Penile squamous cell carcinoma (pSCC) represents an uncommon malignancy characterized by stagnant mortality, psychosexual distress, and a highly variable prognosis. Currently, the WHO distinguishes between human papillomavirus (HPV) related and HPV independent pSCC. Recently, there has been an evolving line of research documenting the enrichment of HPV-independent pSCC with a high tumor mutational burden (TMB) and programmed death ligand-1 (PD-L1) expression, as well as clusters of genes associated with HPV status.

Background: Inguinal lymph node (LN) dissection (iLND) is mandatory in cN2 penile squamous cell carcinoma (PSCC). Open iLND (OIL) is often omitted due to the high rate of complications. A minimally invasive approach may reduce morbidity; however, evidence supporting its role to treat bulky nodes is limited.

Verrucous carcinoma, a well-differentiated variant of squamous cell carcinoma with low-grade malignancy, is often associated with localized human papillomavirus (HPV) infection and excessive penile foreskin. We report a 45-year-old male who presented with a verrucous plaque on the glans penis for over four months, along with itching. Examination revealed a verrucous, erosive lesion with a cauliflower-like surface, firm texture, and irregular shape, accompanied by pain, bleeding, and a foul odor.
