Article Synopsis
  • The patella, or kneecap, is an important sesamoid bone that helps form the patellofemoral component of the knee joint, which can be surgically rebuilt using total knee arthroplasty (TKA) when damaged by various arthritis types.
  • This study analyzes the morphometry of 168 human patellae, examining various parameters and applying the Wiberg classification to categorize the samples.
  • Significant differences were found in patellar measurements between the left and right sides, as well as among different Wiberg types, highlighting the importance of understanding patellar morphology for surgical procedures and medical evaluations.

Article Synopsis
  • The study follows 46 patients with mild-to-moderate knee osteoarthritis who underwent genicular artery embolisation (GAE) to assess the long-term effectiveness and safety of the treatment over two years.
  • Technical success was noted in 87% of cases, with significant improvements in pain and functional outcomes as measured by the Visual Analogue Scale (VAS) and Knee Injury and Osteoarthritis Outcome Score (KOOS).
  • The results suggest that GAE is a safe and effective option for patients, with no severe complications, warranting further controlled research on the treatment.

Article Synopsis
  • Osteonecrosis of the femoral head (ONFH) can lead to serious complications like femoral head collapse and hip dysfunction if not treated properly, with core decompression being crucial for early cases.
  • A case study of a 34-year-old male patient illustrates that after undergoing core decompression for ONFH, he experienced a subtrochanteric fracture when bearing weight while climbing stairs, leading to further surgical treatment.
  • The report emphasizes that factors such as the drill hole's location, the type of insertion (like tantalum rods), and weight-bearing guidelines can influence fracture risk post-surgery, aiming to guide orthopedic surgeons in managing early ONFH.

  • Avascular necrosis (AVN) causes significant pain and impairments, and this study presents a unique case of an 18-year-old with Crohn's disease experiencing severe AVN without apparent risk factors.
  • The patient was undergoing Azathioprine therapy during remission and developed joint pain in the hips and knees, emphasizing the need for a multidisciplinary approach to manage chronic diseases.
  • It's crucial for healthcare providers to recognize joint issues in Inflammatory Bowel Disease (IBD) patients and consider psychological, gastrointestinal, and other related health conditions at each follow-up appointment.

Article Synopsis
  • The study aimed to identify patient-specific demographic and radiological risk factors associated with subchondral insufficiency fractures of the knee (SIFK) in a large group of patients verified through MRI.
  • Among the 253 patients analyzed, a significant correlation was found between SIFK and medial meniscus tears, particularly in cases with root and radial posterior horn tears, with high rates of meniscus extrusion and notable differences in fracture location based on knee alignment.
  • The findings reveal that factors like higher age, female gender, and higher BMI are linked to SIFK, emphasizing that recognizing these risk factors is essential for timely diagnosis and treatment.

  • Periprosthetic knee fractures (PPKFs) after total knee arthroplasty (TKA) are rare but serious, and this study examines risk factors in patients with osteoarthritis and varying levels of implant constraints.
  • The analysis included data from over 120,000 primary TKA patients in Denmark, revealing that the cumulative risk of PPKFs increases over time, with notable risk factors being age, gender, comorbidities, and prior hip surgeries.
  • Among patients with additional knee disorders or high implant constraints, the risk of PPKFs at 10 years was significantly higher, particularly noted for those with previous fractures or osteonecrosis.

  • Long-term survival rates for patients receiving bone marrow transplants (BMT) have improved, but many experience complications like osteonecrosis and secondary osteoarthritis (OA) due to steroid treatments.
  • This study evaluated the outcomes of total joint arthroplasty (TJA) in BMT patients and compared them to patients who had TJA for primary OA, focusing on various health and surgical factors.
  • Findings showed that while BMT patients had higher preoperative immunosuppression, their perioperative outcomes and two-year survivorship rates were similar to those of OA patients, indicating TJA is a effective option for managing joint issues post-BMT.

Article Synopsis
  • Articular cartilage defects in the hip are common during procedures for femoroacetabular impingement syndrome, with over 20% of patients showing significant chondral damage, leading to increased reoperation rates in severe cases.
  • Various arthroscopic and open surgical techniques have been adapted to address these cartilage defects, including methods like microfracture and osteochondral autograft transfer, which have shown promising outcomes, particularly in younger patients.
  • Despite encouraging results, the effectiveness of some techniques varies, especially in certain patient populations, indicating that more research is needed before broadly implementing open cartilage transplantation, particularly for older individuals where total hip arthroplasty remains a reliable option.

  • Medial open-wedge high tibial osteotomy (OWHTO) is a surgery for knee osteoarthritis and varus deformity, but can result in changes affecting the knee's cartilage and joint function.
  • The study involved 20 patients whose knee cartilage condition was evaluated after surgery, aiming to identify factors associated with worsening of the patellofemoral cartilage.
  • Results showed that 44% of knees had deteriorated cartilage; changes in radiographic measurements, particularly the modified Blumensaat index (ΔMBI), were significant predictors of this deterioration.

Osteonecrosis of the Knee: The Unintended Consequence of Steroid Abuse.

Clin Med Res

March 2024

Department of Orthopaedics, Kalinga Institute of Medical Sciences, KIIT University, Bhubaneshwar, Odisha, India, Pin: 751024.

The anti-inflammatory and immunosuppressive properties of steroids allow their use in a wide variety of rheumatological diseases, asthma, inflammatory bowel disease, cancer therapy, and severe viral infections. Though life-saving or organ-saving, long-term clinical use leads to a vast array of complications. Osteoporosis is the most common orthopedic side effect of steroid abuse, while osteonecrosis is a rare occurrence.

The spacer block technique is widely used in medial fixed-bearing unicompartmental knee arthroplasty (UKA) for osteoarthritis (OA) and osteonecrosis (ON), while it is still unclear if there is any difference in the extension gap between UKAs for the two conditions. To clarify the question, the data from 87 consecutive patients (OA: 57 patients and ON: 30 patients) who underwent medial fixed-bearing UKA with the spacer block technique were retrospectively collected and analyzed. The component gap between the medial tibial osteotomy surface and the femoral trial prosthesis in extension was measured using a UKA tensor, and the preosteotomy gap was calculated in each condition.

Rationale: Chronic inflammatory demyelinating polyneuropathy (CIDP) is an autoimmune disorder that affects the peripheral nerves, leading to weakness and sensory symptoms. CIDP is a rare disease, and few studies have reported on anesthetic management in patients with this condition, especially the peripheral nerve block (PNB). Therefore, a safe and standardized anesthetic approach remains to be established.
