Article Synopsis
  • Urban rodents at Kolkata's Alipore Railway Station were studied for their role in spreading zoonotic pathogens, particularly Bartonella spp., through fleas, highlighting the link between high rodent populations and increased disease risk due to nearby human activity.
  • The study involved collecting and identifying rodents and fleas, as well as detecting Bartonella DNA to understand its prevalence and phylogenetic relationships among different species.
  • Results showed a high prevalence of Bartonella infections in rodents (53.33%) and fleas (79.1%), with some haplotypes indicating cross-species transmission between rodents, fleas, and humans, underscoring the public health implications of these findings.

Article Synopsis
  • The study compares various screening methods for allergic bronchopulmonary aspergillosis (ABPA) in asthma patients to find the most effective approach.
  • Serum IgE testing emerged as the best option, being the most cost-effective while providing high diagnostic accuracy.
  • Other methods, like total IgE and imaging, were more expensive and either missed cases or required many additional tests.

Article Synopsis
  • - The study compares the effectiveness of two methods of postoperative oral feeding—on-demand versus predetermined timing—in patients who had elective cholecystectomy surgeries under general anesthesia.
  • - A total of 120 patients were divided into two groups; one received coconut water upon request (On-Demand group), while the other received it at a set time (Predetermined-Time group).
  • - Results showed similar rates of postoperative nausea and vomiting in both groups, but patient satisfaction was significantly higher in the On-Demand group, suggesting that allowing patients to choose when they eat enhances their recovery experience.

Castleman disease is a benign lymph node hyperplasia with two histological subtypes. In the case reported here, a young adult male presented with mucocutaneous lesions mimicking common dermatological conditions. Suspicion of an underlying systemic disease arose when the routine clinical care of the skin lesions failed to cure them.

Malignant thyroid neoplasm with ectopic Cushing's syndrome.

BMJ Case Rep

December 2024

Endocrinology, PGIMER, Chandigarh, India.

An adult man in his 30s with newly diagnosed diabetes mellitus and hypertension presented with a 33 lb weight gain, generalised swelling and classic Cushingoid features, including proximal muscle weakness, easy bruisability, wide violaceous striae and a painless, progressively enlarging right-sided neck swelling over the past month. Physical examination revealed a 4×4 cm thyroid mass, facial plethora and dorsocervical fat pads. Laboratory investigations confirmed hypercortisolism with elevated cortisol and Adrenocorticotropic Hormone(ACTH), with non-suppression on dexamethasone suppression tests.

Peripheral nerve imaging is a powerful tool in evaluating peripheral neuropathy. This review aims to provide an overview of the anatomy, clinical significance and pathologies of small peripheral nerves, typically less than 2 mm in diameter. High frequency ultrasound probes offer the best assessment of these nerves whilst permitting real-time clinical correlation, while the role of MRI is inherently limited by spatial resolution constraints and the lack of patient contact.

Deep caries detection using deep learning: from dataset acquisition to detection.

Clin Oral Investig

December 2024

Department of Computer Science, SLIET, Longowal, Sangrur, 148106, Punjab, India.

Article Synopsis
  • The study aims to improve the early detection of dental caries using advanced Artificial Intelligence (AI) models, addressing a major health issue particularly in low- and middle-income countries.
  • Traditional diagnostic methods have limitations, so the research utilizes deep learning models (YOLOv7, YOLOv8, and YOLOv9) trained on over 3,200 images for enhanced accuracy.
  • Results show that YOLOv8 outperforms other models with a mean Average Precision of 0.982, indicating the potential for AI to improve dental diagnostics and patient outcomes significantly.

Myoepitheliomal tumours, associated with the salivary glands and certain soft tissue sites, are unusually encountered in the central nervous system (CNS). In the brain, although the most common location is the Sella, other areas can infrequently be involved. Such intracranial lesions are exceptionally rare in children.

Purpose: Epstein-Barr virus (EBV) reactivation in allogeneic hematopoietic stem cell transplantation (allo-HCT) recipients can lead to significant complications including post-transplant lymphoproliferative disease. Despite progress in managing EBV reactivation in allo-HCT recipients, data on clinical characteristics and prognostic implications of EBV viral load remain limited. Here, we aim to evaluate the prevalence, identify risk factors, and assess the clinical implications of EBV-DNA positivity in allo-HCT recipients.

Background: Telomerase has been linked to aging and cancer. The MNS16A polymorphism in the hTERT gene plays a significant role in modulating telomerase activity and highlights the complexity of telomere-related genetics in cancer research.

Experimental Design: We genotyped 401 lung-cancer samples treated with platinum-based chemotherapy to identify the MNS16A polymorphism.

Article Synopsis
  • Therapeutic plasma exchange (TPE) can lead to low calcium levels due to the use of Anticoagulant Citrate Dextrose Solution A, and adding different concentrations of calcium gluconate to human serum albumin may help prevent this issue.* -
  • A study comparing two groups of TPE procedures found that while intraprocedural calcium levels were similar, post-procedural ionized calcium was significantly lower in the group with less calcium, leading to more cases of symptomatic hypocalcemia and vasovagal reactions.* -
  • The results suggest that using a higher concentration of calcium gluconate (18.6 mg of elemental calcium/100 ml) is safer and more effective in maintaining calcium levels during

Microbial biofilms are intricate communities that pose significant challenges in clinical and microbiological settings due to their resistance to antibiotics and immune responses. Advanced microscopy techniques, such as scanning electron microscopy (SEM), confocal laser scanning microscopy (CLSM), and fluorescence microscopy, are often employed to visualize and differentiate between these biofilms. However, these methods are not feasible in all laboratories because of their high cost and complexity.
