Substandard or disrespectful care during labour should be of serious concern for healthcare professionals, as it can affect one of the most important events in a woman's life. Substandard care refers to the use of interventions that are not considered best-practice, to the inadequate execution of interventions, to situations where best-practice interventions are withheld from patients, or there is lack of adequate informed consent. Disrespectful care refers to forms of verbal and non-verbal communication that affect patients' dignity, individuality, privacy, intimacy, or personal beliefs.

In high-resource countries, adverse perinatal outcomes are currently rare in term, non-malformed fetuses, undergoing labor, but they remain a leading cause of medico-legal dispute. Precise terminology is important to describe situations related to inadequate fetal oxygenation in labor, to ensure appropriate communication between healthcare professionals and adequate transmission of information to parents. This position statement provides consensus definitions from European perinatologists and midwives regarding the most appropriate terminology to describe situations related to inadequate fetal oxygenation in labor: suspected fetal hypoxia, severe newborn acidemia, newborn metabolic acidosis, and hypoxic-ischemic encephalopathy.

Article Synopsis
  • Urogenital cancer treatments can negatively affect fertility, leading to ongoing debates about how to preserve it, with little evidence and no international guidelines available.
  • An online survey conducted with urologists across five European countries revealed that while a majority recommend sperm cryopreservation before orchidectomy, practices for sperm extraction vary widely and proposals for cryopreservation in prostate and bladder cancer cases are inconsistent.
  • The study highlights significant differences in fertility preservation practices based on national guidelines, emphasizing the need for standardized international recommendations to better address this issue for urological cancer patients.

First-trimester screening for Down syndrome using quadruple maternal biochemical markers.

Clin Chem Lab Med

August 2023

Reproduction, Mother and Child Health Unit, CHU De Québec - Université Laval Research Center, Université Laval, Quebec, QC, Canada.

Article Synopsis
  • The study evaluates the effectiveness of combining placental growth factor (PlGF) with other markers (PAPP-A, free β-hCG, and AFP) for screening Down syndrome in the first trimester of pregnancy.
  • Out of 13,386 pregnancies, only 26 cases of Down syndrome were detected, with the biomarker combination identifying 88% of these cases at a 13% false-positive rate.
  • Incorporating nuchal translucency measurements would significantly improve detection rates, allowing for identification of over 95% of Down syndrome cases while lowering false positives to around 5%.

The mechanisms underlying the immune control of Zika virus infection at the maternal-fetal interface.

Front Immunol

December 2022

Toulouse Institute for Infectious and Inflammatory Diseases (Infinity), CNRS UMR5051, INSERM UMR1291, University of Toulouse III, Toulouse, France.

Unlike other Flaviviruses, Zika virus (ZIKV) infection during the first trimester of pregnancy causes severe pregnancy outcomes including the devastating microcephaly and diseases associated with placental dysfunctions. We have previously reported that the maternal decidua basalis, the major maternal-fetal interface, serves as a replication platform enabling virus amplification before dissemination to the fetal compartment. However, the rate of congenital infection is quite low, suggesting the presence of a natural barrier against viral infection.

Short communication: Is there any benefit of initiating aspirin before the 11th week of gestation?

Pregnancy Hypertens

December 2022

Department of Obstetrics, Gynecology and Reproduction, CHU de Québec-Université Laval, Québec City, QC, Canada; Reproduction, Mother and Child Health Unit, Research Center of the CHU de Québec - Université Laval, Québec City, QC, Canada. Electronic address:

Aspirin initiated between 11 and 14 weeks of gestation reduces the risk of preterm preeclampsia and other placenta-mediated complications in screen-positive women. Most of these adverse outcomes are associated with maternal vascular malperfusion of the placenta, a disease that begins during the early first trimester. Assuming that aspirin has direct beneficial actions on the developing placenta, tempts clinicians to believe in the maxim that "the earlier the better", however neither the safety nor the effectiveness of aspirin started before 11th week of gestation has been demonstrated.

Article Synopsis
  • This clinical trial aimed to determine if daily low-dose aspirin could lower the risks of preeclampsia and fetal growth restriction in first-time pregnant women identified as high risk through early ultrasound screenings.
  • Conducted in France, the study involved over 1,100 participants who were randomly assigned to either take low-dose aspirin or a placebo from early pregnancy until the 34th week.
  • Results showed no significant difference in the rates of preeclampsia or low birth weight between the aspirin and placebo groups, leading to the conclusion that low-dose aspirin does not provide benefits for these outcomes in the targeted population.

Oxidative stress, lipid peroxidation and premature placental senescence in preeclampsia.

Arch Biochem Biophys

November 2022

lnfinity, CNRS, Inserm UMR 1291, University of Toulouse and Gynecology/Obstetrics Department, Paule-de-Viguier Hospital, Toulouse, France.

Accelerated placental senescence is associated with preeclampsia (PE) and other pregnancy complications. It is characterized by an accelerated decline in placental function due to the accumulation of senescence patterns such as telomere shortening, mitochondrial dysfunction, oxidative damages, increased expression of phosphorylated (serine-139) histone γ-H2AX, a sensitive marker of double-stranded DNA breaks, accumulation of cross-linked ubiquitinated proteins and sirtuin inhibition. Among the lipid oxidation products generated by the peroxidation of polyunsaturated fatty acids, aldehydes such as acrolein, 4-hydroxy-2-nonenal, 4-oxo-2-nonenal, are present in the blood and placenta from PE-affected women and could contribute to PE pathogenesis and accelerated placental aging.
