The Effect of a Multidisciplinary Lifestyle Intervention Program on Apelin-12, Vaspin and Resistin Concentrations in Children and Adolescents with Overweight and Obesity.

Nutrients

October 2024

Division of Endocrinology, Metabolism and Diabetes, First Department of Pediatrics, National and Kapodistrian University of Athens Medical School, 'Aghia Sophia' Children's Hospital, 11527 Athens, Greece.

Obesity in childhood and adolescence has reached epidemic proportions in recent decades. In the present study, we determined the concentrations of apelin-12, vaspin and resistin in 106 children and adolescents with overweight or obesity before and after the implementation of a multidisciplinary, personalized lifestyle intervention program of diet, sleep and exercise for 1 year. All subjects attended our Center for the Prevention and Management of Overweight and Obesity in Childhood and Adolescence.

Article Synopsis
  • Diagnosing pediatric tuberculosis (TB) is complicated, but changes in host gene expression in blood can help identify biomarkers and improve understanding of the immune response in children with TB.* -
  • The study involved stimulating blood samples from 102 children with confirmed TB, TB infection, pneumonia, and healthy controls, and analyzing the gene expression differences using microarrays.* -
  • Researchers identified specific gene signatures that effectively distinguish TB from pneumonia, with high accuracy (AUC of 100% for one gene and 91.7% for a combination of two genes), paving the way for new diagnostic tests aligned with WHO standards.*

Article Synopsis
  • The study assesses the effectiveness of white blood cell count (WBC) in predicting serious bacterial infections (SBI) in children compared to C reactive protein (CRP) and absolute neutrophil count (ANC).
  • After analyzing data from over 17,000 febrile children across multiple European emergency departments, WBC showed poor predictive value with a sensitivity of 56% and specificity of 74% for WBC >15.
  • The findings suggest that CRP is a better marker for identifying SBI in children, and WBC should only be used for specific cases rather than as a routine diagnostic tool.

Article Synopsis
  • Maternal primary adrenal insufficiency (PAI) during pregnancy, caused by Addison disease or congenital adrenal hyperplasia, is rare, and there's limited research on its effects on fetal and neonatal outcomes.
  • A systematic review and meta-analysis examined 31 studies, 11 of which were quantitatively analyzed, focusing on outcomes like spontaneous abortion, preterm birth, and small for gestational age (SGA) neonates.
  • Results showed higher rates of spontaneous abortions (18%), preterm births (11% overall), and SGA neonates (8% overall) in women with PAI compared to controls, although fetal birth weights remained normal across all groups studied.

Anamnestic 13-valent pneumococcal conjugate vaccine immunization did not affect the relapse risk in pediatric idiopathic nephrotic syndrome. Pneumococcal serotype (PS)-specific antibody titers increased significantly in all groups. Children receiving immunomodulatory treatments (IMTs) displayed significantly lower levels of PS-specific antibodies for 3/8 serotypes tested.

Background: Multisystem inflammatory syndrome in children (MIS-C) is a rare but serious hyperinflammatory complication following infection with severe acute respiratory syndrome coronavirus 2. The mechanisms underpinning the pathophysiology of MIS-C are poorly understood. Moreover, clinically distinguishing MIS-C from other childhood infectious and inflammatory conditions, such as Kawasaki disease or severe bacterial and viral infections, is challenging due to overlapping clinical and laboratory features.

Antimicrob Resist Infect Control

January 2024

Directorate of Epidemiological Surveillance and Intervention for Infectious Diseases, National Public Health Organization (EODY), Athens, Greece.

Background: The burden of healthcare-associated infections (HAIs) and the extent of antimicrobial use (AU) are periodically recorded through Point Prevalence Surveys (PPS) in acute care hospitals coordinated by the European Centre for Disease Prevention and Control (ECDC). In previous PPSs, Greece demonstrated increased HAI and AU prevalence: 9% and 54.7% in 2011-2012, and 10% and 55.

Article Synopsis
  • * Using data from 12 European EDs, researchers analyzed presenting symptoms and management of febrile children under 18, identifying those suitable for a fast track system based on simplicity and minimal resource use.
  • * Findings revealed that respiratory symptoms were the most common, while neurological patients utilized more imaging and had higher admission rates, and gastrointestinal patients underwent more lab tests, indicating varying needs among the different symptom groups.

Effect of Leflunomide on Treatment of Pediatric Renal Transplant Recipients With BK Virus Infection.

Exp Clin Transplant

October 2023

From the Pediatric Nephrology Department, P. and A. Kyriakou Children's Hospital National and Kapodistrian University of Athens, Laiko General Hospital, Athens, Greece.

Objectives: Infection with the BK virus is a significant complication after renal transplant and can progress to BK virus nephropathy and graft dysfunction. There is no consensus on the management of BK virus infection in pediatric renal transplant recipients. The most common therapeutic option is immunosuppression reduction, which can increase rejection risk.

Background: Using a retrospective cohort study design, we aimed to evaluate the effectiveness of molnupiravir and nirmatrelvir/ritonavir in patients with SARS-CoV-2 who were highly vulnerable.

Methods: The impact of each drug was determined via comparisons with age-matched control groups of patients positive for SARS-CoV-2 who did not receive oral antiviral therapy.

Results: Administration of molnupiravir significantly reduced the risk of hospitalization (odds ratio [OR], 0.

Children constitute 6-10% of all patients attending the emergency department (ED) by emergency medical services (EMS). However, discordant EMS use in children occurs in 37-61% with fever as an important risk factor. We aimed to describe EMS utilisation among febrile children attending European EDs.

Overview: Pandemics are characterized by an abrupt and sudden outburst and absence of preparation for its management. The focus during pandemics is on the medical aspect of the disease and not on its impact on the citizens' or vulnerable groups' psychosocial wellbeing.

Aim: The purpose of this study was to highlight the impact of the pandemics of the Spanish Flu and COVID-19 on children and adolescents as well as to recognize their short and long-terms effects on children's and adolescents' physical and mental health.

Background: To identify a diagnostic blood transcriptomic signature that distinguishes multisystem inflammatory syndrome in children (MIS-C) from Kawasaki disease (KD), bacterial infections, and viral infections.

Methods: Children presenting with MIS-C to participating hospitals in the United Kingdom and the European Union between April 2020 and April 2021 were prospectively recruited. Whole-blood RNA Sequencing was performed, contrasting the transcriptomes of children with MIS-C (n = 38) to those from children with KD (n = 136), definite bacterial (DB; n = 188) and viral infections (DV; n = 138).
