Chronic cough management: Practical guidelines and PICO-based evidence for treatment.

Asian Pac J Allergy Immunol

December 2024

Department of Clinical Epidemiology and Biostatistics, Faculty of Medicine Ramathibodi Hospital, Mahidol University, T hailand.

This part reviews the management of chronic cough and proposes a management algorithm. Despite proven improvements in quality of life following chronic cough treatment, a clear understanding of the disease and the evidence for the efficacy of some treatments remain vague. Eight key questions regarding the treatment in the uncertain areas were systematically addressed based on the PICO framework and applying the GRADE system for evidence synthesis to provide the strength of recommendation and quality of evidence for key questions, with narrative components for the description of other chronic cough treatment including non-pharmacological therapy.

Comparative outcomes of microsurgical dorsal root entry zone lesioning (DREZotomy) for intractable neuropathic pain in spinal cord and cauda equina injuries.

Neurosurg Rev

January 2025

Division of Neurosurgery, Department of Surgery, Faculty of Medicine Siriraj Hospital, Mahidol University, 2 Wang Lang Road, Bangkok Noi, 10700, Bangkok, Thailand.

Treatment of neuropathic pain in patients with spinal cord injury (SCI) and cauda equina injury (CEI) remains challenging. Dorsal root entry zone lesioning (DREZL) or DREZotomy is a viable surgical option for refractory cases. This study aimed to compare DREZL surgical outcomes between patients with SCI and those with CEI and to identify predictors of postoperative pain relief.

Anatomic characteristics of the right aortic arch with aberrant left subclavian artery in patients who do and do not undergo vascular ring repair.

J Cardiovasc Comput Tomogr

December 2024

Department of Cardiology, Boston Children's Hospital, Boston, MA, USA; Department of Pediatrics, Harvard Medical School, Boston, MA, USA. Electronic address:

Introduction: It is unclear if certain anatomic characteristics in patients with a right aortic arch with aberrant left subclavian artery (RAA ALSCA) are associated with undergoing surgical repair.

Methods: This was a single-center retrospective study of patients with RAA ALSCA and computed tomography or cardiovascular magnetic resonance from July 2013-September 2023. The size of the proximal ALSCA or diverticulum of Kommerell (DoK), thoracic inlet index, angle of the proximal ALSCA/DoK from the aortic arch, the position of descending aorta, location of the DoK, and tracheal size were compared between patients who did or did not undergo surgery.

  • - This study evaluated the aMAP score's effectiveness in predicting hepatocellular carcinoma (HCC) in Thai patients with chronic hepatitis B (CHB) by analyzing data from 2008 to 2019, focusing on factors like age, sex, and blood levels.
  • - Out of 1,060 patients, 789 were eligible for analysis, and 51 developed HCC, with significant differences in HCC incidence based on risk group classifications over 3, 5, and 10 years.
  • - The aMAP score showed promising predictive ability with AUROCs of 0.748, 0.777, and 0.784 over time, but the CU-HCC score outperformed it with

  • - Metabolic dysfunction-associated steatotic liver disease (MASLD) is becoming a global health issue, and the primary treatment involves lifestyle interventions (LSI) to manage the disease.
  • - A study conducted in Thailand involved 122 noncirrhotic MASLD patients, who were divided into a control group receiving standard care and an intervention group receiving regular updates and info via the LINE app about LSI and MASLD.
  • - After 24 weeks, while both groups improved in various health metrics, the intervention group showed a significant enhancement in liver stiffness compared to the control group, indicating that social media can be effective in managing MASLD.

Background: Hepatectomy stands as a curative management for liver cancer. The critical factor for minimizing recurrence rate and enhancing overall survival of liver malignancy is to attain a negative margin hepatic resection. Recently, Indocyanine green (ICG) fluorescence imaging has been proven implemental in aiding laparoscopic liver resection, enabling real-time tumor identification and precise liver segmentation.

The Ki-67 proliferative index plays a pivotal role in the subclassification of neuroendocrine neoplasm (NEN) according to the WHO Classification of Digestive System Tumors (5th edition), which designates neuroendocrine tumor (NET) grades 1, 2, and 3 for Ki-67 proliferative index of <3 %, 3-20 %, and >20 %, respectively. Proliferative index calculation must be performed in the hotspot, traditionally selected by visual scanning at low-power magnification. Recently, gradient map visualization has emerged as a tool for various purposes, including hotspot selection.

Modified Extensor Indicis Proprius Opponensplasty.

Tech Hand Up Extrem Surg

September 2024

Upper Extremity and Reconstructive Microsurgery Unit, Institute of Orthopaedics, Lerdsin Hospital, Department of Orthopaedic Surgery, College of Medicine, Rangsit University, Pathum Thani.

Extensor indicis proprius (EIP) opponensplasty is one of the commonly used techniques to restore thumb abduction and opposition in patients with thenar muscle dysfunction from various causes of median nerve palsy. However, its subcutaneous route around the distal ulna may not represent a straight line of pull, and part of the extensor hood sometimes has to be harvested along with the EIP tendon to gain adequate length to reach the insertion. The purpose of the study is to present the alternative method of EIP opponensplasty and report the clinical outcomes.

  • A needs assessment was conducted to develop a continuing professional development (CPD) program for pythiosis, a rare and deadly disease prevalent mainly in Thailand and India.
  • A survey of 267 residents at King Chulalongkorn Memorial Hospital revealed that only 41.67% of respondents scored adequately on knowledge about pythiosis, indicating significant educational gaps, particularly in laboratory investigations and epidemiology.
  • The findings suggest that the CPD program should focus on improving education in critical areas for early detection, and most participants preferred an online format with a blend of synchronous and asynchronous sessions lasting 60-90 minutes.
