further exacerbated by the need for low-cost recruiting, which often leads to use of web-based recruitment methods (eg, email, social media). Despite these challenges, recruitment strategy statistics that support effective enrollment strategies for underserved and HTR populations are underreported.

Introduction: Recent expansion of cannabis legalization in multiple states calls for reexamination of the prevalence of cannabis use, cannabis use disorder (CUD), and the associations between CUD severity and substance use treatment. We used Andersen's behavioral model of healthcare use as the conceptual/analytic framework for examining treatment use.

Methods: We used data from the 2022 National Survey on Drug Use and Health (NSDUH; N = 47,100, age 18+) to describe the prevalence of past-year cannabis use, CUD and CUD severity, other substance use disorders, and substance use treatment.

Myeloid sarcoma (MS) represents a neoplastic proliferation characterized by immature myeloid precursor cells. Among its variants, aleukemic MS is an uncommon subtype, manifesting as skin involvement sparing the peripheral blood or bone marrow. The non-specific cutaneous presentation coupled with the lack of associated symptoms poses a diagnostic challenge for providers.

Association of Operative Approach With Postoperative Outcomes in Neonates Undergoing Surgical Repair of Esophageal Atresia and Tracheoesophageal Fistula.

J Pediatr Surg

November 2024

Division of Pediatric Surgery, Nemours Children's Health, Wilmington, DE, United States; Department of General Surgery, Thomas Jefferson University Hospital, Philadelphia, PA, United States. Electronic address:

Article Synopsis
  • The study investigates the outcomes of minimally invasive surgery (MIS) compared to open surgery for repairing esophageal atresia/tracheoesophageal fistula (EA/TEF) in neonates, utilizing data from 2013 to 2020.
  • A total of 1,738 neonates were analyzed, revealing that although MIS use increased over time, it was linked to longer surgical times and higher rates of overall postoperative interventions.
  • Despite these findings, there was no significant difference in overall morbidity between the two surgical approaches, suggesting a need for further research to assess the long-term effects of MIS on EA/TEF repairs.

Purpose: Malalignment of distal tibia fractures can lead to malunion/nonunion or alter the limb mechanical axis which may cause arthritis. Proposed methods to decrease malalignment include fibular fixation or multiplanar interlocking screws, however these remain controversial. This study aimed to identify factors associated with malalignment in distal tibial fractures with associated fibular shaft fractures.

Amblyopia and physical activity in adolescents.

Strabismus

December 2024

Department of Pediatric Ophthalmology, Wills Eye Hospital, Philadelphia, PA.

: Physical activity in adolescence plays an important role in reducing the lifetime risk of chronic disease. Recent studies have shown that individuals with amblyopia may have difficulties with coordinating balance and may be at greater risk of developing fine motor impairments. The purpose of this study was to determine if there is an association between the presence of amblyopia and physical activity (PA) in adolescents.

Autoimmune encephalitis (AE) is a rare immune-mediated disorder comprised of non-infectious neuroinflammatory disease processes. Clinical presentation overlaps with a broad range of neurodegenerative disorders and infectious encephalitis; therefore, AE remains a diagnosis of exclusion. Patients may present with nonspecific symptoms such as psychiatric disturbances, cognitive deficits, seizures, movement disorders, and confusion.

Thulium laser extraction of angioembolization coils in patient presenting with nephrolithiasis: A case report.

Urol Case Rep

September 2024

Northeast Georgia Physicians Group Urology, Northeast Georgia Health System, Gainesville, USA.

Renal Artery Embolization (RAE) is a first-line treatment for arteriovenous fistula complications, which may occur following percutaneous nephrolithotomy (PCNL). A rare complication of RAE is metal coil migration, which may present with nephrolithiasis symptoms, including hematuria and flank pain. Imaging like Computed Tomography (CT) Angiography can help assess the risk of hemorrhage due to coil location relative to vasculature Subsequently, ureteroscopy can be done with thulium laser excision of the coils and lithotripsy of adherent stone.

Introduction: Fourth-year away rotations are an important modifiable variable proven to increase students' opportunities to match into orthopaedic surgery. The purpose of this study was to determine differences in away rotation eligibility requirements and cost of rotation between allopathic and osteopathic students during the 2023 application cycle. Eligibility requirements and fees were then compared with the 2021 application cycle.

Objectives: Telemedicine has become a widely accepted alternative to face-to-face patient encounters. Although there have been several peer-reviewed journal articles on incorporating telemedicine into the medical school curriculum, particularly during the COVID-19 pandemic, assessments of the effectiveness of remote supervision of medical students have not been reported. This prospective cohort study of student subjects using observational survey data evaluated the efficacy of telemedicine as an educational resource by comparing learning outcomes between osteopathic medical students receiving direct (physically present) supervision with a group who received remote (telemedicine) supervision by clinical faculty within a post-acute/long-term care (PA/LTC) setting.

Our understanding of the DNA damage responses of human cells to radiation has increased remarkably over the recent years although some notable signaling events remain to be discovered. Here we provide a brief account of the key molecular events of the responses to reflect the current understanding of the key underlying mechanisms involved.

The abstract of a research paper functions to attract readers and highlight the clinical significance of a research project in a broadly appealing manner. Abstract structure is commonly dictated by the target journal, however, a basic style typically follows the "Introduction, Methods, Results and Discussion" structure of introduction, materials/methods, results, and discussion/conclusion. The abstract itself is commonly the initial accessible portion of a research paper, so writing in an engaging while informative manner is imperative for increasing manuscript views and citations.

Metastasis is the primary culprit behind cancer-related fatalities in multiple cancer types, including prostate cancer. Despite great advances, the precise mechanisms underlying prostate cancer metastasis are far from complete. By using a transgenic mouse prostate cancer model (TRAMP) with and without Phf8 knockout, we have identified a crucial role of PHF8 in prostate cancer metastasis.

Total joint arthroplasties (TJA) are some of the most commonly performed surgeries in the United States with the number of TJA expected to rise significantly over the next decade as the population ages and arthritic burden worsens. However, the rise in TJA volume correlates with a heightened risk of complications, notably prosthetic joint infections (PJI), despite their low occurrence rate of less than 2%. PJI imposes a significant burden on surgery success, patient well-being, and healthcare costs, with an estimated annual expense of 1.

Article Synopsis
  • An estimated 5 billion people worldwide do not have access to safe surgical care, highlighting the need for increased interest in global surgery among medical students.
  • A survey of 708 medical students across 38 US institutions revealed that 34.6% expressed interest in global surgery, with demographic factors like Hispanic/Latino ethnicity and birthplace influencing this interest.
  • Prior experience in low or middle-income countries, along with public health and international service activities, were significant predictors of students' interest in global surgery, suggesting that medical educators should enhance global health opportunities for students.

Androgen-receptor-negative, androgen-independent (AR-AI) prostate cancer aggressively proliferates and metastasizes, which makes treatment difficult. Hence, it is necessary to continue exploring cancer-associated markers, such as oncofetal Receptor Tyrosine Kinase like Orphan Receptor 1 (ROR1), which may serve as a form of targeted prostate cancer therapy. In this study, we identify that Penta-O-galloyl-β-D-glucose (PGG), a plant-derived gallotannin small molecule inhibitor, modulates ROR1-mediated oncogenic signaling and mitigates prostate cancer phenotypes.
