Article Synopsis
  • Chronic rhinosinusitis with nasal polyps (CRSwNP) is a common condition affecting 2% of people, typically treated with corticosteroids and saline rinses, with surgery as an option for those who don't respond to medication.
  • In France, two biologic treatments are available for severe cases that don’t improve after existing therapies or surgery, and a new study will evaluate the effectiveness and safety of these biologics in real-life settings.
  • The BIOlogics in severe nasal POlyposis SurvEy (BIOPES) is a national, multicenter study aimed at understanding the impact of biologics on CRSwNP patients and gathering various clinical data to inform future treatments.

Article Synopsis
  • Fatigue is a common issue in patients with inflammatory bowel disease (IBD), with 54.1% experiencing fatigue and 37.1% reporting severe fatigue.
  • Patients with active IBD experience significantly higher levels of fatigue compared to those with inactive disease.
  • Key factors affecting fatigue include age, gender, sick leave, physical pain, emotional well-being, and sleep quality, suggesting that a holistic treatment approach is essential for managing IBD-related fatigue.

Article Synopsis
  • Researchers aimed to validate and update a prediction model for relapse risk in Crohn's disease patients after stopping anti-TNF therapy, stemming from a previous trial (STORI).
  • The study analyzed data from 1317 patients across 14 studies, finding a 38% relapse rate within a year after treatment cessation, but the original prediction model performed poorly (C-statistic of 0.51).
  • The updated model slightly improved the ability to predict relapses (C-statistic of 0.59) and identified key factors such as clinical symptoms, age at diagnosis, smoking, and C-reactive protein levels that influenced relapse risk.

A Scoring System to Determine Patients' Risk of Colectomy Within 1 Year After Hospital Admission for Acute Severe Ulcerative Colitis.

Clin Gastroenterol Hepatol

August 2021

Sorbonne Université, Department of Gastroenterology, AP-HP, Hôpital Saint Antoine, Paris; Sorbonne Université, INSERM, Institut Pierre Louis d'Épidémiologie et de Santé Publique, Paris.

Background & Aims: There is consensus on the criteria used to define acute severe ulcerative colitis (ASUC) and on patient management, but it has been a challenge to identify patients at risk for colectomy based on data collected at hospital admission. We aimed to develop a system to determine patients' risk of colectomy within 1 y of hospital admission for ASUC based on clinical, biomarker, and endoscopy data.

Methods: We performed a retrospective analysis of consecutive patients with ASUC treated with corticosteroids, ciclosporin, or tumor necrosis factor (TNF) antagonists and admitted to 2 hospitals in France from 2002 through 2017.

Background: Whether vedolizumab may be effective as a treatment for primary sclerosing cholangitis [PSC] in patients with inflammatory bowel disease [IBD] remains controversial.

Methods: We performed a retrospective observational study of consecutive patients with IBD and PSC, treated with vedolizumab for at least 30 weeks in 22 centres of GETAID from January 2015 to June 2016. The outcomes included a decrease in the serum alkaline phosphatase [ALP] concentration of at least 50% from baseline to Week 30 or 54, a change in any serum liver enzymes concentrations, and an assessment of the efficacy and safety of vedolizumab in IBD.

Curative therapy for individuals with severe sickle cell disease (SCD) who lack an HLA-identical sibling donor has been frustratingly elusive. In with the goal of improving engraftment while minimizing transplantation-related morbidity, a multi-institutional learning collaborative was developed in the context of a Phase II clinical trial of nonmyeloablative, related HLA-haploidentical (haplo) bone marrow transplantation (BMT) with post-transplantation cyclophosphamide. All eligible participants had hemoglobin SS, and 89% (16 of 18) had an identifiable donor.

Background: Fungi of the genus Scedosporium are emerging pathogens responsible for severe infections in lung transplant recipients. These infections are associated with poor prognosis and some centers consider now Scedosporium species colonization as a contraindication to lung transplantation (LT) even though no published evidence demonstrates that Scedosporium species colonization is associated with higher morbidity or mortality after LT.

Methods: Here, we aim to describe characteristics and outcome of cystic fibrosis (CF) lung transplant recipients colonized with Scedosporium species in a single center over a 15-year period.
