Introduction: Thyroid and salivary gland cytopathology frequently present diagnostic challenges due to complex presentations, overlapping features between benign and malignant conditions, particularly with grey-zone entities and rare pathologies. To address these issues, the 45th European Congress of Cytology (ECC) held a slide seminar focused on challenging cases. This article reviews key findings from the six cases discussed, emphasizing the importance of a comprehensive diagnostic approach.

The Impact of Inflammation on the Etiopathogenesis of Benign Salivary Gland Tumors: A Scoping Review.

Int J Mol Sci

November 2024

Department of Otolaryngology, Academy of Applied Medical and Social Sciences, 82-300 Elblag, Poland.

Salivary gland tumors are rare head and neck tumors constituting up to 6% of all head and neck neoplasms; despite being mostly benign, these tumors present in diverse histological subtypes, making them challenging to diagnose and treat. Our research aims to investigate the link between inflammation and tumorigenesis within the salivary glands based on the literature regarding the etiopathogenesis of benign salivary gland tumors. This scoping review was conducted following the PRISMA extension for scoping reviews and reporting guidelines.

Background: Modern imaging techniques with magnetic resonance imaging (MRI) or positron emission tomography/computed tomography (PET/CT) have recently been developed to assess radiation-induced damage to salivary structures. The primary aim of this review was to summarize evidence on the imaging modalities used for the assessment and prediction of xerostomia after head and neck radiotherapy (RT).

Methods: A systematic review of the literature was performed using successively the MeSH terms "PET," "MRI," "scintigraphy," "xerostomia," and "radiotherapy.

Pleomorphic adenoma (PA) is a benign salivary gland tumour that may recur or undergo malignant transformation (CXPA). Toll-like receptors (TLR) mediate immune responses triggered by various agents such as viruses and are related to tumour formation either by stimulating or suppressing their growth, with variation across different tumour entities. We compared TLR immunohistochemical expression in PA, its recurrent counterparts and CXPA and evaluated the effect of virus presence in these tumours.

National Trends and Benchmarks for Operative Time and Hospital Length of Stay in Parotidectomies.

Otolaryngol Head Neck Surg

December 2024

Department of Otolaryngology-Head and Neck Surgery, Mayo Clinic, Rochester, Minnesota, USA.

Objective: The extent of parotidectomy for benign tumors has de-escalated in the United States. We aim to define modern benchmarks for operative time and hospital length of stay (LOS) in parotidectomy and identify risk factors that may prolong these benchmarks.

Study Design: This is a retrospective cross-sectional study of all adults who underwent parotidectomy for a primary parotid neoplasm between January 2011 and December 2021 using the American College of Surgeons National Surgical Quality Improvement Program database.

Purpose: Palliative whole-brain radiotherapy (WBRT) is a useful treatment modality for patients diagnosed with brain metastasis. Our study aimed to document parotid doses using a short course of 20 Gy in five fractions (SC-WBRT) and compare them with the known xerostomia parameters.

Methods And Materials: We retrieved dosimetric parameters for 59 patients who received SC-WBRT.

Primary head and neck lymphoid neoplasms(PHNLN) are described as a series of lymphoid system-derived neoplasms which originally arising from head and neck region. Our study is aimed to present a panoramic view of PHNLN among adolescent and young adult(AYA) patients aged from 15 to 39 years-old. The individual patient information was obtained from Surveillance, Epidemiology and End Results(SEER) database.

Colorectal cancer ideally spreads via the haematogenous or lymphatic route with the most common metastasis to the liver or lungs. Metastasis of this cancer to the parotid gland has not been previously reported. This paper presents a case of rectal adenocarcinoma in a 34-year-old Pakistani male, who presented with histopathological evidence of metastasis in the parotid gland within one year of treatment.

Article Synopsis
  • The study explored the connection between Human Papillomavirus (HPV) and salivary gland tumors, which remains unclear in existing literature.
  • The research involved 66 salivary gland lesions, using fine needle aspiration cytology to detect HPV-DNA, resulting in HPV found in two cysts and three metastatic carcinomas, but none in primary tumors.
  • Ultimately, the study concluded that HPV did not play a role in primary salivary gland tumors, but its presence in intraglandular cysts suggests a new area for further investigation.*

Feasibility of Periauricular Incision for Parotidectomy: A Retrospective Study.

J Craniofac Surg

December 2024

Medical Doctor, Department of Otorhinolaryngology, Kocaeli City Hospital, Kocaeli, Turkey.

Backgrounds: Various incisions are used in parotid surgery, yet they often result in visible scarring on the neck and potential deformities. This study aimed to assess the efficacy and feasibility of utilizing a periauricular approach for excising parotid tumors irrespective of their location and size.

Methods: This retrospective study involved 46 consecutive patients with parotid lesions, who underwent minimally invasive excision through a periauricular incision performed by a single surgeon between January 2022 and January 2024.

Article Synopsis
  • - Phenotypic plasticity in tumor cells allows them to adapt and change due to stressors, playing a key role in tumor progression, as illustrated by a case of a 70-year-old man with pleomorphic adenoma who developed pleomorphic liposarcoma.
  • - Sarcomas, like liposarcoma, can develop from existing pleomorphic adenomas in the parotid gland, with only a few documented cases of primary parotid liposarcoma.
  • - This case report examines the uncharted adenoma-carcinoma-sarcoma progression in pleomorphic adenomas, aiming to provide insights for treatment planning and management from a surgical oncology standpoint.

Background: Deep-lobe tumors have been shown to possess a significantly thicker capsule with less tumor penetration compared to superficial tumors. Thus, more conservative surgical approaches, rather than aggressive methods, have been proposed for treating benign deep-lobe tumors of the parotid gland.

Aim: To evaluate the surgical outcomes and oncological safety of selective deep-lobe parotidectomy (SDLP) in patients with benign lesions located in the deep lobe of the parotid gland.

Clear cell tumors of parotid gland encompass a wide spectrum of neoplasms, including benign and malignant epithelial neoplasms. Additionally, tumors from adjacent structures such as paraganglioma, and metastatic neoplasms may also show clear cells. Overlapping cytological features may cause difficulty in diagnosis.

The Importance of Elastography in the Early Diagnosis of Highly Differentiated Parotid Tumors: a Case-Report.

Maedica (Bucur)

September 2024

Department of Otorhinolaryngology, Head and Neck Surgery, Faculty of Medicine, School of Health Sciences, University of Ioannina, Ioannina, Greece.

It is indisputable that high-resolution ultrasound (US) is the diagnostic gold standard for the evaluation of superficial parotid gland diseases. It is a dynamic, quick, simple, easily available, cost-effective, noninvasive procedure, with absence of ionizing radiation examination that can be performed safely and in special categories of patients such as pregnant women and children. It is widely accepted that on US, benign tumors have clear, smooth and well-defined borders, homogeneous hypoechoic parenchyma and a defined distribution of vessels.
