Infection and mortality of healthcare workers worldwide from COVID-19: a systematic review.

BMJ Glob Health

December 2020

Oxford University Global Surgery Group, Nuffield Department of Surgical Sciences, University of Oxford, Oxford, Oxfordshire, UK

Article Synopsis
  • The study aimed to estimate global COVID-19 infections and deaths among healthcare workers during the early pandemic phase, utilizing systematic reviews and comprehensive searches of literature until May 2020.
  • A total of 152,888 infections and 1,413 deaths among healthcare workers were reported, with notable differences in gender and job roles: most infections were in women and nurses, while most deaths were among men and doctors.
  • The findings highlighted that while Europe had the highest absolute numbers, the Eastern Mediterranean region had the highest death rate per infection, indicating a need for further investigation into the reasons for these variations, especially in high-risk specialties and lower reported rates in regions like Africa and India.

Background: Travel time and healthcare financing are critical determinants of the provision of quality maternal health care in low resource settings. Despite the availability of pregnancy-related mHealth and smart travel applications, there is a lack of evidence on their usage to travel to health facilities for routine antenatal care and emergencies. There is a shortage of information about the feasibility of using a custom-made mobile technology that integrates smart travel and mHealth.

Acquisition costs and prohibitively expensive technical support for endoscope maintenance, repair, and reprocessing have deterred adoption of flexible cystoscopes by many urologists internationally. This study evaluated the performance of a novel single-use digital cystoscope that can directly connect to a laptop or computer monitor for visualization, obviating the need for a tower. The performance characteristics of flexible single-use cystoscopes (NeoScope) were prospectively evaluated a reusable flexible cystoscope (Olympus) in three clinical cases and two independent benchtop testing episodes in Canada.

Article Synopsis
  • Crocodile attack injuries, although rare, pose significant trauma risks in communities near water bodies and present unique challenges in medical treatment.
  • A case study of an adult male with a severe crocodile bite illustrates the effective use of regional anesthesia for wound management, including initial debridement and subsequent care, all conducted at the bedside without the need for operating room resources.
  • The findings suggest that regional anesthesia can be a beneficial approach for managing extremity traumas, minimizing complications and resource use, while also reducing the need for opioids.

Background: Cryptococcal meningitis (CM) is a major cause of mortality in HIV programmes in Africa despite increasing access to antiretroviral therapy (ART). Mortality is driven in part by limited availability of amphotericin-based treatment, drug-induced toxicities of amphotericin B deoxycholate and prolonged hospital admissions. A single, high-dose of liposomal amphotericin (L-AmB, Ambisome) on a fluconazole backbone has been reported as non-inferior to 14 days of standard dose L-AmB in reducing fungal burden.

Esophageal squamous cell carcinoma (ESCC) remains the predominant histological subtype of esophageal cancer (EC) in many transitioning countries, with an enigmatic and geographically distinct etiology, and consistently elevated incidence rates in many Eastern and Southern African countries. To gain epidemiological insights into ESCC patterns across the continent, we conducted a systematic review and meta-analysis of male-to-female (M:F) sex ratios of EC age-standardised (world) incidence rates in Africa according to geography, time and age at diagnosis. Data from 197 populations in 36 countries were included in the analysis, based on data from cancer registries included in IARC's Cancer Incidence in Five Continents, Cancer in Africa and Cancer in Sub-Saharan Africa reports, alongside a systematic search of peer-reviewed literature.
