Article Synopsis
  • The study aimed to investigate why pregnant women with glucose intolerance often have large infants, focusing on amino and fatty acid transport in the placenta, beyond just glucose transport.
  • Researchers analyzed postpartum placental tissues from 101 eligible deliveries, finding higher expression levels of certain transporters (LAT1 and placenta-FABPpm) in women with glucose intolerance compared to those without.
  • The results suggest that, despite controlled blood sugar levels, managing other nutrients like amino acids and fatty acids in glucose-intolerant pregnancies is crucial for fetal growth.

Purpose: To investigate the effect of studying abroad on catch-up vaccination coverage for measles, rubella, mumps, varicella, and tetanus during the pretravel consultation among young adult travelers.

Methods: This retrospective cohort study analyzed data from the Japan Pretravel Consultation Registry (J-PRECOR) on individuals aged 18-21 years with childhood vaccination records. Propensity score weighting was used to estimate the average treatment effect on the proportion of participants receiving catch-up vaccination.

Background: An ambulance traffic crash not only leads to injuries among emergency medical service (EMS) professionals but also injures patients or their companions during transportation. We aimed to describe the incidence of ambulance crashes, seating location, seatbelt use for casualties (ie, both fatal and nonfatal injuries), ambulance safety efforts, and to identify factors affecting the number of ambulance crashes in Japan.

Methods: We conducted a nationwide survey of all fire departments in Japan.

Background: Primary fascia closure is often difficult following an open abdomen (OA). While negative-pressure wound therapy (NPWT) is recommended to enhance successful primary fascia closure, the optimal methods and degree of negative pressure remain unclear. This study aimed to elucidate optimal methods of NPWT as a tentative abdominal closure for OA to achieve primary abdominal fascia closure.

Article Synopsis
  • - This study examined the pretravel consultation (PTC) of older adults (60+) in Japan to understand their vaccination habits and health concerns when traveling abroad, using data from the Japan Pre-travel Consultation Registry (J-PRECOR).
  • - Out of 1000 clients studied, 523 received a yellow fever vaccine, but only 38.6% of those not vaccinated received the tetanus vaccine, and just 25.7% were prescribed malaria prophylaxis for short trips to high-risk areas.
  • - The results indicate a need for increased preventive measures for older travelers, showing that younger age and lower hepatitis A vaccination rates are linked to receiving yellow fever vaccination.

Background: Awareness of pre-travel consultations (PTCs) and prevention methods for overseas travel-related diseases, and the understanding of PTCs among Japanese travelers and medical professionals remains low in Japan. A multicenter registry was established to examine PTCs in Japan. This study assessed the PTC implementation rate and examined the indicators of PTCs that can be used as criteria for evaluating quality.

Article Synopsis
  • The study aimed to establish minimum clinical practice standards for physical therapists in Japan's intensive care units (ICUs) and compare these with international standards.
  • A modified Delphi method involved 54 experienced therapists rating 272 items for their essentiality, leading to 199 items being deemed essential for practice.
  • The resulting standards differed from those in the UK and Australia, highlighting variations due to national laws, cultural, and historical contexts.

Microscopic polyangiitis (MPA) is a type of necrotizing vasculitis associated with high levels of myeloperoxidase-specific antineutrophil cytoplasmic antibody (MPO-ANCA). While generally associated with renal dysfunction, MPA can also cause intraabdominal hemorrhage in rare cases. A 66-year-old man was admitted to our hospital for renal dysfunction, numbness, and weight loss for 3 months.

Background: In laparoscopic gastrointestinal surgery, the location of the tumor is identified mainly with marking methods, such as ink tattooing and intraoperative gastrointestinal endoscopy and marking with a metal clip followed by confirmation with intraoperative x-ray fluoroscopy. Each method has disadvantages, such as complexity, instability of ink sticks, and radiation exposure. Thus, a simple and less-invasive marking method is needed.

Burr hole surgery in the emergency room can be lifesaving for patients with acute subdural hematoma (ASDH). In the first part of this study, a strategy of combined burr hole surgery, a period of intracranial pressure (ICP) monitoring, and then craniotomy was examined for safe and effective treatment of ASDH. Since 2012, 16 patients with severe ASDH with indications for burr hole surgery were admitted to Kenwakai Otemachi Hospital.

Objectives: Given that the optimal antibiotic treatment duration for acute cholangitis with successful biliary drainage remains unknown, this study aimed to validate whether antibiotic treatment duration could be reduced to ≤3 days among patients presenting the same.

Methods: This retrospective study included patients who presented with mild to moderate acute cholangitis due to choledocholithiasis who had undergone successful biliary drainage through endoscopic retrograde cholangiopancreatography (ERCP). After that, 30-day mortality rates and 3-month recurrence rates following short-course antibiotic treatment (SCT, ≤3 days) and long-course antibiotic treatment (LCT, ≥4 days) were compared.

Article Synopsis
  • The study explores hypervirulent Klebsiella pneumoniae (hvKp) infections, which are severe infections occurring mostly in healthy individuals, focusing on cases in healthcare settings across Japan.
  • A total of 140 bloodstream infection cases were analyzed, revealing that 26 (18.6%) were caused by different clones of hvKp, identified through whole-genome sequencing.
  • Patients with hvKp infections showed higher rates of conditions like diabetes and experienced more severe complications such as pneumonia, liver abscess, and disseminated infections, with many cases being healthcare-associated or hospital-acquired.

Purpose: Procedural sedation and analgesia (PSA) is widely performed outside of the operating theater, often in emergency departments (EDs). The practice and safety of PSA in the ED in an aging society such as in Japan have not been well described. We aimed to characterize the practice pattern of PSA including indications, pharmacology and incidence of adverse events (AEs) in Japan.

Foreign body-induced arthritis is a rare etiology of mono-arthritis. It should be suspected in the case of postoperative arthritis, even if some decades have passed since surgery. Histopathology of the tissues is required for a definitive diagnosis, and debridement is essential for complete recovery.

Survival of severe blunt trauma patients treated with resuscitative endovascular balloon occlusion of the aorta compared with propensity score-adjusted untreated patients.

J Trauma Acute Care Surg

April 2015

From the Department of Emergency Medicine, University of New Mexico (T.N., C.C.), Albuquerque, New Mexico and Department of Emergency Medicine, Kenwakai Otemachi Hospital (Y.T.), Kitakyushu, Japan.

Background: Despite a growing call for use of resuscitative endovascular balloon occlusion of the aorta (REBOA) for critically uncontrolled hemorrhagic shock, there is limited evidence of treatment efficacy. We compared the mortality between patients who received a REBOA with those who did not, adjusting for the likelihood of treatment and injury severity, to measure efficacy.

Methods: We analyzed observational prospective data from the Japan Trauma Data Bank (2004-2011) to compare the mortality between adult patients who received a REBOA with those who did not.

Twenty-six Japanese cases of type II enteropathy-associated T-cell lymphoma (EATL) were examined. Multiple tumors throughout the small intestine were found in 15 patients (58%) and duodenal and colonic mucosal lesions in 8 and 6 cases, respectively. Histologically, intramucosal tumor spread and a zone of neoplastic intraepithelial lymphocytes (IELs) neighboring the main transmural tumors were detected in 20 (91%) and 17 (77%) of the 22 cases examined, respectively.
