Background: The currently approved frontline treatments for diffuse pleural mesothelioma (DPM) are ipilimumab-nivolumab or platinum-pemetrexed. Objective: Patients' psychological reactions to multigene cancer panel testing might differ compared with the single-gene testing reactions because of the complexity and uncertainty associated with the different possible results. Understanding patients' preferences and psychological impact of multigene panel testing is important to adapt the genetic counselling model.

Methods: One hundred eighty-seven unrelated patients with clinical suspicion of hereditary cancer undergoing a 25-gene panel test completed questionnaires after pretest genetic counselling and at 1 week, 3 months, and 12 months after results to elicit their preferences regarding results disclosure and to measure their cancer worry and testing-specific distress and uncertainty.

Objective: To describe that topiramate may cause a false positive in an overnight 1-mg dexamethasone suppression test (DST) for hypercortisolism screening.

Methods: We present a case in which topiramate induced dexamethasone metabolism, leading to a false positive on the DST.

Results: A 44-year-old female with an incidentally found adenoma in the right adrenal gland underwent a DST for hypercortisolism screening.

Background: Comatose patients present a high risk of early-onset ventilator-associated pneumonia (EO-VAP) for which antibiotic prophylaxis has been proposed. Comatose patients were studied to evaluate the efficacy of a single-dose of antibiotic prophylaxis at intubation against EO-VAP.

Methods: A prospective cohort of comatose patients (Glasgow Coma Score ≤ 8) who were admitted in 2009-2010 and administered a single-dose of antibiotic within 4 h of intubation was compared with comatose patients (admitted ≥ 4 h after intubation in 2009-2010 or admitted in 2007-2008) who did not receive antibiotic prophylaxis.

Hospital-acquired infections (HAI) occur in 5%-10% of patients admitted to hospitals in the United States, and HAIs remain a leading cause of morbidity and mortality. Patients admitted to ICUs account for 45% of all hospital-acquired pneumonias and bloodstream infections (BSIs), although critical care units comprise only 5% to 10% of all hospital beds. The severity of underlying disease, invasive diagnostic and therapeutic procedures that breach normal host defenses, contaminated life-support equipment, and the prevalence of resistant microorganisms are critical factors in the high rate of infection in the ICUs.

This article reviews the most significant developments reported between July 2006 and July 2007 in the fields of pediatric cardiology and congenital heart disease, in diagnosis and treatment in particular. In the area of imaging techniques, the increasing clinical roles of magnetic resonance imaging, tissue Doppler imaging and three-dimensional echocardiography are highlighted, as is the application of these techniques to fetal cardiology and its repercussions. The incidence and treatment of arrhythmias in children are also discussed and, with regard to heart failure, there is an assessment of recent findings in epidemiology and diagnosis, new drugs, ventricular assist systems, and the current status of heart transplantation.

Objective: To examine the characteristics, prognostic factors, and outcome of patients with severe hospital-acquired pneumonia admitted to the ICU.

Design And Setting: Prospective observational clinical study in two medical-surgical ICUs with 16 and 20 beds

Patients And Participants: During a 7-year period all hospitalized patients requiring admission to either ICU for hospital-acquired pneumonia were followed up.

Measurements And Results: We diagnosed 96 episodes of severe hospital-acquired pneumonia, and in 67 cases a causal diagnosis was made.
