Ostomy Management for Pediatric Perianal Crohn's Disease.

J Pediatr Surg

June 2024

General, Thoracic, and Fetal Surgery, Children's Hospital of Philadelphia, Philadelphia, PA, USA; FAAP, USA.

Introduction: Pediatric patients with perianal Crohn's Disease (CD) suffer recalcitrant fistulas, abscesses, and strictures. Fecal diversion is a palliative last resort, but the expected clinical course and long-term management of the ostomy for this population is unclear. We sought to identify factors predictive of ostomy takedown and establish management recommendations for fistulizing and stenosing disease.

Liposarcomas are a prevalent subtype of soft tissue sarcomas, constituting less than 1% of all malignancies. Originating in the adipose tissue, they can manifest in various locations and are categorized by the World Health Organization into several subtypes: well-differentiated liposarcomas, dedifferentiated liposarcomas, myxoid liposarcomas, pleomorphic liposarcomas and mixed-type liposarcomas. These tumors typically affect middle-aged and older individuals, and their incidence has been progressively increasing over the years.

Postoperative packing of perianal abscesses following incision and drainage.

Br J Hosp Med (Lond)

November 2023

Department of General Surgery, Ealing Hospital, London North West University Healthcare, NHS Foundation Trust, London, UK.

A best evidence topic in general surgery was written according to a structured protocol, to address the question: in adult patients with perianal abscesses, should postoperative wound packing be undertaken considering the rates of pain experienced, wound healing and abscess recurrence? The literature search identified 159 papers on Ovid, Embase and Medline and 48 on PubMed. These were independently screened, and three articles were included in this review as these offered the best information to answer the question. One was a systematic review without meta-analysis, one was a randomised controlled trial and one was a multicentre observational study.

Article Synopsis
  • Perianal abscesses are common conditions that may require surgery, but some patients struggle with slow healing or complications like fistulas after the procedure.
  • A case study highlighted a patient who experienced unhealed wounds for over three months following surgery, prompting the use of Platelet-rich plasma (PRP) as a treatment.
  • After receiving PRP treatment five times, the patient’s wound healed within 15 days, suggesting that PRP can be an effective option for improving recovery in patients with difficult-to-heal surgical wounds.

Resection of anorectal fistula cancer associated with Crohn's disease after preoperative chemoradiotherapy: a case report.

Surg Case Rep

November 2023

Department of Gastroenterological Surgery, Graduate School of Medicine, Osaka University, 2-2 Yamadaoka E-2, Suita, Osaka, 565-0871, Japan.

Background: Anorectal fistula cancer is often diagnosed in an advanced state, and radical resection is difficult when invasion of the pelvic wall is observed. In addition, there is currently no clear evidence for perioperative treatment of locally advanced cases. We report a case of anorectal fistula cancer with widespread infiltration diagnosed during the course of Crohn's disease, which was curatively resected after preoperative chemoradiotherapy.

Dose escalation of adalimumab as a strategy to overcome anti-drug antibodies: A case report of infantile-onset inflammatory bowel disease.

World J Gastroenterol

October 2023

Pediatric Gastroenterology and Endoscopy Unit, IRCCS Istituto Giannina Gaslini, Genova 16147, Italy.

Article Synopsis
  • Treating infantile-onset inflammatory bowel disease (IO-IBD) can be difficult because standard treatments often fail, necessitating the use of biologics like anti-tumor necrosis factor therapy.
  • A case study of a 5-month-old boy with IO-IBD showed that therapeutic drug monitoring (TDM) for high-dose adalimumab (ADA) helped manage the disease, especially after the patient developed antibodies against the drug.
  • The successful adjustment in dosing led to the disappearance of these antibodies and sustained remission, suggesting that TDM-guided high-dose therapy could be a viable alternative to combination treatments, particularly for very young patients.

Article Synopsis
  • - This study explores the natural course of perianal abscess (PA) in infants who received conservative treatment instead of surgery, focusing on outcomes over a median follow-up of 5.3 years.
  • - Out of 153 infant patients, 90.8% were cured with conservative treatment, while around 10% eventually needed surgery due to ongoing issues like fistula formation or abscess recurrence.
  • - The results suggest that PA in infants is often a self-limiting condition that can be effectively managed without surgery, highlighting the importance of conservative management as the first line of treatment.

Cyclic neutropenia is a rare hematological condition characterized by periodic fluctuations in neutrophil counts, with a 21-day periodicity. Clinical presentation varies from mild to severe forms of the disease, with the onset of recurrent fever, painful oral ulcers, recurrent bacterial infections, peritonitis, and septic shock. The availability of granulocyte colony-stimulating factor (G-CSF) has revolutionized the management and natural history of this disease, regulating the proliferation, differentiation, and maturation of the progenitor cells, and reducing the duration of neutropenia.
