Four Novel Disease-Causing Variants in the Gene in Russian Patients with CADASIL.

Background: Cerebral autosomal dominant arteriopathy with subcortical infarcts and leukoencephalopathy (CADASIL) is an inherited disease with unknown mechanisms and a broad phenotypic spectrum. It is caused by pathogenic variants in the gene. The symptoms of the disease mainly include recurrent strokes with vascular risk factors, migraine with aura, dementia, and mood disturbances.

[Delayed complication of surgical treatment of duodenal atresia in neonatal period].

We report a 48-year-old woman who underwent surgery in early neonatal period for duodenal atresia and developed subsequent diseases of the upper gastrointestinal tract. Symptoms of gastric outlet obstruction, gastrointestinal bleeding and malnutrition developed over the past 5 years. Inflammatory and cicatricial lesions of gastrojejunostomy formed for congenital duodenal obstruction following annular pancreas required reconstructive surgery.

Aim: Study the impact of various combinations of comorbid original diseases in patients infected with COVID-19 later on the disease progression and outcomes of the new coronavirus infection.

Materials And Methods: The ACTIV registry was created on the Eurasian Association of Therapists initiative. 5,808 patients have been included in the registry: men and women with COVID-19 treated at hospital or at home.

A randomised trial of anti-GM-CSF otilimab in severe COVID-19 pneumonia (OSCAR).

Background: Granulocyte-macrophage colony-stimulating factor (GM-CSF) and dysregulated myeloid cell responses are implicated in the pathophysiology and severity of COVID-19.

Methods: In this randomised, sequential, multicentre, placebo-controlled, double-blind study, adults aged 18-79 years (Part 1) or ≥70 years (Part 2) with severe COVID-19, respiratory failure and systemic inflammation (elevated C-reactive protein/ferritin) received a single intravenous infusion of otilimab 90 mg (human anti-GM-CSF monoclonal antibody) plus standard care (NCT04376684). The primary outcome was the proportion of patients alive and free of respiratory failure at Day 28.

[Liver echinococcosis in a non-endemic region].

Objective: To study clinical manifestations and difficulties in diagnosis of liver echinococcosis in a non-endemic region and to assess the long-term results of surgical treatment and quality of life.

Material And Methods: The study included 36 residents of a non-endemic region with liver echinococcosis operated on in the Perm Regional Clinical Hospital over the past 15 years. IgG antibodies to echinococcal antigens were determined using enzyme immunoassay.

Efficacy of combined immunosuppression with or without eltrombopag in children with newly diagnosed aplastic anemia.

We compared the efficacy and safety of eltrombopag (ELTR) combined with immunosuppressive therapy (IST) and IST alone in treatment-naïve children with severe (SAA) and very severe (vSAA) aplastic anemia. Ninety-eight pediatric patients were randomized to receive horse antithymocyte globulin (hATG) and cyclosporin A (CsA) with (n = 49) or without (n = 49) ELTR. The primary endpoint was the overall response rate (ORR) at 4 months.

[Case of extra-anatomical prosthetic repair in local infection of a bifurcated aortofemoral graft].

Presented in the article is a clinical case report regarding successful treatment of a patient with infection of a vascular graft after bifurcation aortofemoral bypass grafting by means of partial removal of the graft's branch with extra-anatomical graft-to-femur prosthetic repair through the iliac wing. The patient was admitted 6 months after bifurcation aortofemoral bypass grafting with a purulent and ligature fistula, discharge in the inguinal area. The findings of computed tomography showed no infection of the central anastomosis in the retroperitoneal space, with however periprosthetic infection in the area of the distal branch and severe comorbid background, thus not allowing complete removal of the prosthesis.

[Five year experience in ibrutinib therapy for relapsed and refractory mantle cell lymphoma in real world Russian clinical practice].

Background: Mantle cell lymphoma (MCL) is a rare and clinically aggressive lymphoma subtype. Current approaches have greatly improved patients outcomes, but relapse is inevitable. In phase IIIII clinical trials, ibrutinib has shown significant activity in patients with relapsed or refractory (R/R) MCL.

[Results of loop endarterectomy and femoropopliteal bypass grafting in TASC C and D lesions].

