Purpose: Targeted prostate biopsies are increasingly being performed by urologists in the United States including those in the Pennsylvania Urologic Regional Collaborative, a physician-led data-sharing and quality improvement collaborative. To evaluate the performance of MRI guided fusion needle prostate biopsies in the collaborative, we analyzed the variability by practice in rates of detection of clinically significant prostate cancer and patient characteristics associated with detection of clinically significant prostate cancer.

Methods: We analyzed 857 first-time MRI fusion biopsy procedures performed at five practices (minimum 20 procedures) between 2015 and 2019.

Objective: To combat the heightened risk of opioid overdose death for individuals with criminal justice involvements, enhanced access to Medicaid remains paramount. This study examines the effect of a 2017 policy change in Pennsylvania that allowed for suspension, rather than termination, of Medicaid coverage while in prison on post-release opioid overdose mortality risk (OOMR) for adults released from Pennsylvania prisons.

Methods: This retrospective cohort study utilizes administrative records from the Pennsylvania Department of Corrections for adults ≥18 years diagnosed with opioid use disorder (OUD) released in either 2015 or 2018.

Article Synopsis
  • The aldosterone renin ratio (ARR) is essential for deciding whether adrenal vein sampling (AVS) is necessary in diagnosing primary hyperaldosteronism.
  • The case emphasizes the importance of interpreting plasma renin and aldosterone levels within context rather than relying solely on imaging.
  • Relying only on imaging without conducting AVS can lead to unnecessary surgeries, such as adrenalectomy on the wrong side or even an unnecessary surgery altogether.

  • This study analyzes the distribution of otolaryngologists in urban versus rural counties in Pennsylvania using a retrospective database approach.
  • A total of 388 otolaryngologists were identified, with the majority located in metropolitan areas, leaving a significant shortage in rural counties except for those with large academic centers.
  • The findings reveal that while the average number of otolaryngologists is similar in both urban and rural settings, many nonmetropolitan counties lack any specialists, highlighting a disparity in healthcare access.

Aldehyde dehydrogenases (ALDHs) are a family of enzymes that aid in detoxification and are overexpressed in several different malignancies. There is a correlation between increased expression of ALDH and a poor prognosis, stemness, and resistance to several drugs. Several ALDH inhibitors have been generated due to the crucial role that ALDH plays in cancer stem cells.

  • - The study aimed to analyze the volume of facial plastic and reconstructive surgeries (FPRS) before, during, and after the COVID-19 pandemic lockdown, using data from over 68 million individuals from 2017 to 2022.
  • - During the peak of the pandemic in 2020, there was a significant drop in surgical volume by -36.8%, with specific procedures like rhinoplasty and rhytidectomy showing notable declines (up to -54.9%).
  • - In 2021, there was a strong recovery with a +75.0% increase in surgeries and +61.3% in procedures, and by 2022, FPRS rates surpassed pre-pandemic volumes, indicating a rebound

Background: Just-in-time training (JITT) occurs in the clinical context when learners need immediate guidance for procedures due to a lack of proficiency or the need for knowledge refreshment. The master adaptive learner (MAL) framework presents a comprehensive model of transforming learners into adaptive experts, proficient not only in their current tasks but also in the ongoing development of lifelong skills. With the evolving landscape of procedural competence in emergency medicine (EM), trainees must develop the capacity to acquire and master new techniques consistently.

Sex-determining region Y-box 2 (SOX2) is a transcription factor with a central role in embryologic development. SOX2 is also an oncogene in several cancer types. Prior work by our group has shown SOX2 activity associates with cell cycle dysregulation in early-stage bladder cancer.

Objective: Albumin is considered to be a surrogate marker for inflammation and nutritional status. Levels usually decrease after surgery but little is known about the predictive value of preoperative albumin levels in patients undergoing thyroidectomy. This study aimed to investigate the 30-day incidence of postoperative outcomes in thyroidectomy patients with and without preoperative hypoalbuminemia.

This Policy Resource and Education Paper (PREP) from the American College of Emergency Physicians (ACEP) discusses the use of high-sensitivity cardiac troponin (hs-cTn) in the emergency department setting. This brief review discusses types of hs-cTn assays as well as the interpretation of hs-cTn in the setting of various clinical factors such as renal dysfunction, sex, and the important distinction between myocardial injury versus myocardial infarction. In addition, the PREP provides one possible example of an algorithm for the use of a hs-cTn assay in patients in whom the treating clinician is concerned about potential acute coronary syndrome.

Leukocytoclastic vasculitis can be an uncommon and/or underreported adverse event of immune checkpoint inhibitor therapy, an established cancer treatment option. Differentiation among other cutaneous manifestations of adverse medication reactions-such as Stevens-Johnson syndrome, erythema multiforme, and drug reaction with eosinophilia and systemic symptoms (DRESS) syndrome-is crucial for guiding management.

Regional anesthesia between the pectoralis major and minor was first described in 2011 as an alternative method to paravertebral blocks or epidurals for post-operative mastectomies. Since then, the use of pectoral nerve (PECS) blocks for post-operative pain management following thoracotomy, sternotomy, and other procedures in the anterior thorax has increased. While experience with this block is growing, the current understanding of its use in pediatric patients is limited.

Pathologic evaluation of lumpectomy resection margins for invasive breast cancer: a single institution's experience.

Int J Clin Exp Pathol

February 2023

Department of Pathology and Laboratory Medicine, Penn State Health Milton S. Hershey Medical Center, Penn State College of Medicine Hershey, PA 17033, USA.

Objectives: Breast conservation therapy (BCT) or lumpectomy followed by radiation has been established as a preferred treatment for most patients with early-stage invasive breast cancer. About 20-40% of patients after initial lumpectomy will have to undergo re-excision due to a positive margin.

Methods: To determine the factors predicting higher risk of positive resection margin, we retrospectively analyzed 409 patients who underwent initial lumpectomy for invasive breast cancer from January 2019 through November 2022.

Objective: Lowering hospital readmission rates is a national goal, and presents an opportunity to lower health care costs, improve quality, and increase patient satisfaction. We aim to assess whether discharge disposition is associated with readmission.

Methods: A retrospective cohort study using logistic regression to quantify risk factors of hospital readmission in patients with confirmed head and neck cancer (HNC) who underwent surgery from 2010 to 2018 contained in the Pennsylvania Health Care Cost Containment Council database, which includes patients treated in Pennsylvania hospitals.
