  • - Hidradenitis suppurativa (HS), or acne inversa, is a chronic inflammatory condition affecting the body's apocrine glands, resulting in painful nodules and abscesses primarily in areas of skin friction; it impacts about 4% of the U.S. population, especially women in their 20s and 30s, notably within the African American community.
  • - The condition is diagnosed using the Hurley staging system, which helps determine treatment options like antibiotics, biologics (e.g., adalimumab), and surgical interventions.
  • - A case report highlights a 30-year-old African American male with perianal HS, illustrating the long-term complications and management challenges associated with this often misdiagnosed

Cyclic neutropenia is a rare hematological condition characterized by periodic fluctuations in neutrophil counts, with a 21-day periodicity. Clinical presentation varies from mild to severe forms of the disease, with the onset of recurrent fever, painful oral ulcers, recurrent bacterial infections, peritonitis, and septic shock. The availability of granulocyte colony-stimulating factor (G-CSF) has revolutionized the management and natural history of this disease, regulating the proliferation, differentiation, and maturation of the progenitor cells, and reducing the duration of neutropenia.

November 2023

Division of Dermatology, Department of Paediatrics, The Hospital for Sick Children, University of Toronto, 555 University Avenue, Toronto, ON, M5G 1X8, Canada.

Hidradenitis suppurativa (HS) is a chronic, recurrent, inflammatory skin condition that manifests as painful, deep-seated, inflamed nodules and abscesses in the axillary, groin, perianal, perineal, and inframammary regions. The associated pain, malodour, and disfigurement contribute to its profound negative impact on psychosocial spheres and overall quality of life in affected individuals. Although the symptoms of HS classically begin in the second or third decade of life, HS affects children and adolescents as well.

  • The Buschke-Löwenstein tumor (BLT) is a rare HPV-related condition, primarily affecting men, characterized by aggressive yet benign growth, a significant rate of malignant transformation, and high recurrence after treatment.
  • Treatment options include various surgical and nonsurgical methods, with the HPV vaccine helping to lower the incidence of BLT.
  • A recent case study highlights successful surgical removal of a large perianal BLT in a 53-year-old man, with effective reconstruction and no need for a protective stoma.

Article Synopsis
  • The study aims to explore how clinical characteristics and treatment methods for perianal abscesses impact the likelihood of postoperative issues like recurrence or anal fistula formation.
  • Data from 394 patients were analyzed, revealing significant differences between cured and uncured groups in factors like preoperative blood results, abscess space, MRI usage, and surgical methods.
  • The findings suggest that assessing the size of the abscess and using precise surgical techniques can significantly enhance cure rates following treatment.

Article Synopsis
  • - Hidradenitis suppurativa (HS) is a chronic inflammatory disease affecting about 1-2% of the population, though it’s likely underreported due to various factors including diagnosis challenges and data collection differences.
  • - The condition leads to painful nodules and abscesses mostly in areas like the armpits and groin; chronic inflammation can cause severe tissue damage and scarring over time.
  • - Current treatments range from topical and systemic medications to surgical options, with adalimumab being the only FDA-approved biologic, but it works in only about half of patients; ongoing research explores new therapies targeting various pathways for better outcomes.

Article Synopsis
  • This study evaluates the safety and effectiveness of endoscopic ultrasound-guided pelvic abscess drainage (EUS-PAD) in treating 14 patients with pelvic abscesses in one hospital.
  • All procedures were successful, with most patients discharged within an average of 21.4 days, indicating that EUS-PAD is a reliable option for treating acute infections.
  • Though effective for many, some patients with uncontrolled conditions experienced worsened abscesses, suggesting that careful selection criteria are essential for this procedure's success.

Perianal fistulas in Crohn's disease (CD) are a poor prognostic phenotype requiring a combination of medical and surgical management. Perianal fistulas in CD are characterized by more complex and multi-branched fistulas, association with skin tags, and frequent presence of proctitis. A comprehensive approach with clinical examination, endoscopic and MR assessment is required, and in particular, MR interpretation provides detailed information on the type of fistula with its internal component and activity, secondary tracts and extension, internal, external openings, associated abscess, and presence of proctitis.

New Perianal Sepsis Risk Score Predicts Outcome of Elderly Patients with Perianal Abscesses.

J Clin Med

August 2023

Department of General, Visceral, Thoracic, Transplant and Pediatric Surgery, University Hospital of Giessen, Rudolf-Buchheim Strasse 7, 35390 Giessen, Germany.

Antibiotic therapy following surgical perianal abscess drainage is debated, but may be necessary for high-risk patients. Frailty has been shown to increase the risk of unfavorable outcomes in elderly surgical patients. This study aims to identify high-risk patients by retrospectively analyzing a single-center cohort and using a pretherapeutic score to predict the need for postoperative antibiotics and extended nursing care following perianal abscess drainage surgery.