Aim: The aim of our investigation was to analyse the results of treatment of patients with atherosclerotic lesions of the femoropopliteal segment by means of femoropopliteal bypass grafting or loop endarterectomy within the framework of a single-centre retrospective study.

Patients And Methods: The study included a total of 177 patients who were divided into two groups. Group One was composed of 108 patients subjected to loop endarterectomy from the arteries of the femoropopliteal segment, and Group Two comprised 69 patients who endured femoropopliteal bypass surgery using a synthetic graft.

Analysis of the Mortality of Russian Patients With Chronic Myeloid Leukemia in the Multicenter EUTOS ELN Population-based Study (EUTOS-PBS).

Introduction: Russia took part in the multicenter population-based study (Europe) and included 6.8% adult patients with newly diagnosed chronic myeloid leukemia (CML). The objective of this study was to analyze the mortality in the Russian cohort of patients with newly diagnosed CML in the EUTOS PBS observational study.

[CADASIL syndrome: differential diagnosis with multiple sclerosis].

Two cases of clinical and MRI manifestations of genetically verified CADASIL syndrome in female patients under 40 years of age are presented. The primary misinterpretation of clinical data and the neuroimaging results within multiple sclerosis indicates a lack of awareness of radiologists and neurologists about this disease. The article reviewed the current literature on the problems of diagnosis and treatment of CADASIL.

[Efficacy of using loop endarterectomy].

Endarterectomy is considered to be the most physiological technique of correcting arterial blood flow, since it does not require implantation of a synthetic material, with blood flow restored through its own bed. However, surgeons have long sought to work out the most effective modification of endarterectomy, which eventually resulted in creation of a method known as loop endarterectomy. Mention should be made that some researchers report patency which may be comparable to that of autovenous bypass grafting, while others report negative results of using loop endarterectomy.

[Aneurysmal Degeneration of a Bioprosthesis].

We present a clinical case of successful correction of aneurismatic dilatation of the anastomosis zone after allotransplant implantation. In 2004 this patient underwent implantation of an arterial allotransplant of "complex architectonics" because of trauma of the iliofemoral arterial segment. After 12 years he reported appearance of discomfort in the left side of the abdomen.

[Surgical correction of retroaortic position of the left renal vein].

Described herein is a variant of surgical reconstruction performed in a male patient with a retroaortic position of the left renal vein by means of creating a vein-allograft-vein configuration. Unfortunately, to date, there are no generally accepted recommendations on proper treatment of patients presenting with similar pathology. A situation wherein it is impossible to create an intervenous anastomosis (because of a significantly altered portion of the vein and prestenotic dilataion) dictates the necessity of searching for new variants of surgical management.

[The case-report of progressive multifocal leukoencephalopathy in a patient with multiple sclerosis treated with natalizumab].

The article describes the first case of progressive multifocal leukoencephalopathy in a patient with multiple sclerosis, developed during treatment with natalizumab. The causes, possibly, influencing the outcome of the disease, are analyzed.

[The clinical and microbiological characteristics of oropharyngeal candidiasis in the HIV-infected patients at the late stages of the disease].

Oropharyngeal candidiasis in 512 HIV-infected patients at the late stages of the disease was studied with special reference to the clinical and microbiological characteristics of this condition. The diagnosis was established based on the results of the clinical and microbiological examination of the patients including investigation of the tissue samples taken from the oral cavity and the throat with the use of the device specially developed for this purpose. It was shown that the disease existed in various clinical forms the most common of which were monocomponent pathology represented by pseudomembranous candidiasis in 37.

[Effect of antioxidant therapy on neurotrophins and processes of rehabilitation after stroke].

The aim of the research was to study the effect of the inclusion in the scheme cytoflavin patient care during the recovery period of ischemic stroke in the neuropsychological changes in the status and content of neurotrophins in serum. For this purpose we surveyed 52 patients who underwent a first ischemic stroke (29 women and 23 men) aged 52-74 years. Patients were divided into 2 groups: primary (25 patients) received in addition to basic therapy cytoflavin: intravenously at 20 ml per 400 ml of 5% glucose solution, 1 time a day for 10 days, then into 2 tablets 2 times a day for half an hour before food for a month, and the comparison group (27 patients) who received standard treatment.
